The medication history technician position is a full-time position that requires a 7 day-on, 7 day-off schedule and the flexibility to work until at least 9 pm each shift. Under the supervision of the Pharmacy Director, this position obtains medication histories for new patients admitted to hospital. These histories are obtained directly through patient interview or other sources such as a family member, retail pharmacy, physician office or another healthcare facility. The technician is required to enter information into an electronic patient database and communicate the completed medication history with the care team. In addition to taking medication histories, the technician must be able to perform all duties of an inpatient Pharmacy Technician.
Requirements
High school diploma or equivalent.
Licensed Pharmacy Technician in the Commonwealth of Virginia.
National Certification preferred
Requires a minimum of 2 years Pharmacy Technician experience
Essential Job Responsibilities
Weekend and holiday shifts are required. This position requires commitment to a seven day-on, seven day-off schedule and the flexibility to work until 9 or 10 pm each shift.
Coordinate medication history assessment with provider and nursing teams.
Perform medication history assessments for new admissions primarily in the Emergency Department but also in other units of the hospital.
Collect a medication history directly through a patient interview or interview of the patient's family or other care giver.
Using the information available, provide complete information for each medication identified (i.e., drug, dose, route, frequency, indication for PRN, and last dose taken-date/time).
Verify patient allergy information and ensure complete documentation (i.e., substance, allergy category, reaction description).
Participate in performance improvement activities related to medication reconciliation.
Maintain an excellent knowledge of brand and generic medication names, common dosages and dosage forms and frequency of administration of each medication through self-study.
Precept Pharmacy Technicians, Pharmacy Students, Pharmacists, and Nurses regarding home medication interviews and documentation.
In addition to these specialized responsibilities, the medication history pharmacy technician will also be able to function as a pharmacy technician in the inpatient pharmacy.
Other duties as assigned by their direct supervisor or the Director of Pharmacy.
