1. Medication Administration

PURPOSE AND SCOPE:
The Inpatient RN provides direct patient care in accordance to company policies and procedures which includes FMCNA compliance programs and contracted hospitals' policies and procedures. Areas of practice are diverse, including, but not limited to, hemodialysis, peritoneal dialysis, continuous renal replacement therapies, apheresis, and ultrafiltration. The Inpatient RN will partner with the hospital to provide quality care to the patient and deliver care that provides a positive patient experience.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Under limited supervision, applies considerable knowledge to perform a broad range of tasks which include but are not limited to direct patient care in multiple modalities not limited to hemodialysis, peritoneal dialysis, ultrafiltration, continuous renal replacement therapy, and Apheresis.
Provides supervision and direction of Patient Care Technicians and clerical staff; provides direction and education to ICU staff for CRRT.
Responsible for direct patient care of assigned patients in the Inpatient Renal Replacement and Apheresis Services setting and providing the appropriate treatment technique for assigned patients as prescribed in all modality procedures, as appropriately defined - hemodialysis, peritoneal, continuous renal replacement therapies, ultrafiltration, and Apheresis.
Assesses patients' responses to treatment therapy making appropriate adjustments and modifications to the treatment plan as indicated by the appropriately credentialed physician. Communicate problems or concerns to the Inpatient Program Manager, appropriately credentialed physician, patient hospital primary nurse and others as indicated.
Delivers safe, effective care in a timely an efficient manner.
Gives organized, concise shift reports for patients on assigned shifts. Apply high level thinking skills to shift patient schedules to meet needs of more critical patients due to labs, MD order, or patient assessment.
Responsible for the implementation, administration, monitoring, and documentation of patient's response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier.
Collaborates and communicates with physicians and other members of the healthcare team to interpret, adjust, and coordinate daily patient care plan to ensure continuity of care.
Initiates and communicates to FMS facility nursing staff and appropriate hospital personnel as needed.
Accurately documents all treatment information in the individual patient record (ACES) and the hospital record, as needed.
Cleans and disinfects dialysis machine surface, chair, equipment, and surrounding areas between treatments according to inpatient renal services policies and procedures.
Conducts all tasks necessary for preparation for dialysis treatment and documents where appropriate.
Performs all required pre-treatment dialysis machine alarm testing.
If applicable, initiates Solution Delivery System.
Operates all related equipment appropriately and safely and provides minor trouble shooting when necessary; efficiently utilizes supplies to prevent waste.
Understands, conducts, and documents appropriately Dialysis/Apheresis machine safety tests/alarm tests, equipment calibration, dialysate testing, machine safety tests, functional testing, and internal and external disinfection on all water & dialysis machines, and complies with the documentation/notification standards per FMS policies.
Completes and documents ongoing participation in QAI activities.
Participates in staff meetings as scheduled.
Participates in staff training and orientation as assigned.
Acts as a subject matter expert to hospital staff for dialysis related patient/equipment complications.
Responsible for reporting all new or unusual incidents, information, complaints, or problems to the inpatient manager and other parties per the Adverse Event Report requirements. May be assigned to in-center patient care on an as needed basis
Review and comply with the Code of Business Conduct and all applicable company policies and procedures, local, state and federal laws and regulations.

PURPOSE AND SCOPE:
The Inpatient Services Charge Nurse is an additional “responsibility” assigned to the Inpatient Services RN, for a limited period of time, and for specific additional duties, as assigned by the Program Manager.
The Inpatient Services Charge Nurse ensures appropriate provision of Acute Dialysis Services and treatments within assigned hospitals/facilities by coordinating, scheduling, and directing Acute Patient Care staff and by performing Acute treatments as needed or directed by Program manager. Assumes the provision of quality patient care to all hospitalized patients in the most efficient manner in accordance with company policy, as well as regulations set forth by the company, state and federal agencies.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Under limited supervision, applies considerable complex knowledge to perform a broad range of tasks which include but are not limited to the following:
Acts as a subject matter expert and resource for staff members on assigned Inpatient Center treatment floor.
Normally receives no instruction on routine daily work, and general instructions on newly introduced assignment, but may escalate issues to Program Manager for resolution, as deemed necessary.
Coordinate and schedule direct patient care staff to provide coverage of Acute Dialysis treatments in contracted hospitals/facilities and ensure efficient patient scheduling and staff utilization in accordance with the TAP scheduling tool.
Maintain and review the daily TAP tool for all hospitals/facilities under their direct supervision and submit results to the Program Manager for review.
Ensure all Acute treatments are performed in contracted hospitals/facilities according to the established policies and procedures, and in compliance, with all applicable regulatory requirement. Ensure accurate documentation of information related to patient treatment is entered into ACES during the time of treatment.
Monitor on a daily basis ACES compliance.
Perform Quality Assessment and Improvement (QAI) Program audits as directed to ensure the provision of quality patient care and compliance with all Joint Commission requirements.
Participate in QAI meetings as requested by the Program Manager to assure proper reporting of QAI activities
Assess daily patient care needs and communicate concerns to attending physician. Implement changes in patient care/treatment as directed and in collaboration with prescribing physicians.
Perform Acute treatments according to current
Charge Nurse Guidance
recommendations and oversee all dialysis related medical and emergency functions.
Communicates with the physician and other members of the healthcare team to interpret, adjust, triage and complete patient care plans in the most efficient and safe manner.
Coordinate with appropriate technical staff to ensure preventative maintenance program for routine machine maintenance, to ensure the timely repair of disabled machines, to coordinate infection control measures, routine water culturing, water testing, logging of results, transportation of equipment and supplies, and communicate technical information to the staff.
Serve as liaison between the administrative staff of each hospital/facility and the Program Manager.
Practice cost containment strategies in all areas including medical supply inventory and personnel.
Reviews and comply with the Code of Business Conduct and all applicable company policies and procedures, local, state and federal laws and regulations.
Assist with various projects as assigned by the Program Manager.
Other duties as assigned.

