Requirements
U.S. citizenship is required
Possess a current valid driver's license
Carry and use a firearm. Maintaining firearm proficiency is mandatory.
Be at least age 21 at the time of application and less than age 37 at the time you receive a conditional offer of employment, unless you have previous service in a Federal Civilian Law Enforcement position covered by special law enforcement or firefighter retirement provisions, including early or mandatory retirement. ESSENTIAL FUNCTIONS OF THE CHILD CARE TEACHER:
Implements curriculum within the established guidelines while encouraging curiosity, exploration and problem solving.
Designs and implements daily lesson/activity plans.
Supervise children, classroom, playground and all activities, and on field trips.
Makes ongoing, systematic observations and evaluations of each child.
Provides opportunities and support for children to understand, acquire and use verbal and nonverbal means of communicating.
Conducts parent conferences and maintains positive relationships.

Requirements:
CHILD CARE TEACHER REQUIREMENTS/QUALIFICATIONS:
Preferred Bachelor's or Associates degree in related field or equivalent or CDA or 24 hours of Early Childcare courses.
Have six months of supervised programmatic experience.
Minimum age requirement of 18 years.
Requirements within 30 days of hire include completion of: CPR, First Aid, DHO, Virginia Pre- service Training and Virginia Pre-service Training Update (within a year of original Pre-Service).
Positive role model with strong leadership, conflict resolution, and supervisory responsibilities of all program areas.
A current negative TB test and satisfactory Criminal Record check and Child Protective Services check are required.
Requires strong organizational skills, lesson planning and experience working with children and parents.
Ability to relate effectively with diverse groups of people from all social and economic segments of the community.
Must possess oral, auditory, and written communication skills appropriate for interacting with both children and adults.

Keep the PCP aware of patient condition via e-mail, DASH, HITS or other appropriate means of communication.

Provide high intensity engagement with patient and family.

Provide telephonic or outpatient visits to patients at high risk for readmissions (as identified by CM Plan) to the ER or hospital, to patients with active care planning requirements, and to others as referred via transitional care team, acute case managers and IDT team.
Visits may include evening and weekend hours with the goal of preventing ER visits or hospital admissions.
Perform clinical functions including disease-oriented assessment and monitoring, medication monitoring, health education and self-care instructions in the outpatient setting.
Coordinate the Plan of Care:
Conduct/coordinate initial case management assessment of patients to determine outpatient needs.
Ensure individual plan of care reflects patient needs and services available.
Make recommendations to the team.
Complete individual plan of care with patients and team members.
Communicate instruction and methodologies as appropriate to ensure that the plan is implemented correctly.

Transitional Case Manager (Blended Acute and Community Case Manager Roles)
Responsibilities include but are not limited to:
Onsite patient visitation, risk assessment, and care coordination in the acute and community settings.
Discharge needs assessment and planning.
Assist patient with engaging community resources.
Post discharge telephone calls with medication reconciliation.
Post discharge follow up appointment scheduling.
Home visits with case management assessment including risk and needs assessments.
Ongoing monitoring of high risk patients with select conditions (congestive heart failure, chronic obstructive pulmonary disease, etc.)
Multidisciplinary case conferences.

City/State Norfolk, VA Work Shift First (Days) Sentara Leigh Hospital is hiring an experienced Clinical Nurse Manager (RN) to lead our Medical/Surgical Hematology unit.This is a Full Time, Day Shift position. The Clinical Nurse Manager has overall accountability for the daily operations of one or more clinical areas. Supervises patient care services staff, ensures excellent patient safety and quality outcomes as well as extraordinary patient/family experience. Responsible for interviewing, hiring, developing and retaining staff with the goal of developing a highly reliable team. Demonstrates effective communication and interpersonal skills to achieve unit and corporate goals. Assists in the development and monitoring of unit budget, coordinates purchasing and payroll processes and serves as a clinical resource to team members.

2 years experience in a support leadership RN position, i.e. Unit Coordinator, Team Lead, or other like position may be accepted in lieu of 1 year formal management experience.
Demonstrates knowledge, skills, and abilities related to department operations for systems management, HR management, patient safety, quality outcomes and customer service. Must have excellent communication and interpersonal skills; must possess and demonstrate management and leadership skills.

Sentara Leigh Hospital , located in Norfolk, VA, is a 274-bed acute care facility that opened in 1903 and relocated to its present site in 1977. Our hospital completed a renovation in 2016, including two new patient towers, a beautiful atrium, and a multi-story parking garage. Sentara Leigh Hospital includes a dedicated Orthopedic Hospital providing patients access to a full continuum of orthopedic care, from the preoperative phase and surgery to rehabilitation and home care services. Along with being a recognized accredited Primary Stroke Center, and Magnet hospital for nursing excellence, we also specialize in orthopedic and spine care, heart, vascular, maternity care, and general surgery.
