A DAY IN THE LIFE OF A CARE COORDINATOR
Creates a welcoming and professional environment for our patients and visitors by demonstrating exceptional customer service based upon expected service behaviors.
Monitors and collects urine specimens for drug testing.
Collects specimens of hair and saliva for drug collection and processing.
Performs Breath Alcohol Analysis as a trained B.A.T. (Breath Alcohol Technician).
Act as liaison with other review-related and discharge planning related functions and communicates with departments as necessary to ensure coordination of related processes and functions.
Identify and address any unmet needs of patients.
Monitor and coordinate the delivery of necessary assessments (psychiatric, medical, education, vocational, health, etc.) in a timely manner to ensure that client's service needs are adequately identified and a plan to access resources to meet the need timely is developed.
Assist in the coordination and development of the client's Behavioral Health Service Plan (Individualized Service Plan ISP). The plan shall include services and assistance necessary to achieve specific objectives as well as an explanation of the availability, intensity and duration required of each service. Review and update the BH Service Plan as required by Meridian HealthCare policy, OMHAS standards and as additional needs and goals are identified.
Interviews clients, coordinates assessments, and prepares a narrative summary including problem areas, client strengths, treatment recommendations and diagnosis.
Provides client access to individualized treatment to ensure client rights are upheld, to facilitate client's active participation in treatment planning whenever possible, and to inform clients of alternative treatment services and procedures that are available.
Develops treatment plans and reviews, prepares discharge summary and aftercare plan as per agency procedure.
Meets productivity goal of a minimum of 50% (1,040 direct service hours annually).
Makes referrals and provides necessary case management consultation, correspondence and advocacy to optimize client's accessibility to needed services that are not provided by Meridian HealthCare.
Maintains confidentiality standards with regard to client service records and client identifying information according to state, federal and agency guidelines.
Assures accurate documentation of services through:
Development and implementation of individual treatment plan.
Chronological updates of all treatment notes and correspondence.
Description of response to treatment interventions and resulting behavior changes by client and significant others.
Aftercare planning and discharge recommendations including follow-up and re-admission criteria.
QUALIFICATIONS FOR A CARE COORDINATOR
High School diploma and CDCA required or eligible. Competency or training in chemical dependency treatment preferred.

Position Summary:
As a senior member of the Diagnostic Imaging Department, this position produces clinical diagnostic radiographic images, CT Scans and/or Mammographic Procedures. Position requires certification in 2 or more of the 3 modalities mentioned. Performs routine diagnostic imaging, CT exams and/or Mammographic procedures according to departmental protocol, policy and procedures and established standards of practice. Operates diagnostic imaging equipment, prepares room, equipment, supplies and medications. Provides leadership to Rad Tech II technologists or other staff as is needed. Provides patient care services using general and specialized imaging modalities under the direction of a physician but not requiring constant technical supervision. Provides diagnostic imaging services to patients of all ages. The Rad Tech III will take a supervisory role where designated or in the absence of supervisory or management staff. Applies ionizing radiation in the performance of a variety of technical procedures for radiological diagnosis. Responsible for patient care safety. Assists physicians in the performance of interventional procedures and the administration of contrast media. Assumes responsibility for designated procedures and equipment. Participates in orientation of Radiology Department staff. Qualifications
Minimum Education:
Graduate of an accredited radiology program
Minimum Experience/Skills:

Current Basic Life Support (BLS) certification for Healthcare Provider issued by the American Heart Association

Job Responsibilities:
Under the direction of the Radiation Oncologist and Medical Physicist produces an optimal external beam treatment plan (IMRT/VMAT, 3D) using Eclipse and/or Ethos TPS (adaptive or non-adaptive planning)
Guided by the ALARA principles and having patient's best interest in mind participates actively in the decision process for the selection of the best patient treatment course
Performs rigid and deformable fusions of multiple image datasets.
Accurately identifies and contours Organs at Risk on different modality datasets (CT, MR, PET)
Assist with HDR procedures when needed
Generally supervises each initial treatment set up to assure compliance with treatment plan and prescription
Designs custom cut-outs for electron treatment and places orders using dot. Decimal software
Assists the physicist in generating QA plans and calculations.

Work experience in all aspects of respiratory care preferred.
Competency in blood gas laboratory, maintenance, PFT and bronchoscopy assistance a plus.
Registration by the NBRC as a Registered Respiratory Therapist (RRT) preferred.