Day-to-day work includes desk and personal computer work, interaction with patients, facility staff and physicians.
The position requires travel between assigned facilities and various locations within the community. (Travel to meetings may be required).
The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials.
The position provides direct patient care that regularly involves heavy lifting and moving of patients, and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. This position requires frequent, prolonged periods of standing and the employee must be able to bend over. The employee may occasionally be required to move with assistance, machines and equipment of up to 200 lbs., and may lift chemical and water solutions of up to 30 lbs. up as high as 5 feet. There is a two-person assist program and “material assist” devices for the heavier items.
SUPERVISION:
Inpatient Services RN's, Inpatient Services PCT's at assigned facilities
EDUCATION:
Current appropriate state licensure.
Current or successful completion of CPR BLS Certification
Must meet the practice requirements in all states employed
EXPERIENCE AND REQUIRED SKILLS:
Minimum of 9 months nursing experience; plus 3 months of specialized experience providing clinical nursing care to dialysis patients in an Acute or Chronic setting required.
6 months dialysis experience in an Acute setting preferred
Demonstrated leadership competencies and skills for the position, including continuous quality improvement, relationship development, results orientation, team building, motivating employees, performance management, and decision making.
Demonstrated management skills necessary to provide leadership in the supervision of dialysis personnel and to ensure the delivery of maximum quality care to all patients..
Excellent communication (written and verbal) interpersonal and customer service skills.

The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. RESPONSIBILITIES & QUALIFICATIONS

Responsibilities:

Medication Administration:
Provides the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
Reviews, reads, notates, and initializes Daily Log to document and learn about pertinent information about residents.
Receives medication updates from Resident Care Director (RCD) or Wellness Nurse.
Administers, assists with, and observes medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
Ensures that medications are passed according to times.
Documents and initials as medications are given and ensures that appropriate documentation is completed for refusal or missed doses.
Maintains confidentiality of all resident information including resident medication among other residents.
Reports all resident concerns made while administering the medication to the RCD or Wellness Nurse.
Restocks medication cart after all medication passes.
Assists in checking medication regardless of packaging system.
Assesses the residents to determine need for "as needed medication" and appropriately documents and reports to supervisor.
Counts all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift.
Maintains and cleans the Medication Room, medication carts and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
Follows re-fill process for medications.
Helps residents maintain independence and promotes dignity and physical safety of each resident adhering to the Sunrise Principles of Service.
Strives to understand and respond to each resident with empathy, always remaining mindful of the resident's unique communication patterns, history, and basic human needs.
Practices routinely good standard care precautions of cleanliness, hygiene, and health .
Audits medication carts.
Resident Care:
Notifies RCD of any resident and/or family concerns through the LEAD process.
Attends and actively participates in daily Cross Over meetings facilitated by the LCM.
Conducts Service and Health Updates as directed by RCD.
Participates in the development of the Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates.
Transcribes orders.
Risk Management and General Safety:
Partners with community team to ensure community is in compliance with OSHA/British Columbia Workers Compensation Act and Occupational Health and Safety Regulation requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations.
Practices safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
Reports all accidents/incidents immediately.
Reports all unsafe and hazardous conditions/equipment immediately.
Ensures any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits and in compliance with fire codes.
Complies with all infection control techniques, placement of bio-hazard containers, removal techniques, procedures, and policies.
Understands and practices the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
Reports occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.
Ensures oxygen tanks are stored safely, exchanges guest/resident's tanks when empty and monitors to make sure liters of oxygen are at prescribed levels.

Maintains compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.
Qualifications:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed throughout this are representative of the knowledge, skills, and abilities required.
LPN, LVN, or state/province specific licensed nurse credential.
In states/provinces where appropriate, must maintain certifications.
Maintain the following certifications and ongoing training and re-education as required by Sunrise and state/provincial regulations:
CPR and First Aid
Must be at least 18 years of age.
Previous experience working with seniors preferred.
Desire to serve and care for seniors.
Ability to make choices and decisions and act in the resident's best interest.
Ability to react and remain calm in difficult situations.
Ability to handle multiple priorities.
Possess written and verbal skills for effective communication and level of understanding.
Demonstrates good judgment, problem solving and decision-making skills.
