Order bulk laboratory orders to target specific populations of member.

Participate in community-based “Pop Up Clinics” as way of building relationship with community while addressing gaps in health care
Drive up to 120 miles a day on a regular basis to a variety of locations within the assigned region. There may be drives beyond 120 miles as part of Extended Mileage Special Project days. Special Projects may include an overnight hotel stay.
Obtain and maintain cross state license in other states besides home state based on business need.

Under the direct supervision of the Associate Medical Director/Medical Director, the Primary Care Nurse Practitioner (PCNP) is responsible for conducting initial medical evaluations of new clients in residential, withdrawal management as well as outpatient branches within Protypes Pomona to ensure clients are medically stable to participate fully in Prototypes' recovery plan. Furthermore, PCNP is also expected to identify clients that will benefit from Medication Assisted Treatment (MAT) for substance use disorders such as buprenorphine or naltrexone and prescribe and follow up with clients for ongoing MAT services. The PCNP is responsible for completing a medical history, performing a physical examination, ordering and interpreting laboratory tests, and diagnosing and treating acute common illnesses and injuries within the scope of Incidental Medical Services (IMS). This will include managing curative treatment for uncomplicated Hepatitis C infections. If client requires further ongoing chronic medical management, PCNP is expected to work with residential staff so that clients can be referred to a primary care provider in the community. In the future, this care may be integrated within the Prototypes program in which case the PCNP would follow up with these patients and provide direct primary care. PCNP is also expected to help prescribe bridging medications as needed for clients until they can be connected to a specific primary care provider (30-60 days). PCNP should also feel comfortable providing first line management for patients with uncomplicated stable mental health diagnosis such as depression, anxiety, bipolar disorder or schizophrenia. Appropriate supervision will be available for consultation for management of more complex mental or physical health issues. Essential Functions & Responsibilities
Provider is responsible for rounding and managing the clinical care of the population at the assigned SNF.
The provider is responsible for rounding daily and prescribing medications and treatments for residents residing at SNFs.
Provider will provide high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
Provider is responsible for timely documentation of all patient care/encounters
The provider will be available for phone/secure messaging consultations with facility nursing staff.
Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy™.
PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems.
Provider is expected to be on-site on and on time for their in-person scheduled shifts.
Timely attendance is imperative.
Reliable transportation required.

Shift Structure
Rounding time is 7AM-1PM local time, during which the provider must be available to nursing staff and address acute/emergent issues that arise.
Shifts begin around 7AM local time with flexibility within reason.

As a successful candidate, you will:
Your primary responsibility is to deliver advanced nursing care to patients diagnosed with blood disorders and/or cancer.
Patient Assessment: Conduct comprehensive assessments of patients with hematological or oncological conditions, including reviewing medical histories, performing physical exams, and interpreting diagnostic tests such as blood work, imaging, and biopsies.
Symptom Management: Assist patients in managing the symptoms and side effects of their condition and treatments. Provide education on pain management, nutrition, and coping strategies.
Diagnosis and Treatment: Collaboration with physicians and other interdisciplinary team members to establish and develop personalized treatment plans. Administer hydration, immunotherapy, or blood transfusions, as needed.

Advanced Practice Provider Role:
Performs physical examinations and treats common complications of cancer treatment, counsels patients and families and monitors patient progress.
