The Client Services Specialist assists FSL's community service programs to attain contractual program objectives and goals by providing direct assistance and case management services to vulnerable populations using resources from various funding sources. Encourages and promotes self-sufficiency through assessment of client needs, listening to concerns, and connecting clients to community resources and other educational opportunities designed to assist them with services such as budgeting and finances, employment services and other necessary support systems that will increase a person's ability change, be self-sufficient and/or improve their quality of life. Serves as a liaison between FSL community service programs and external community organizations.
This position does not have direct reports.

Requirements
Education & Experience, Knowledge/Understanding
Bachelor's degree (B.A.) from a four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience
Previous case management experience
Know all FSL programs provided in Service Area
Knowledge of resources available in the community
Possess valid AZ DL and acceptable MVR
Bilingual (English/Spanish)
Clear fingerprint background screening through the Department of Public Safety and comprehensive background clearance
Skills

Safely and efficiently transport riders to and from non-emergency medical appointments, senior centers, day programs, social activities, and grocery shopping in and around the valley. The PRN Driver will provide transportation services on an as-needed basis, covering shifts at various FSL locations. This position is not routinely scheduled and requires flexibility to pick up shifts as required.
This position does not have direct reports.
Essential Functions:

Provide safe, on-time transportation to older adults and/or disabled riders using company provided vehicles. Always maintain a friendly and supportive demeanor.
Assist riders boarding and exiting the vehicle, ensuring their comfort and safety during transport. This may involve helping riders with buckling seat belts, and mobility aids such as wheelchairs and walkers.
Plan and optimize transportation routes to minimize travel time, fuel consumption, and vehicle wear and tear.
Maintain open communication with the Call Center and riders to coordinate pick-up and drop-off times, address any concerns or special requirements, and ensure timely arrivals.
Comply with all traffic laws, safety guidelines and FSL/NVC policies and procedures to minimize the risk of accidents or injuries during transport.
Perform daily pre-trip and post-trip vehicle inspections to ensure the vehicle is in proper working condition and complete all documentation as required. Notify Transportation Manager immediately of any unsafe vehicle concerns.
Maintain cleanliness of assigned vehicle(s).
Complete daily manifest which includes times and mileage at each pick-up and drop-off, along with fare collected (if applicable).
Ensure that incident/accident reports and all supporting data/documentation are submitted for review by the next business day.
Respect rider privacy and maintain confidentiality of sensitive medical information in accordance with HIPAA regulations and company policies.
Be prepared to respond calmly and efficiently in case of emergencies, including accidents, medical crises, or vehicle breakdowns. Follow established procedures found in each vehicle binder for handling emergency situations, notifying the appropriate individuals, and seeking assistance when needed.
Participate in all safety training and keep all certifications and licenses current.

Works with the health care team to promote positive adjustment, rehabilitation and improved quality of life for our patients. In collaboration with the interdisciplinary team, informs, educates and supports staff in understanding the emotional, psychological and behavioral impact of Chronic Kidney Disease on the patient and family to ensure comprehensive quality care of our patients. Supports the Fresenius Kidney Care (FKC) commitment to the Quality Indicators and Outcomes and Quality Assessment and Improvement (QAI) Activities, including those related to patient satisfaction and quality of life and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FKC Quality Goals. This is an entry level MSW role.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient Assessment / Care Planning / Counseling
As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
Utilizes FKC patient education programs, established social work theory and methods, social work focused interventions, and quality of life measurement instruments as part of assessment and care planning to address barriers and meet patient treatment goals.
Provides monitoring and interventions for the patient to adjust to dialysis and achieve optimal psychosocial status and quality of life.
Provides supportive counseling services to patients as permitted within the scope of their clinical training and state license.
Provides educational and goal directed counseling to patients who are seeking transplant.
Provides information and assists the team and patient with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal treatment outcomes.
Maintains current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to vocational rehabilitation to enable them to remain employed, become employed or receive education.
Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons.
In collaboration with the physician and nurse, participates in the discussion of patient DNR status in the facility to ensure patient and/or family understand and make an informed decision about their care.
Knowledgeable of and adheres to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy, including documentation.
Documents based on MSW interaction and interventions provided to patient and/or family.
Quality
Provides psychosocial support and/or Social Work Focused Interventions to address non-adherence, quality outcome, and quality of life concerns for all patients based on acuity level.
Participates in monthly Quality review meetings with the interdisciplinary team. Reports on quality indicators related to adherence, such as Missed and Shortened Treatments, Quality of Life Trends, and Service Recovery.
Patient Education
Assesses patient knowledge of kidney disease for barriers that may affect adherence to treatment. Works with patient, family and health care team to provide education tailored to the patient's learning style, communication barriers, and needs.
With other members of the interdisciplinary team, provides appropriate information about all treatment modalities.
Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management.
Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
Reviews patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.
Collaborates with the team on appropriate QAI activities.
Patient Admission and Continuity of Care
Reviews Patient Rights and Responsibilities, Grievance Procedure & Important Numbers Handout, FKC Non-discrimination policy, DNR Statement (if applicable) and address any immediate needs/concerns.
Understands the referral and admission process and supports the clinic in regard to the patient needs for scheduling to maximize adherence and adjustment.

