Conduct face-to-face presentations in our sales center located at our Poconos Mountain Villas resort. Your Role as an Advanced Practitioner (AP):
As the core provider on the AHCaH team, you'll work closely with patients and collaborate with a dynamic interdisciplinary care team under physician guidance. Key responsibilities include:
Conducting in-person and remote patient assessments
Developing and implementing treatment plans
Providing ongoing monitoring and coordination with field staff
Team Composition:
Advanced Practitioner (AP): Primary provider managing patient care
RN Coordinator: Care coordination and program oversight
Field Staff: Mobile paramedics and RNs for in-home care delivery
Physician: Direct care and interdisciplinary team supervision
Location:
Based at the Geisinger Healthplex CenterPoint office in Pittston, PA, with limited travel to Geisinger Wyoming Valley, Geisinger South Wilkes-Barre, in Wilkes-Barre and Geisinger Community Medical Center, Scranton as needed.
Schedule:
