Position Summary
Aspire Indiana Health's FQHC is looking for someone who is excited to be a part of their patient care team in an integrative healthcare setting. The Behavioral Health Consultant (BHC) will work in a fast paced environment and provide comprehensive care to medical and psychiatric patients, assisting patients to improve their overall health and wellness in a primary care/psychiatric setting. Unlike traditional therapy treatment, the BHC provides the choice to the patient on whether they would like to follow up on the goals they set at their next provider appointment, schedule a follow up directly with the BHC or access the service if and when they feel they need it again
This dynamic clinical position is full-time and involves “in-the-moment” collaboration with the FQHC patient care team members made up of medical and psychiatric providers, medical assistants, nurses and administrative staff. The BHC role is primarily onsite, allowing for collaboration with team members and the ability to immediately engage face to face with patients through warm hand offs. * Provide short-term crisis intervention as needed/identified. Will follow intensive, evidence-based service models based on Family Wellness and other Trauma Informed approaches. Establish mutually respectful rapport and implement interventions necessary to meet needs of client/collaterals; appropriately set limits and maintain helping role as a Mental Health practitioner.

* Prepare all required clinical documentation per established time frames involving all appropriate parties as required by policy and regulations of the state/ program/ contract/ SAFY policy and COA standards.
* Compliance/Reporting:
* Timely documentation as required by policy, regulations, and contract requirements. Prepare and submit timely all required company, RFP, program, referral agency and regulatory reports, progress notes, and billing tickets.
* Maintain contact with referral agency personnel using discharge reports, Diagnostic Assessment forms, telephone calls, and incident reports in addition to regularly scheduled and required written reports.
* Must be in compliance with monthly and quarterly requirements as defined annually by SAFY QI/CQI standards. Meet state required utilization standards for Mental Health services.
* Other:
* Establish and maintain a positive working relationship with all levels of SAFY staff by participating in joint meetings, and problem identification and resolution.
