Performs and documents visits to patient/family based on plan of care and patient/family need. Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician. Consults with and provides education to other IDG members on a prn basis for clinical issues relating to palliation. Actively participates in the IDG. Communicates effectively, timely, and appropriately at all times. Informs physicians and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care. Environmental and Working Conditions: Works in patients' homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; the ability to work a flexible schedule; the ability to travel locally; some exposure to unpleasant weather; PRN emergency call. Under the direction of the Chief Medical Officer, and in accordance with CHOT policies and procedures, is responsible and accountable for collaboration with CHOT physician(s), and other care team members, in an active practice to deliver palliative care and hospice related health care services to patients and families. In addition, performance of a face-to-face assessment for assigned patients in consultation with the hospice Medical Director. In collaboration with the IDT (interdisciplinary team), patient and family, provides consultation, assessments, and evaluation of hospice eligibility. Reviews, follows, and/or updates written plan of care in coordination with an interdisciplinary Team (IDT). Assures that patient receives appropriate measures to control symptoms, through collaboration with IDT members Document care provided in the patient's clinical record providing evidence of the progression of the end-stage disease process. The Nurse Practitioner may not certify the terminal illness in the state of Texas. The Nurse Practitioner also serves as an educator and researcher of hospice care. Attends Physicians meetings and other meetings as directed by Medical Director or President/CEO. Establishes, builds and nurtures relationships with staff and community referral sources to facilitate program growth. Maintain current knowledge of current research and trends in hospice care and pain/symptom management. Work in a collaborative team approach with the IDT/IDG. Participate in the QA/PI process as required. Provide consultation & education to colleagues and attending physicians related to admission criteria for hospice and palliative care. Review and develop protocols for treatment and propose options for interventions based on the literature. Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issue. Participate in resolution of interpersonal conflict and issues of clinical and ethical concern. Maintain and enhance professional growth and development through attendance at seminars, conferences, and continuing education offerings. Maintain knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice service. Act as a professional role model by demonstrating behavior consistent with the philosophy, mission and purpose of the organization. Adhere to all Confidentiality policies and procedures. POSITION QUALIFICATIONS Registered Nurse duly licensed to practice as an Advanced Practice Nurse in the state of Texas under the Texas State Board of Nurse Examiners provisions and accreditation from an approved certifying body for advanced practice nursing (American Academy of Nurse Practitioners or American Nurse Credentialing Center). Comprehensive knowledge and well-developed skills in: Medicine and Pharmacology Pain and Symptom control Psychology of loss Understanding and acceptance of Hospice principles of care.
