Minimum Qualifications
Any combination equivalent to: Associate degree and two years of increasingly responsible program planning experience, including experience working with students in an academic environment.

* Setting up equipment, weigh and mix ingredients, operate equipment, collect samples and data, tear down and clean equipment according to GMPs established for the food industry
* Executing experiments and/or preparation of prototypes in the Lab/Pilot Plant under the direction of R&D Scientists/Engineers that involve testing ingredient substitutions, formula changes for cost savings, process variables, etc.
* Proficiently operate, clean, and maintain lab/pilot plant facilities and equipment for food product and process development activities (appropriate training will be provided)
* Collect, organize, and maintain experimental and production samples/data
* Collect samples and/or data during experiments or production trials as directed by R&D Scientists/Engineers

The Internship I position is meant to provide current freshman and sophomore undergraduate students an introduction to work through related experiences in their degree interests. The position is paid and is typically a ten-to-twelve-week experience in the months of May-August. Requests for college credit are supported.
ESSENTIAL JOB FUNCTIONS:
Job duties will be dependent on individual department needs. Work may require teamwork and independent work. Job duties will provide experience and opportunities to learn how to navigate in a professional organization, provide opportunity to build and leverage problem solving skills, apply educational knowledge, contribute to real business needs, and have an opportunity to develop soft skills in support of professional development. Travel may be required, depending on the department's needs.

PHYSICAL REQUIREMENTS:
Varies by position and may include the ability to frequently move about inside the office, food processing facilities and/or farming fields. Ability to operate a computer and other office productivity equipment. Occasionally, must be able to remain in a stationary position throughout much of the working day. Ability to work outdoors. Must be able to work in different temperatures which range between 30° F and 110° F. Ability to inspect equipment and product. Must be able to work in a high noise area.
STARTING WAGE: $17.50 - $19.00 (dependent on year in college)

We're looking for someone to join our team as an ECM- Care Coordinator who:
The Care Coordinator will report to the Practice Manager. Care Coordination allows primary care physicians to use dedicated time to direct proactive care for their patients, uses staff support to conduct outreach, and leverages new panel-based information technology tools.
Essential Functions :
Meet with all new patients, explaining PCP's, Patient Portal and all aspects to accessing care.
Assign patients to provider panels ensuring balance.
Receives monthly panel report and reviews PCP assignments.
Determines continuity percentages for each provider - assure that majority of visits with PCP
Resolves unassigned patients by reviewing appointment history (and possibly the clinical record) to determine appropriate assignment.
Collaborates with appropriate site.
communication with outside provider to ensure continuity.
Proactively engage priority patients to promote availability of expanded access clinic and reduce unnecessary Emergency Room utilization.
Run, manage and analyze standard CSV reports.
Oversee and analyze data from assigned panels in regard to CSV-priority conditions. This includes the running of reports within the CSV computer structure, Excel etc.
Responsible for clinic-wide compliance with CSV, PCMH, CMS, Meaningful Use and California Department of Public Health (CDPH) requirements.
Clinic-wide required to meet or show consistent improvement on CSV clinical quality goals.
You'll be successful with the following qualifications:
Education: Medical Assistant certification or program completion required.
Computer proficiency: Excel, Word, Outlook, PDF, Electronic Health Records, etc.
Bilingual (Spanish-English) preferred.
Maintain excellent internal and external customer service at all times.
Maintain the highest degree of confidentiality possible when performing the functions of this department.
Possess the tact necessary to deal effectively with patients, providers, and employees, while maintaining confidentiality.
Must be able to work independently, handling high volume and multiple tasks.
Must be reliable with attendance.
Must be highly organized and detail oriented.
Possess knowledge of modern office equipment, systems and procedures.
