Hospitalist- UMC Main Campus The Advanced Practice Provider II is responsible for assessing patient needs and assisting in the implementation of patient care under the direction of the department Medical Director and department Nurse Director or supervising physician.
Job Specific Responsibilities
Daily assignments may include but are not limited to:
* Works collaboratively with Hospitalist and primary care physician to perform patient assessments, provide evaluation and consultation for patients being admitted and treated at UMC
* Performs examinations/ procedures approved by Medical Director or licensed Hospitalist and documents accordingly in patient's progress notes
* Coordinate with nursing staff to develop treatment plans consistent with patient's condition and appropriately order medications, activities, diet, any amendments to current treatment plans, post care, etc…
* Facilitate patient discharges on behalf of Hospitalist or primary attending physician
* Maintain compliance related to patient orders, records, treatments plans, etc.., according to UMC policies, procedures, and standards set forth by Det Norske Veritas (DNV)
* All other assigned duties related to patient plan of care
Education and Experience
* Graduated from an accredited school of nursing.
* 3 years of nurse or nurse practitioner experience, preferably in a multidisciplinary hospital setting
Required Licensures/Certifications/Registrations
* Current valid APN (Advance Practice Nurse) license
Skills and Abilities
* Ability to apply the theory of an Advance Practice Provider under the bylaws of Texas Board
* Effective communicator verbally and in writing; Must speak, read, and write English
* Possess analytical skills to evaluate care needs, ability to gather relevant clinical data, perform thorough physical examinations, and assist with clinical patient management.
* Possess a high degree of accuracy in carrying out prescriptions, receiving and translating information, charting, and reporting
* Able to work long hours, various shifts as assigned to meet the department's business needs
Interaction with Other Departments and Other Relationships
This position requires close interaction with the medical staff, nursing staff and ancillary areas in order to maintain a quality multi-disciplinary approach to care of the critically ill patient.
Physical Capabilities
Work requires walking and standing most of assigned shift. Visual acuity, adequate hand/eye coordination, fine motor skills, talking and hearing are essential for this role. May be required to lift up to 50pounds
Environmental/Working Conditions
This position is mostly subject to inside environmental conditions, however, may be required to be outdoors occasionally. Occasionally, may require work with chemical hazards (i.e., sprays, mists, vapors and fumes). Hours may be long, irregular, and subject to call back on weekends or emergency call.

The Direct Support Professional (DSP) II works as part of a team that helps people with intellectual disabilities achieve their goals. This includes routine and emergency care, treatment, and training for people with intellectual disabilities in various settings. Responsibilities can vary, and may include such duties as grooming, bathing, feeding, observing and reporting individual's conditions and behaviors, providing meaningful interactions with individuals, and participating in individualized training and/or active treatment programs for both males and females. The DSP documents information on the person's life, such as things that occur throughout their daily schedule, how they respond to activities on the home, training, and unusual incidents. Completion of work requires use of simple and/or routine duties, while decision making is based on guidance that will be provided to you as you are prepared for the job and from other professional staff on your team. Work is in a team environment with guidance provided through supervision and training. May provide training/mentorship to new DSP Is and/or volunteer to serve as a shift/team lead. This position may be required to drive a state-owned vehicle.
Essential Job Functions (EJFs):

Daily assignments may include but are not limited to:

Responsibilities:
Providing behavior intervention, health, socialization, recreational, and personal care services and supports, outlined in the Individual Program Plan, for individuals receiving services.
Provide assistance in personal hygiene tasks, ranging from total physical assistance to monitoring (including toileting, bathing, grooming, dressing, washing hands, brushing teeth, etc.).
Provide physical assistance as needed, such as using adaptive equipment to/from wheelchair to bed, tub, shower, etc.

The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The Direct Support Professional (DSP) I works as part of a team that helps people with intellectual disabilities achieve their goals. This includes routine and emergency care, treatment, and training for people with intellectual disabilities in various settings. Responsibilities can vary, and may include such duties as grooming, bathing, feeding, observing and reporting individual's conditions and behaviors, providing meaningful interactions with individuals, participating in individualized training and/or active treatment programs for both males and females. The DSP I documents information on the person's life, such as things that occur throughout their daily schedule, how they respond to activities on the home, training, and unusual incidents. Completion of work requires use of simple and/or routine duties, while decision making is based on guidance that will be provided to you as you are prepared for the job and from other professional staff on your team. Work is in a team environment with guidance provided through supervision and training.
This position may be required to drive a state-owned vehicle.
Essential Job Functions (EJFs):
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
Assists persons served with training to teach daily living and vocational skills.
Provides behavior intervention by following the recommended behavior intervention techniques to ensure that persons served are protected from harm or injury due to abuse/neglect, sexual incidents, serious injuries, or other sources of immediate danger and provides emergency care as needed.
Participates in delivering therapeutic and individualized training per the individual's active treatment program, and documents individual progress.
Assists persons served (male and female) with carrying out necessary self-care skills (i.e., bathing, dressing, oral hygiene, toileting, feeding and grooming).
Ensures environment is free of safety hazards by correcting any hazards found and reporting them to the supervisor.
Observes persons served for signs and symptoms of disease, injury, reactions to medications, or other conditions that warrant medical intervention.
Provides assistance in the transporting of individuals going on scheduled activities and in emergency situations, as needed.
May prevent or manage aggressive behavior among individuals and administer CPR in emergency intervention.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
Knowledge, Skills and Abilities (KSAs):
* Ability to learn patient care techniques and of treatment, therapy, and developmental programs.
* Ability to read, write and understand English to effectively communicate and carry out verbal and written instructions.
* Ability to learn behavior modification techniques as required for training of individuals with certain specific behaviors.
* Ability to provide individual care to administer first aid or cardiopulmonary resuscitation, to conduct Safe Use of Restraints (SUR), and to communicate effectively.
* Ability to work effectively with others and as a member of a team and to accept supervision.
Registrations, Licensure Requirements or Certifications:
Valid Texas driver's license or ability to obtain it no later than 90 days after hire date, is preferred. Applicants with an out-of-state driver's license must provide an original certified driving record from the state of driving licensure. Eligible driving record required based on HHSC Fleet Management policy.
Initial Screening Criteria:
High School Diploma or GED or previous employment experience.

Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS List of Excluded Individuals/ Entities (LEIE).

All State Supported Living Center Employees are subject to Random Drug Testing.
Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.

Position Summary
The Direct Support Professional Floater is responsible for fostering patient independence by providing direct daily support, education and transportation to clients with developmental and intellectual disabilities under the supervision of the Residential Services Director and in accordance with Individual Service Plans and physician orders in a supported living home or community-based setting. Essential Functions and Responsibilities
* Providing direct care to consumers served by the agency.
* Providing emotional support to consumers and their families.
* Following directions of the RSD to meet the consumer's service needs.
* Preparing healthy, well balanced meals as well as specific physician-ordered diets.
* Completing the required number of in-service hours each year to maintain active status.
* Documenting the services provided as well as turning in paper work within specified time frames.
