Manages individualized, outcome-oriented nursing care through the use of the nursing process. Provides care, treatment and services through the successful coordination and completion of a series of processes that include assessment, planning care, providing care, in accordance with departmental and hospital policies and procedures
1. Medication Administration
a. Demonstrates knowledge of and follows SSH policies and procedures for administering, transcribing, and recording medications.

What you can expect:
In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. Responsibilities
In addition to performing visits and completing coordination of client care, the Registered Nurse RN Home Health is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The Registered Nurse RN consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
Qualifications
Registered Nurses (RNs) must meet the following requirements:
Be currently licensed as a Registered Nurse (RN) in the state of employment
A minimum of one year of clinical experience is preferred
Demonstrate knowledge and skill in current nursing practice
Possess a valid state driver's license and automobile liability insurance

The nurse-patient/family relationship, continuity of care and accountability are central to the delivery of individualized high quality patient care.
A Staff Registered Nurse: Tertiary Care demonstrates the knowledge and skills necessary to provide expected standards of care based on physical, psychosocial, cultural, educational, spiritual, safety and age-appropriate considerations of patients.
The Staff Registered Nurse: Tertiary Care, in accordance with the Nurse Practice Act of Massachusetts Rules & Regulations- 244 CMR 3.0, shall bear full and ultimate responsibility for the quality of nursing care he/she provides to individuals and groups. Lahey Hospital and Medical Center is now offering up to a $5,000 in signing bonuses for our Medical Surgical RN positions. Medical Surgical RN position must be licensed with at least one (1) year of RN experience to be eligible for the signing bonus.
Important Details:
• Signing Bonuses are paid out in three payments. You will receive your first payment (one third of the total amount) upon completion of the first 30 days of employment. You will receive the second payment upon completion of six (6) months and the third payment (final of the total amount) owe upon completion of one year following your start date. The bonus payments will be paid on the next regular pay day following the date on which you become eligible for the bonus. All payments are subject to applicable taxes.
• Full time status at LHMC is considered for the schedules greater than or equal to 30 hours per week; part-time is 20-29 hours per week. Roles less than Full-Time will be prorated. Please note, per diem employees are not eligible for the signing bonus.
To be eligible:
• Current and former BILH candidates - restrictions apply. Employees within the BILH system are not eligible for the bonus or if you have been employed by a BILH entity within the last 12-months.
• If you leave Lahey Hospital & Medical Center before your first anniversary you will pay back the signing bonus.
• Please note, signing bonuses are subject to change based on the organizations hiring needs and will be determined by Talent Acquisition on an ongoing basis. BILH/LHMC Talent Acquisition reserves the right to change signing bonus eligible jobs and amounts at any time.
