We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including primary care, hospital, chronic disease, home health. Will work collaboratively with a multi-disciplinary team to create individualized treatment plans for patients; embrace and promote a patient-centric model of care. The Nurse Practitioner will have the following responsibilities: Performing comprehensive in-home assessments for identified patients of the practice and assisting the primary care team in better managing chronic diseases Reviewing current symptoms and identifying diagnosis to be used in care management and active medical management of treatment Completing physical examinations, medication reviews, vital checks, lab draws, wound care and depression screenings Communicating assessment findings that will be used to inform the PCP of potential gaps in care Accuracy on risk capture, high quality metrics and closure rates High patient satisfaction and improved health status including reduced hospitalization and ER visits Thinking critically to address patient needs, providing the appropriate resources and follow-up care to improve stability Educating patients on topics such as disease process, medication, and compliance Complying with all HIPAA regulations and maintaining security of protected health information Ability to travel 5 days a week to provide care to patients in their home based on the needs of business/market and/or in a clinic setting in a residential care facility. Required Qualifications Nurse practitioners must have completed accredited education and training for nursing and nurse practitioners. An active unrestricted medical license in the State of New York is required. At least 3 years of active practice required. DEA Certification required. Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner. Ability to build and effectively manage relationships with patients, the community business leaders, and external constituents.
