The Transition to Practice Program (TPP) is a 12-month program designed to provide the nurse resident with an opportunity to transition into the role of a professional nurse under the guidance and support of our clinical nursing staff, nurse educators and program facilitators.
The program includes didactic and simulated learning combined with hands-on training in the clinical practice based environment, facilitating the nurse resident's transition from novice to competent nurse. Additionally, the program aims to create a strong social network that fosters a sense of community, support, resiliency building and professional development.
The TPP is offered twice a year to newly-licensed graduate nurses from an accredited nursing program with a passion for children. All new graduates with less than one year of clinical experience are eligible to participate in the program after obtaining licensure.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Medication Administration:
Provides the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
Reviews, reads, notates, and initializes Daily Log to document and learn about pertinent information about residents.
Receives medication updates from Resident Care Director (RCD) or Wellness Nurse.
Administers, assists with, and observes medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
Ensures that medications are passed according to times.
Documents and initials as medications are given and ensures that appropriate documentation is completed for refusal or missed doses.
Maintains confidentiality of all resident information including resident medication among other residents.
Reports all resident concerns made while administering the medication to the RCD or Wellness Nurse.
Restocks medication cart after all medication passes.
Assists in checking medication regardless of packaging system.
Assesses the residents to determine need for "as needed medication" and appropriately documents and reports to supervisor.
Counts all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift.
Maintains and cleans the Medication Room, medication carts and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
Follows re-fill process for medications.
Helps residents maintain independence and promotes dignity and physical safety of each resident adhering to the Sunrise Principles of Service.
Strives to understand and respond to each resident with empathy, always remaining mindful of the resident's unique communication patterns, history, and basic human needs.
Practices routinely good standard care precautions of cleanliness, hygiene, and health .
Audits medication carts.
Resident Care:
Notifies RCD of any resident and/or family concerns through the LEAD process.
Attends and actively participates in daily Cross Over meetings facilitated by the LCM.
Conducts Service and Health Updates as directed by RCD.
Participates in the development of the Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates.
Transcribes orders.
Risk Management and General Safety:
Partners with community team to ensure community is in compliance with OSHA/British Columbia Workers Compensation Act and Occupational Health and Safety Regulation requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations.
Practices safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
Reports all accidents/incidents immediately.
Reports all unsafe and hazardous conditions/equipment immediately.
Ensures any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits and in compliance with fire codes.
Complies with all infection control techniques, placement of bio-hazard containers, removal techniques, procedures, and policies.

Qualifications:

Perform transcutaneous oxygen monitoring testing as ordered and per departmental policy.
Ascertain that the patient's diagnosis is one of the appropriate approved disorders for Hyperbaric Oxygen Therapy, and that the said diagnosis is clearly entered on the patient's charts/records.
Implement the physician's orders as indicated by the patient's treatment plan.
Educate patient and/or patient's family as to the benefits, risks, contraindications, and safety issues of hyperbaric oxygen therapy before treatment.
Perform blood glucose monitoring as ordered and indicated by patient diagnosis.
Inspect the patient prior to the commencement of each Hyperbaric Oxygen Therapy session to ensure all safety standards are being adhered to, i.e. 100% cotton attire, no makeup, no hairspray, etc.
Perform the hyperbaric treatments and monitor the patient during therapy.
Perform dressing changes and/or wound care within scope of practice, as ordered and participate in clinic when HBO volumes are below par.
Take serial photographs of the wound for reporting of the patient's progress.
Evaluate the patient outcome and meet with the manager and the hyperbaric physician to discuss patient treatment and adjust treatment as necessary.
Consult with hyperbaric physician regarding any adverse reactions.
Document treatment notes and maintain patient treatment/diagnostic records.
Complete the patient's treatment charges, if required.
Perform quality control daily on department glucometer, or as required by policy.
Clean and maintain the equipment according to infection control standards.
Maintain continuous quality improvement duties and chart audits.
Answer telephone inquiries and schedule patient treatment times.
Perform marketing duties as needed, intra- and interdepartmental.
Perform an inventory check, and maintain a record of supplies and laundry items.
Maintain equipment and checks backup air and oxygen supplies on a daily basis.
Perform data entry into the Wound Information Data System and collate reports as requested by the director.
Orient new employees as necessary.
Participate in New Patient Reviews to assess for HBO indicators.

Qualifications
To qualify for the position, the candidate must be a graduate of an accredited school of nursing; currently licensed as an RN in the State of Maryland; successful completion of required hospital, nursing and unit orientation, including but not limited to BLS certification and medication certification required. Maintains criteria for and has demonstrated proficiency at the Clinical Nurse I level and meets eligibility requirements for Clinical Nurse II. ACLS required (PACU, ICCU, ED), NALS required (Obstetrical-Nursery) or eligible for completion of course within six (6) months from date in position. Certification in specialty is preferred. Telemetry Nurse must pass a basic arrhythmia exam and have ability to recognize critical rhythms and seek intervention.

PRINCIPAL RESPONSIBILITIES AND DUTIES
CLINIC OPERATIONS:

PATIENT CARE:
Coordinates all aspects of patient care with the appropriate staff members, from admission through discharge of the patient.
Ensure education of the patient regarding quality measures, transplant options, modality awareness, and access care.
Acts as a resource for the patient and family to address concerns and questions.
Accountable for timely completion of patient care assessments and care plans.
Manages timely patient schedules to ensure facility efficiency and develops action plans for missed treatments.

EXPERIENCE AND SKILLS:
Required:
6+ years business operations experience in a healthcare facility.
