Coordination of services include communicating and making assignments for visiting nurses, home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services, as well as, ordering medications and Durable Medical Equipment, and supplies • Provide consultation and supervision to all CNAs and LPNs involved in the care of the patient • Participate in the performance evaluation of clinical members of LPN(s) and CNA(s) involved with the care of case load. • Assist with the orientation of clinical members participating in the care of the assigned caseload • Ensure cost effective practices by self and others • Assist with QAPI (Quality Assurance and Performance Improvements) activities • Work as a team member at all times and uphold the Hospice Code of Ethics • Provide care to manage patient • Utilize the medication formulary whenever possible for the management of patient symptoms • Triage scheduling and symptom management calls from members of IDT • Reports to the Clinical Director as patient condition changes or change in the patient condition that require change in level of care Work Environment/Physical Requirements: The responsibilities of this position involve significant physical activities including standing, lifting, bending, stooping, pushing, pulling and twisting. Position Summary WHAT YOU WILL DO This role will be working closely with Child Protective Services (CPS) Case Managers by providing services to families to reunify children on a permanent basis to families that may have barriers that prevent them from obtaining these services on their own. Examples of the barriers may be mental illness, developmental or physical disabilities, and/or alcohol or drug abuse. SOME ESSENTIAL RESPONSIBILITIES You will be successful in this role by completing the following tasks and responsibilities: * Supervises family visitation and interaction in the community, in homes, and at the agency. * Supports families through teaching of everyday living skills (i.e. nutrition, housekeeping, hygiene, employment support, financial planning); transporting families to various appointments; educating families in parenting skills; and assisting in crisis management. * Ensures information is communicated and understood between families, care providers, and case managers and documents family interactions and visitations in State database. Assesses client's situation, including strengths, challenges, and progress. * Attends court hearings and when necessary, gives testimony to observations. * Participates in childcare for evening hours trauma parenting group, foster care group or other group meeting the agency may hold. * Provides a "drop-in" service to case managers for families that may have partially unsupervised visits. * Assists case managers with administrative tasks, such as record requests and case closures. Salary Information Wage is $23.76 - $25.13/ hour. Full range extends to $31.27/hour.
