Position: RN / Registered Nurse / CVOR / Surgery / 13-week Seasonal Contract / $75 hr
Must be able to show three months proof of a permanent residence 75 miles from NWTHS.
Summary: Provides safe, competent nursing care for patients in accordance with facility policies, standards, and philosophy. Provides professional leadership and support to the healthcare team. The concepts of Patient Centered Care will provide the foundation for all nursing care. Demonstrates Service Excellence standards at all times.
Job Duties/Responsibilities:
Develops, implements, and evaluates an age-appropriate plan of care utilizing the nursing process, clinical skills, and patient education
Maintains professional behavior when dealing with issues or conflicts, utilizing appropriate resources, as needed
Provides an environment conducive to safety for patients, families, visitors, and employees
Assesses risks to patient safety and implement the appropriate precautions
Complies with appropriate and approved standards, as outlined by the organization and accrediting agencies
Complies with safety and injury prevention standards to minimize risk to self, co-workers, patients, and visitors

Qualifications
EDUCATION, TRAINING, EXPERIENCE AND LICENSES / REGISTRATIONS REQUIRED
Graduate of an accredited or NLN-approved RN program
Minimum of 1 year of similar experience required
BLS from the American Heart Association (or obtained during orientation) and maintain certification throughout employment
Current state nursing license or permit to practice nursing in the State of Texas.
De-escalation training within 90 days of hire; does not require maintaining
Ongoing compliance with demonstration of clinically pertinent competencies as determined by the facility.
Stipulations:
· Must be able to show three months proof of a permanent residence 75 miles from NWTHS.
· Traveler/Seasonal RN staff will be required to have a minimum of one (1) year of recent relevant acute care experience on a full-time bases or two (2) years on a part time basis.
· Traveler/Seasonal RN staff are committed to 6 month contracts: both NWTHS and the Traveler/Seasonal RN have a 30 day out, which must be served in writing.
· Traveler/Seasonal RN will be required to be available to work at least 36 hours a week as needed by the hospital. Flexing may be necessary, no more than 12 hours will be flexed in a two-week period.
· Traveler/Seasonal RN must agree to be on call for the division when flexed. Call pay is $2 per hour.
· Traveler/Seasonal RN staff will be required to float to other areas where they are deemed competent to work.
· Traveler/Seasonal RN staff will be required to be available to work a minimum of four full weekend shifts per month as needed by the hospital. Weekends are defined as beginning on Friday at 6:30 p.m. and ending on Sunday 6:30 p.m.
· Traveler/Seasonal RN staff will be required to work one holiday per quarter as assigned by the facility which may include Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day or any other holiday designated by NWTHS if applicable.
· Traveler/Seasonal RN is considered a full-time, temporary status employee.
· A new agreement will be on file with every extension, which must be approved by Senior Leadership prior to the start of a new agreement.
· Traveler/Seasonal RNs may convert to full or part-time status NWTHS employees at any given time, and are eligible for applicable sign on bonuses at the time of conversion.

The Infusion Nurse, Independent Contractor is responsible for providing care to patients that have been assigned to and accepted by her/him including the provision of primary patient care, patient and family education, coordination with AOM Infusion's in house RN staff in the development, implementation, and revision of the patient's plan of care.

Education:

At least one year of experience as a registered nurse preferably in a home infusion setting.
When providing care to pediatric patients, at least one year of experience as a pediatric nurse preferably in a home infusion setting and/or the ability to become competent in pediatric practices associated with pediatric home infusion.
Knowledge and experience in managing infusion access devices such as peripheral lines, PICC lines and central venous catheters including implantable ports.
Free from any communicable diseases, illnesses or injuries that may interfere with the employee's ability to perform tasks required of the position.
Flexible with work schedule. RESPONSIBILITIES
General
The professional nurse employed with AOM Infusion will strictly adhere to providing patient care consistent with the applicable State Nurse Practice Act or title acts relating to reporting and peer review.
Clinical
Assesses and documents the patient's biophysical, psychosocial, and educational needs.
Performs a physical assessment consistent with standards of Nursing Practice and reports any identified problems to the patient's physician and the in-house RN staff.
Cooperates with in house RN staff on the design and implementation of the nursing care plan to be consistent with the physician orders and plan of treatment for the purpose of meeting the patient's needs in conjunction with the patient and caregiver.
Routinely identifies and reports changes in the patient's condition that require a change in the plan of care by nursing documentation and verbal reporting to the in-house RN staff.
Provides comprehensive patient and family education related to the patient's health care needs, prescribed therapy, infection control procedures and disaster planning.
Inserts, maintains and assesses all types of intravenous access devices used for patient care. Reports any changes or issues with the access devices to in house RN staff for the purpose of adjusting supply needs, documentation and tracking of Central Venous Access Devices (CVADs).
Demonstrates competence in performing all treatments per the physician's plan of treatment/plan of care.
Reports need for discharge planning to in house RN staff and documents all needs and changes in the Patient Visit Record and verbally reports as well.
Demonstrates sound clinical judgment and expertise in the planning and decision-making process of the patient's plan of care in cooperation and conjunction with the in-house RN staff.
Operational
Coordinates all patient care needs for an assigned group of patients with other members of the health care team.
Maintains high standards of nursing care and implements policies and procedures as established by AOM Infusion.
Coordinates nursing visits and care consistent with the physician's plan of treatment and the plan of care. Reports necessary changes to the plan of treatment and plan of care as the patient's health care needs require to the in-house RN staff.
Monitors and maintains patient's supply inventory on each visit. Reports any supply needs or overstocking issues to the pharmacy department.
Maintains an inventory of emergency back-up supplies that are available during patient care visits. Emergency supplies include supplies necessary to maintain infection control procedures and also to provide emergency care such as a CPR valved mask.
When assigned an infusion pump, the nurse clinician will clean and test the pump as per AOM policy between each patient use and will document this procedure on the Pump Maintenance Log to be submitted monthly.
Completes all required patient care documentation and submits electronically via iPad supplied by the company at the end of each patient visit and a patient signature must be obtained.
Consistently adheres to universal precautions, aseptic technique and infection control guidelines and includes the need for these techniques in all patient teaching sessions.
Maintains confidentiality of patient information in accordance with HIPAA regulations and AOM Infusion policy.
Quality Assurance
Completes all charting as per AOM documentation policy within the required timeframe.
Reports after each visit and to in house RN staff or RN on call after hours for any adverse events and carefully documents any patient incidents or issues that require the writing of an Unusual Occurrence Report. If the RN has an iPad, he or she must email the report.
Reports any suspected in appropriate behavior or breaches to Federal, State and Local regulatory requirement or to AOM Infusion policies and procedures to the company's Compliance Officer or Compliance Committee.

Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
Communication
Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. Provides and maintains a safe environment for the patient. Assists the patient and family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver. Attends interdisciplinary group meetings.
Additional Duties
Participates in on-call duties as defined by the on-call policy. Ensures that arrangements for equipment and other necessary items and services are available. Supervises ancillary personnel and delegates responsibilities when required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. Fulfills the obligation of requested and/or accepted case assignments. Actively participates in quality assessment performance improvement teams and activities.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
Graduate of National League for Nursing accredited school of nursing.
Minimum of two years of experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
Management experience not required. Responsible for supervising hospice aides.
Registered nurse with current licensure to practice professional nursing in the state.
Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist.
Shows ability to organize and prioritize workload independently.
Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills.
Maintains a current CPR certification.
The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile.
Self-directed and able to work with minimal supervision.
