Details: Our New Grad Program is run on an annual basis for 4-6 providers, training over 4 months, running from June 2025 through the end of October/November 2025 working full time with hours divided between time in center and didactic time. Attendance of important new grad program related and Go Health related meetings is mandatory which includes but is not limited to didactics, skills lab, and monthly Town Halls. The clinician will document all clinical interventions in accordance with CHC policy, Joint Commission standards, and state regulatory expectations. The position is primarily focused on direct client care but may include supervision of unlicensed staff and/or students. This School Based Health Clinician is a full time salaried position that follows a school based calendar, mirroring the school schedule of 195 days per year. ROLE AND RESPONSIBILITIES CLINICAL Completes initial behavioral health evaluations; individual, group and family psychotherapy with children and/or adults in an outpatient clinic. Will be expected to run at least five (5) groups for each week of direct clinical work. Practices as part of an interdisciplinary, inter-professional team, accepting “warm hand offs” from primary care providers and other members of the health care team, as well as reaching in to the patient population to identify patients potentially in need of Behavioral Health services as flagged by the BH clinical dashboard. Location: Bridgeport and Norwalk, CT Area Position Summary: As an IFP (Intensive Family Preservation) Worker, you will provide intensive, strength-based support in the families homes. Responsibilities include offering case management, parent education, advocacy, and clinical support, while collaborating closely with DCF. You will assess family needs, develop service plans, deliver goal-directed services, and connect families with community resources. Common family challenges include: Substance use Domestic violence Mental health concerns Poverty Cultural conflicts Parenting limitations Key Responsibilities: Maintain a caseload of up to six (6) IFP cases. Conduct strength-based assessments to address safety risks and prevent out-of-home placements. Deliver concrete and clinical interventions, skills training, and community resource referrals. Use approved assessment tools and a solution-focused approach. Attend weekly supervision meetings and bi-monthly Family Support team meetings. Maintain timely and confidential electronic client records, adhering to documentation guidelines. Collaborate with DCF through verbal and written updates and participate in monthly case reviews. Assist with outreach efforts, such as presentations and mailings to referral sources. Provide transportation for clients when aligned with treatment goals. Participate in a 24/7 on-call rotation, offering crisis intervention for IFP and RTFT clients. Qualifications: Education: Master's degree in social work, social sciences, or education (or a bachelor's degree with relevant experience). Experience: Counseling, crisis intervention, and working with minority populations. Experience working with diverse populations. Additional Requirements: Valid driver's license and access to a personal vehicle for regional and statewide travel. Awareness of cultural and socioeconomic characteristics of clients. Ability to provide home-based supports, maintain professionalism, and work independently in challenging conditions. Why Join Us?