Refers patients to patient billing solutions (PBS) department for questions/concerns in regard to treatment related bills
Staff Related
Assists with interview process and decision to hire new personnel if requested by SW Manager/Senior Manager.
Works with the administrative support staff to maintain updated patient resource lists (e.g. maintain updated list of transportation resources).
Provides training to staff pertaining to psychosocial topics as needed.
Contributes and participates with weekly team huddles. Discusses any urgent patient issues with staff.
Adheres to work defined caseload guidelines based on state regulatory requirements.

POSITION SUMMARY

CORE FUNCTIONS
1. Provides comprehensive nutrition assessment of patients' nutritional needs, to include nutrition-focused physical exams as appropriate. Obtains timely and appropriate data and analyzes/interprets data based on evidence-based standards and established practice guidelines.
2. Demonstrates and maintains competency in enteral and parenteral clinical practice protocols and oral nutrition management standard of practice as demonstrated by successful completion of initial competency exams and ongoing evaluation.
3. Writes orders to initiate and manage the nutrition care and of all assigned patients including those receiving nutrition support (parenteral and enteral nutrition) per protocols.
4. Evaluates effectiveness of the care plan and communicates effectively to the other members of the healthcare team to provide optimal nutrition outcomes. Provides pertinent information in the medical records, patient plans of care and/or clinical pathways while meeting requirements of regulatory agencies (i.e. Joint Commission, CMS, State and Local Agencies).
5. Provides nutrition education to patients, families, staff and other health care team professionals through the development of effective teaching materials and in-service programs; Evaluates patient and/or family response to education.
6. Provides leadership in area of coverage, monitors proper patient care procedures and acts as liaison with nursing areas. May participate in process improvement teams. Serves as mentor to new staff and interns where assigned.

8. MINIMUM QUALIFICATIONS
Must possess knowledge of nutrition and dietetics demonstrated through the completion of a Bachelor's degree in Nutrition and/or Dietetics. Must be a Registered Dietitian or RD Eligible based on academic and professional requirements as defined by the Commission on Dietetic Registration(CDR) of The Academy of Nutrition and Dietetics. Effective January 1, 2024 CDR requires a minimum of Master's degree to be eligible to take the registration examination. Must possess RD credentials within 6 months of hire. In collaboration with the interdisciplinary team, informs, educates and supports staff in understanding the emotional, psychological and behavioral impact of Chronic Kidney Disease on the patient and family to ensure comprehensive quality care of our patients. Supports the Fresenius Kidney Care (FKC) commitment to the Quality Indicators and Outcomes and Quality Assessment and Improvement (QAI) Activities, including those related to patient satisfaction and quality of life and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FKC Quality Goals. PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient Assessment / Care Planning / Counseling
As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.

Maintains current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to vocational rehabilitation to enable them to remain employed, become employed or receive education.
Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons.

With other members of the interdisciplinary team, provides appropriate information about all treatment modalities.
Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management.
Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
Reviews patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.
Collaborates with the team on appropriate QAI activities.
Patient Admission and Continuity of Care
