Overview:
We're looking for a Hospice Nurse Practitioner to join our team.
Nurse Practitioner - NP - Hospice - PRN - Flexible Schedule
- The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
Hourly Rate - $50-$55/hr
Compensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity. You will report directly to the Executive Director or Administrator. You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis. These findings are then communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.
The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
Performs and completes physical assessments for the F2F encounter within the regulatory time frames established by CMS
Provides clinical findings, including review of the medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibility
Makes appropriate recommendations to the IDG based on findings of the F2F assessment
Notifies hospice agency when unable to complete a F2F visit according to schedule
About You:
Master's or higher degree in Nursing.
Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
Current state license as a registered nurse
Current state license or certificate as an advanced practice nurse
Current national certification as a Nurse Practitioner. (ANCC or AANP)
Must have a collaborative agreement in place.

As a Certified Registered Nurse Practitioner at Malvern Treatment Centers, you will report to the Medical Director. The CRNP is responsible for general medical and psychiatric services that reflects new or ongoing medical and psychiatric presentations. Psychiatric problems and needs will be addressed in collaboration with a psychiatrist. Certified Registered Nurse Practitioner uses an understanding of the principles of growth and development to assess each person-served' s specific needs. Your objective is to ensure optimal outcomes in prevention, diagnosis, treatment, and transitions for persons-served at Malvern Treatment Centers using a multidisciplinary approach.
In this role, you will significantly interact with adults struggling with substance abuse addiction. Your key contribution will be to create an organizational culture that promotes high-performing, reputable, and evidence-based behavioral health practices. To excel in this role, you will need strong organizational and communication skills, attention to detail, and a commitment to working collaboratively with other professionals to ensure the best possible outcomes for persons-served and their families.
Summary of Essential Position Functions:
Renders general medical and psychiatric advanced practitioner services, care, and interventions, including but not limited to assessments, evaluations, medication management, and psychotherapy.
Prescribes medically necessary medications and treatment interventions. Orders and refers to all medically necessary services, care, and interventions.
Interacts with physicians and advanced practitioners to improve quality care and improve levels of cooperation with the treatment team.
Formulates a plan of care for person-served, which includes but is not limited to ordering of appropriate diagnostic studies, pharmacological management, and behavioral interventions.
Leads treatment team members to develop, review, and/or approve treatment, safety, continuing support, and transition plans.
Completes and submits accurate encounter, service, and claims documentations within policy time frames.
Ensures quality and timeliness of utilization review activities, including the provision of peer reviews and appeals of denials by insurances and payors.
Collaborate closely with the Leadership Team at our Willow Grove campus.
Participates in preparations for audits, surveys, and inspections by county, state, managed care organizations, and accreditation entities.
Meets all qualification, credential, training, and supervision requirements according to standards of Malvern Treatment Center regulations. Regular attendance and high level of investment in treatment team meetings, in services trainings, and team conferences, etc.
Performs other related functions as assigned.

Working knowledge of generally accepted quality assurance and integrity standards and procedures.
