As a Home Health Occupational Therapist Assistant , you will:
Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
Assist in the implementation of vocational/education programs and activities established by registered Occupational Therapist designed to restore, reinforce, and enhance task performances, diminish or correct pathology, and to promote/maintain health and self-sufficiency.
Design/adapt equipment and working and/or living environment. Fabricate devices to assist and improve function and independence and participation in the program and/or community where possible.
Provide therapeutic treatment and instruction to patients as directed by the qualified Occupational Therapist and in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
Report information and observations to Occupational Therapist and/or Clinical supervisor, document observed information in patient records and prepares clinical notes. Assist with preparation of progress reports. Maintain and submit documentation as required by the Company and/or facility.
Instruct patients and family members regarding home programs as well as care and use of adaptive equipment.
Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's needs.
Design community reintegration activities, as appropriate, to assist the client in the physical reconditioning effort, and/or the psychological adjustment and coordinate the plan with members of the interdisciplinary team.

Current and unrestricted OTA licensure
Minimum of six months occupational therapist assistant experience preferred

Students attend Brightmont Academy campuses for multiple reasons including; learning disabilities, attention deficits, emotional issues, health concerns or other individual special needs. Our students look to our instructors for better learning strategies needed for school and career success.
This is a part-time opportunity with a flexible schedule that requires you be on campus 3 days a week. Our school is open Monday - Friday from 8:30-5, you would be able to let us know your hours of availability and we would create a schedule around what works for you. No lesson planning, grading, or administrative duties outside of the school. There is no cap on hours and ideally we'd like someone to be available at least 15 hours a week.
The Special Education Administrator will be held accountable for the following responsibilities:
Facilitate the provision of special education services to students that are placed with Brightmont from a district.

High School Bridge Counselor

Chicago students have the right to a well-resourced high school education, but the placement process is complicated to navigate alone. High School Bridge (HSB) helps students attending elementary schools in communities impacted by systemic disinvestment find and succeed in their best-fit high schools. We advocate for 7th, 8th, 9th and 10th graders and their families as they build the foundation for their success in high school, after high school and the start of their careers.
Over the next five years, the High School Bridge Program will grow to serve 1,800 students from 18 different elementary schools in seventh through tenth grades as they chart their unique path by accompanying them through their transition to best-fit high schools.
The responsibilities of the HSB Counselor include but are not limited to: developing and maintaining relationships with partner elementary schools; working directly with families in the high school application and decision-making process; and developing best practices to support high school and postsecondary access for first-generation, low-income and under-represented students in Chicago through the implementation of a HSB program at partner schools.
The person in this position reports to the Director of the HSB Program and works closely with the HSB Team. International SOS is looking for qualified individuals to be the Social Service Associate for the Sailor Assistance and Intercept for Life (SAIL) Program for The Navy Fleet and Family Support Program (FFSP) The FFSP serves active duty members of the Navy, Marine Corps, Army, and Air Force plus their eligible family members; active duty members of foreign military services who are stationed in the area; reserve military personnel and their family members when they are eligible for military medical care, military members designated as severely wounded, ill and injured and their identified caretakers and, on a space available basis, military retirees and their family members and widows, eligible family members of military decedents and other groups as directed and authorized by higher authorities in response to special circumstances.
Key Responsibilities
Maintain current schedules of SAIL Case Managers and Contract Remote SAIL Clinical Counselors providing individual appointments. Utilize FFSMIS to view schedules, to make client appointments, and send reminders to clients to confirm appointments.
Manage daily scheduling to ensure clients are notified of unscheduled SAIL Case Manager absence as soon as possible, rescheduling appointments in a timely manner at the client's request.
Maintain direct service metrics for SAIL Case Managers and Contract Remote SAIL Clinical Counselors for whom scheduling services are provided.

Tier I clinical providers must perform all clinical duties under the supervision of a licensed practitioner and under no circumstances can they provide independent clinical care. Sailor risk, safety, and progress in their recovery in accordance with the DoDI 6490.08.
Collaborate with Sailors behavioral health treatment provider(s), and other service providers upon receipt of release of information from the Sailor.
Participate in virtual, program-specific, quarterly quality assurance procedures to ensure that all SAIL services provided are safe and of good quality.
Document and maintain current records in the FFSMIS SAIL Record System for all SAIL contacts, assessments, and case management services provided IAW Navy requirements.
Ensure Command referral information for Sailors who decline or who cannot be contacted is documented in the FFSMIS SAIL Record System. At the conclusion of SAIL services, alert command that SAIL services are ending and advise on procedures if the Sailor's risk of suicide related behavior returns.
Report suspected child abuse and domestic abuse/violence to the local civilian authorities and the Family Advocacy Representative IAW Navy and state requirements. For clients wanting to make a restricted domestic abuse report, provide a warm handoff to the installation Family Advocacy Representative or FAP Victim Advocate. For clients wanting to make a restricted sexual assault report, provide a warm handoff to the installation Sexual Assault Response Coordinator or SAPR Victim Advocate.
Ensure Sailors receive an initial evaluation by Military Treatment Facility/Emergency Department qualified staff after suicide-related behavior or ideation in accordance with local emergency response procedures and in coordination with the Sailors command
Required Work Experience

POSITION DESCRIPTION

ESSENTIAL DUTIES AND RESPONSIBILITIES
Plan and realize presentations on immigration legal services, citizenship, emergency relief, adult education, and other services that immigrants need to help integrate into life in Chicago and the US.
Recruit and supervise community residents to become Community Navigators, a leadership initiative that helps immigrants help themselves.
Follow up with clients who request services and assist them with anything needed to register and access legal services, adult education, emergency relief, counseling, public benefits, and other needed social services.
Attend funder program meetings in representation of Instituto del Progreso Latino.
Reach the public through presentations.
Recruit, onboard, and train volunteers and Navigators to assist in community outreach.
Maintain data and fill out reports based on program activities, including client outcomes, as well as properly documented outreach activities.
Makes program visible in the community as needed.
Attend Navigator meetings and trainings.
Prepares reports as requested.
