MDS / Care Plan Coordinator **$7,500 Sign on Bonus** IS LOCATED IN BRADLEY, IL** SUMMARY: The MDS / Care Plan Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility.
