As part of the primary care treatment team, the Behavioral Health Therapist (BHT) identifies, assesses, triages , documents and manages primary care patients with medical and behavioral health problems. In addition, this BHT will utilize skill training through psycho-education and patient education strategies to develop patient self-management, population health programs, and will develop specific behavioral change plans for patients using best practice and brief evidence based treatment interventions. The BHT is a core member of the collaborative team, including the patient's medical provider and the larger primary care team. The BHT is responsible for supporting and coordinating the mental and physical health care of patients on an assigned caseload, while also providing crisis intervention and short-term consultation and support to the primary care team for patients in urgent need of same-day contact.
Responsibilities
(30%)* Assesses the clinical status of patients referred by the primary care provider. Assists in the detection of "at risk" patients and intervenes to attenuate further psychological or physical deterioration. Assists primary care providers in recognizing and treating mental disorders and psychological problems. Consults with the physician as necessary, and refers cases to other behavioral health providers for longer-term care as appropriate. Facilitates and ensures patient engagement and follow-up care. Works with primary care team to treat and manage patients with chronic emotional and/or physical health problems efficiently and effectively. Provides brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate. Assists in preventing relapse in conditions that tend to recur over time.
(30%)* Uses brief assessment tools with patients to assist in identifying concerns, triaging , and outcomes measurement. Systematically documents treatment response and monitors patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications. Improves clinical outcomes with high prevalence medical and mental health conditions. Connects with new and established patients regarding integrated care model and care team information. Evaluates patient care plans with primary care team and develops appropriate goals for behavioral health conditions. Performs coding/billing following appropriate guidelines.
(20%)* Assist in program development (e.g., pain, diabetes, weight loss, insomnia, stress, etc.) Obtains and keeps available patient education materials about common mental health and substance use disorders. Provides and updates local available treatment options and education/resources for patients and their family.
(20%)* Tracks patient follow up and clinical outcomes using a registry. Documents in-person and telephone encounters in the registry and uses the system to identify and re-engage patients. Documents patient progress and treatment recommendations in the Electronic Health Record ( EHR ) and other required systems so as to be shared with medical providers, psychiatric consultants and other treating professionals.

Eliminate barriers to treatment, increase patient/family satisfaction, and improve appropriate utilization of resources.
Facilitate referrals for clinically indicated services outside of the health system (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Assist in obtaining financial and other resources for patients and families in need.
Identify the need for and conduct family meetings, with or without the physician, that result in understanding, comfort, decision-making, and other important outcomes.
Integrate relevant theories of family dynamics, crisis intervention, strengths based, solution focused, and meaning of illness into everyday casework.
Summarize a patient and family situation concisely, with helpful guidance to non-behavioral health staff members about recommendations for actions to be taken and barriers to wellness.
Actively participate in scheduled caseload consultations with psychiatrists or other specialties and communicates recommendations to the patient's medical provider or primary care team.
Consultations may focus on patients new to the caseload and those who are not improving as expected under the current treatment plan.
Accepts accountability for the clinical outcomes.
Utilizes evidence-based scales to screen patients and then uses the data to guide treatment and follow-up.
Maintains timely, clear and concise documentation in all required systems.
Provides pertinent clinical data to designated outside agencies to assure compliance with their requirements.
Assures compliance with regulatory requirements and acts a resource/liaison to physicians and other staff regarding all aspects of Integrated Care Professional activities.
Actively participates in practice provider meetings, team huddles, and quality assurance activities as scheduled. Provides education/training to practice staff when requested.
Builds professional relationships with patient, families, facilities and resources to address conflict resolution with positive outcomes.
Completes formal outreach and training to other providers including community education as requested.
Provides de-escalation and crisis intervention, and Identifies high risk patients and addresses opportunities for intervention.
Willingly and consistently offer to help peers as needed. Communicates teamwork, caring and compassion.
Sustains productivity expectations established by leadership team.
Provide excellent customer service with internal and external customers.
Assume responsibility for professional development and education requirements for maintenance of professional licensure .
Provides for the age specific needs of the population served according to department standards and policies/procedures.

Certifications and Licensures
MSW - Masters of Social Work
Preferred Equivalent Experience - Licensure as a Licensed Psychologist or Master's Social Worker in the State of Michigan. Limited License employees must be actively working to obtain full licensure, and obtain it within one year of employment.
PCSW - Psychologist/Counselor/Social Worker

EXPERIENCE, TRAINING AND SKILLS:
Membership in at least one professional organization (i.e. APA, NASW, CFHA, MPA, SSWLHC-MI Chapter, national SSWLHC, etc.) preferred.
Training and experience, and/or demonstrated competency in casework principles, including diagnosis, assessment, crisis intervention, treatment and transition planning; techniques in individual, family and group therapy; awareness of community resources, public assistance and entitlement programs, ensuring transition to appropriate levels of care; and issues relating to age and stage of development, special needs and cultural patterns of the patient populations being served.
Knowledge of Integrated Behavioral Health practices, processes and procedures as normally acquired in a primary care setting (preferred), hospital, social agency or community organization dealing with physical and/or mental health and/or welfare.
Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner.
Adherence to the American Psychological Association or National Association of Social Workers' Code of Ethics.

Position Summary
The Therapist is responsible for providing individual and group therapy sessions for clients in a specialized Outpatient facility. Position is responsible for assessing the therapeutic needs of the individuals and families, and for developing appropriate treatment. Therapists act as the primary contact for clients and families throughout the treatment process and serve as a client advocate with the treatment team. Position acts as a liaison with referral sources, clinical partners, and individuals to coordinate care throughout the treatment process.

