Our Intake and Referral department is the heart of Home Health & Hospice operations, and we want you on our team! We have an open position for a registered nurse who is interested in supporting an exceptional patient experience, every time. Our registered nurses and team assistants work together to make certain we have all required information and documentation for a complete referral, and coordinate with referring providers and internal directors and managers to support patient access to our services. This position offers an opportunity for a registered nurse to use their clinical experience to evaluate patient eligibility for our services and support patients, their families, community members and referring providers in understanding the breadth and depth of the services we offer.
Schedule is every other Fri/Sat/Sun, and then 3 week days.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Connects with patients via phone or video call. Interviews and questions patients to collect health history and uses a computer system to record and store comprehensive and focused data relating to the health needs of patients and families.
Provides health assessment and treatment solutions, monitors patient health and at-home care, aids in emergency scenarios and promotes patient wellness.
Based on technology available, monitors a patient's blood oxygen levels, heart rate, respirations, blood pressure and blood glucose as well as other assessment measures.
With the help of video chatting, identifies patient's symptoms and conditions. Analyzes data to determine the appropriate health maintenance and identify appropriate outcomes for patient and family.
Collaborate with on-call providers as needed to support expected clinical outcomes for the patient and family.
Evaluates and documents progress toward the anticipated outcome. Assist in ensuring achievement of optimal patient outcomes using Telemedicine. Documents interventions in a readable, understandable language.
Aids in enhancing the quality and efficacy of the organization's telehealth practices and professional nursing practice through successful utilization and improvement of outcomes that demonstrate program efficacy.
Utilizes appropriate resources to plan and provide services that are safe, effective and fiscally responsible.
Performs other duties as assigned and modified at manager's discretion.
EDUCATION AND EXPERIENCE CRITERIA:
Associate Degree in Nursing required, Bachelor's Degree in Nursing preferred
Nurse Licensure Compact license required, ability to obtain additional licenses as requested by the organization within 90 days of hire
Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required Minimum of 3 years acute clinical nursing work experience preferred
Minimum of 2 years experience in Emergency Nursing Services, or Emergency Triage, or Urgent Care highly preferred
Experience working with older adult populations highly preferred
Minimum of 1 year virtual care experience is a bonus
Bilingual fluency in Spanish highly preferred

The vAcute team works collaboratively with the patient’s Primary care physician and specialists to optimize the patient experience, increase communication, and ensure the patient receives care in the most appropriate setting. Through the use of technology, such as on demand video visits and triage tools, the team is able to reach patients when their need for care arises.
vAcute RN Overview:
Performs as a (RN) Nurse in the Mercy Virtual vAcute program in accordance with all applicable Federal, State, and Mercy rules, regulations, policies and procedures, and performs all duties assigned by Medical Director, in the vAcute program, concerning patients and their care.
These duties differ according to the assigned areas and range from education to care coordination. vAcute provides care on a 24-hour basis.

Overview
Registered nurses provide illness management and health promotion for patients and families who require care. QUALIFICATIONS

Registered Nurse licensure in state of residence.

Strong interpersonal skills (including ability to minister to diverse ethnicities).

Pay Range: $39,288.00 - $52,392.00 per year
Pay: This position is funded through personal fundraising. Accordingly, compensation will vary based on the ability of the individual to secure a donor team to fund the cost of his/her salary. The anticipated salary range for this position on a full-time basis (40 hours/week) is dependent on a variety of factors, including location and cost of living. The actual salary received, including any geographic adjustment to account for location and cost of living, is subject to the individual's ability to raise funds necessary to cover the full amount of such salary within the range set forth in the job posting details.
