The Queens Community Justice Center seeks a Senior Case Manager for its Focus Forward program. Focus Forward provides employment-focused services as an alternative to incarceration or as an addition to a probation mandate. Focus Forward serves young adults ages 18 to 35 with moderate to high employment needs. Focus Forward will deliver evidence-based programming to assist participants in obtaining employment and provide them with case management support to ensure they can maintain employment and work towards a long-term career. Reporting to the Associate Director, Adult Operations, the Senior Case Manager will provide direct participant support and increase the range of employment opportunities available to participants using participant interests, experiences, and ambitions as the foundation for participant job placements and, more broadly, for the identification and cultivation of potential employers for prospect portfolios. Conducts facility safety checks as assigned to visually assess clients for the use of alcohol/drugs or the environment of care for unsafe conditions. Ensures the maintenance of accurate, complete, timely and high-quality client records that comply with external regulatory standards and agency policy and procedure. Complies with 42 CFR confidentiality and HIPPAA privacy and security regulations. Works cooperatively with other staff as a member of the interdisciplinary team. Functions as a liaison/advocate to client's families and other agencies as needed. Support patients through education and training - Teach patients and caregivers how to manage lymphedema at home, including self-care strategies, exercises, and lifestyle modifications for long-term symptom management. Develop home exercise programs - Tailor movement and strengthening exercises to each patient's needs, ensuring they maintain functional abilities and prevent complications. Ensure accurate and thorough documentation - Maintain detailed patient records, including assessment findings, treatment progress, and any modifications to care plans, in compliance with regulatory and reimbursement requirements. Advocate and coordinate care for patients - Help patients access the services, resources, and financial assistance they need, ensuring equitable care for all individuals with lymphedema. Collaborate with healthcare providers and stakeholders - Work closely with physicians, specialists, and team members to develop cohesive, interdisciplinary care plans that address the full spectrum of patient needs. Partner with vendors for compression therapy solutions - Assist patients in obtaining appropriate compression garments by navigating insurance coverage, securing financial funding resources, and ensuring the best fit for their needs.