Demonstrates a high level of clinical skill in assessing the clinical needs of clients, determining appropriate interventions, and conducting individual and group therapy sessions in accordance with these needs.
Completes psychosocial evaluations, treatment plans, contact logs, discharge plans and other reports required for each client in compliance with State, accrediting bodies, quality guidelines and payor source guidelines.
Conducts weekly individual and family therapy sessions for assigned caseload.
Monitors behavioral interventions for each client and provides skills coaching as needed.
Conducts specialized groups as assigned based on education, training, and experience; supervises or co-leads groups with staff or interns when appropriate.
Selects appropriate interventions to meet the clinical needs of individual clients.
Maintains therapeutic relationship with clients using praise and other reinforcers to encourage appropriate behaviors and attitudes.
Sets limits and hold boundaries in a kind and firm manner.
Demonstrates effective verbal crisis intervention and de-escalation, involving other staff as needed.
Collaborates with an interdisciplinary team to coordinate client care from admission through discharge.
Attends weekly clinical, consultation and business meetings; present cases to be discussed as pre-assigned.
Completes documentation in a timely and accurate manner reflecting ongoing assessment of client's clinical presentation and behaviors.
Communicates client progress with the referral source, family, parent, or guardian informed of treatment goals and plans, as appropriate.
Documents in accordance with policies and procedures services provided, incident reports, shift reports, or department reports in a timely and accurate manner.

Education and Experience
Position requires a master's degree in social work, marriage and family therapy or psychology. Must hold or be working toward MI licensure (LMSW, LPC, LMFT, or LMHC). Position requires a minimum of one (1) year experience in a behavioral healthcare setting.

The TRC has an opening for an experienced, full-time licensed Psychotherapist. Your clinical practice will include providing trauma-informed intake assessments and psychotherapy to survivors of violent crime.
Opportunity Highlights:
* Conducts clinical intakes of new clients using a trauma-informed approach and identifies service needs when appropriate. Assessment includes identifying presenting problems, psychosocial history, mental status exam, and DSM-V-TR diagnosis.
* Caseload consists of complex clients, all of whom have histories of trauma, including interpersonal violence such as sexual assault, physical assault, domestic violence, gunshot wounds, political torture, and immigration trauma. **$5,000 Sign on Bonus available to external candidates** As part of the behavioral health telehub team, the Behavioral Health Therapist (BHT) identifies, assesses, triages, documents and manages patients who are in crisis and/or need follow up care due to medical and behavioral health problems. The BHT will utilize safety planning and suicide management, along with skill training through psycho-education and patient education strategies to develop patient self-management, and will develop specific behavioral change plans for patients using best practice and brief evidence based treatment interventions. The BHT is a core member of the collaborative team, including the patient's emergency department or inpatient medical providers, psychiatrists, Community Mental Health staff, as well as the primary care teams. The BHT is responsible for supporting and coordinating the mental and physical health care of patients in the ED and on an assigned caseload, while also providing crisis intervention and short-term therapy for patients in urgent need of same day contact.
Responsibilities
(30%) * Assesses the clinical status of patients referred by ED providers, inpatient providers and primary care providers due to mental health crisis. Assists in the detection of 'at risk patients' and intervenes to attenuate further psychological or physical deterioration. Provides de-escalation and crisis intervention, and addresses opportunities for intervention. Assists providers in decision making regarding appropriate levels of care. Consults with the physician as necessary and refers cases to other behavioral health providers for longer term care as appropriate. Works with primary care team to treat and manage patients with chronic emotional and/or physical health problems efficiently and effectively. The Direct Liaison for Behavioral Health (DLBH) is a partnership between Pine Rest Christian Mental Health and local schools. The DLBH will seek to improve communication and coordination for the mental health and behavioral health needs of students and families in the district. The DLBH will contribute to student success and wellbeing by bringing an experienced and educated clinical perspective to the academic domain. The DLBH position is year-round and requires flexibility and adaptability regarding schedule and location demands. The Direct Liaison for Behavioral Health is further divided into two categories: therapist and liaison. Our Direct Liaison for Behavorial Health are dedicated to excellent customer service, enthusiastic about the care we offer, and foster a sense of belonging and empowerment in a diverse workplace.
Principal Duties and Responsibilities:
Participates in the interviewing of persons served and assessing them through mental status as well as reviewing records obtained from other patient care staff.

Knowledge of Integrated Behavioral Health practices, processes and procedures as normally acquired in a primary care setting (preferred), hospital, social agency or community organization dealing with physical and/or mental health and/or welfare.
Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner.
Adherence to the American Psychological Association or National Association of Social Workers' Code of Ethics.

As a Hospice Medical Social Worker, here's a big-picture view of what you'll do:
Provide home-based social services to patients dealing with end-of-life issues including care management, pain, symptoms associated with chronic and terminal illnesses, depression, anxiety, poor nutrition or a lack of resources that make managing their journey difficult
Work as part of a hospice team which will include an RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT, OT or SLP, focused on the patient's plan of care and individual goals
Monitor a patient's psychosocial condition and identify social and emotional needs
Conduct patient assessments, document progress and ensure patient is moving toward goals
Provide counseling, community resource planning, crisis intervention and advocacy
Consult with family and caregivers on patient's plan of care and how to help them progress
A few must-haves for Hospice Medical Social Workers:
Graduate of a Bachelor's Degree program in social work accredited by the Council on
Social Work Education.
Licensed to practice as a Social Worker in the state in which employed. The license is active and in good standing.
Hospice experience preferred
