PURPOSE AND SCOPE:

Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for assigned patients under the direct supervision of a registered nurse. Participates in the implementation and evaluation of patient care. Responsible for reporting and documenting all new or unusual incidents, information, complaints, or problems to the supervisor. Promotes and assists in the maintenance of a sage and clean work environment.
DUTIES / ACTIVITIES:

Patient Related:
Provide direct patient care for assigned patient(s).
Perform all technical aspects of dialysis procedures for assigned patients as prescribed. Administer and monitor treatments in accordance with current treatment plan or as directed by the supervisor.
Monitor and document dialysis treatment parameters on dialysis flow sheets. Document other information related to the care of the patient in the individual patient record.
Responsible for reporting any significant information, change in patient condition, or equipment problems to the supervisor.
Assist in developing and following the teaching plan to educate the patient and family regarding end stage renal disease, dialysis therapy, diet and medications.
Assist with all emergency operational procedures.
Administer intravenous medication in accordance with physician orders and state nurse practice laws.
Perform and document pre, interim, and post treatment review of patient condition.
Initiate basic CPR measures in the event of cardiac and/or pulmonary arrest, and respond to emergency situations related to dialysis treatment.
Technical:
Safely operate all dialysis related equipment according to the proper procedures.
Provide minor troubleshooting when necessary.
Ensure a clean, safe and sanitary environment in the dialysis facility treatment area.
Ensure all blood spills are immediately addressed according to FMCNA Bloodborne Pathogen Control Policies.
Operate all emergency equipment effectively in accordance to the standard operation procedures.
Staff Related:
Participate in staff meetings as scheduled.
Assist in short term and long term patient care plan meetings.
Acquire information and knowledge in current practice related to dialysis principals and technique by participating in scheduled in-service classes.
Act as a resource person for other staff members. Assist with the coordination of the care plan with Physician, Dietitian and Social Worker.

This position is vital in the health care delivery system. This is traditionally the patient's first clinical point of contact so patient relationship, management of patient care, management of patient information, team communication, and demonstration of conduct and activities using patient centered medical home principles, policies and procedures is critical. Supports the vision and the mission of the Clinic at all times.
ESSENTIAL JOB FUNCTIONS: (with or without accommodations)
Patient relationship: Will greet and welcome the patient in a pleasant, respectful, culturally and linguistically appropriate manner. Assure all patient questions are answered in a positive and cordial manner. Assures the patient has a clear understanding of all communications. Communicates patient's problem/complaint to the provider. Reinforces to the patients the importance of keeping appointments or communicating changes in their appointments. Management of patients care: Prepares patients for examination and treatment. Takes patient's history and vital signs. Team communication: Actively participates in team huddles to assure maximum appointments, gaps in the day, and works on same day access. (PCMH 1A, 1B, 1G) Meets with the team to ensure that the provider's productivity and schedule is reviewed and populated throughout the day. Helps in the orientation of new employees or students. Works with unit team members as well as other clinic personnel to ensure a smooth patient flow. Communicates specific patient needs and/or population management information.

MINIMUM QUALIFICATIONS/CERTIFICATES/LICENSES/REGISTRATIONS REQUIRED:
High School or GED. Must be a licensed vocational nurse in the State of Texas. Experience in working with the public preferred. Must be computer literate. Must be current with BLS/CPR. Bilingual (English/Spanish) a must. This position requires an initial N95 mask fit testing and annual test fitting thereafter unless there is a significant change during the year that requires retesting prior to the annual fit testing.
SPECIAL INSTRUCTIONS: This position requires travel. Employee must provide transportation. If employee operates a personal motor vehicle in the performance of their official duties, the employee must possess a current valid Texas driver's license for the appropriate type of vehicle and Texas Liability insurance. This position requires that an employee's driving record be verified with law enforcement to ensure compliance with SC's driving policy.
