Collaborate with Community Partners: ● Collaborate with formal systems, service providers and informal networks involved with the youth and family. ● Provide outreach and needs assessment with community partners specific to the role of Family Peer Support. Documentation: ● Utilize New Directions electronic case recording system for assessment, service planning, progress notes, and outcome monitoring purposes. ● Maintain accurate and timely case records, including progress notes and tracking mechanisms, electronically and in each client's file. ● Keep all client and family information, verbal and written, confidential. ● Adhere to all Medicaid requirements for billing and documentation. ● Maintain documentation for all group meetings (group participant roster, dates, locations, topics), units of service and related expenditures, and submits required information to Supervisor. ● Conduct client satisfaction surveys at regular intervals and discharges. Training: ● Complete Level One and Level Two of the Family Peer Advocate Core Training/ Parent Empowerment Program (PEP) training or approved comparable training. ● Family Assessment of Strengths and Needs (FANS). ● Complete 20 hours of continuing education and renew FPA credential every two years. ● Attend all mandatory in-service trainings as per NDYFS policy, OMH, and local county requirements. The clinical department at Venture Forthe focuses on HCBS (Home and Community Based Services)/CORE (Community Orientation Recovery and Empowerment Services) programs under the direction of the Office of Mental Health. The goal of the CORE/HCBS program derives around the focus of increased behavioral health and addiction recovery through an outpatient system. This includes providing clients with the tools, resources, and skills they can utilize to help in their recovery journey. We can proudly say that we have helped our clients reach milestones such as obtaining a higher education degree, grow relationships with friend and family members and regain the ability to be fully independent in the community. Peer Support Specialists work closely with individuals in our program to advocate for them in their recovery journey. This includes supporting individuals with their goals and provide guidance to them in a non-clinician geared role. These individuals will work with clients in their home/community to provide this support which could look like a walk in the park or maybe getting a cup of coffee. Peers will have supervisions on either a bi weekly or weekly basis to ensure the goals of the individual are coming to fruition and ensure their own success in the role. Conducts casework contacts with families, as needed but at least twice a month, in their homes and community settings to focus on treatment goals. Engages youth and family by developing effective and caring relationship. Ensures children are maintained in safe, secure environments. Acts as a bridge between youth and family, and foster care treatment teams through telephone, face-to-face contacts, family conferences, comprehensive treatment reviews, etc. Attend treatment review meetings. Assists with reunification of youth with family, community, school by implementing permanent supports and resources including but not limited to identifying and working with a mentor, setting up after school recreational and job opportunities and developing a relapse prevention program. Job Requirements Minimum Bachelor's degree in human services or related field, Masters degree preferred. Experience working with at-risk youth and families. Must be able to work a flexible schedule with variable hours including day, evening, weekend, holiday and subject to call hours. Must have a Valid Driver's License with acceptable driving history to the Agency. Must have a reliable car. Travel is required. Ability to coordinate and plan travel for visits and appointments in efficient manner. Ability to work with economically and culturally diverse population. Skills: Ability to handle crisis intervention in solution focused manner as well as communication skills including written, verbal and listening skills. Pay Minimums and maximums may vary based on skills, experience and other relevant factors. $23.08 /hour ($48,000 annually) - Bachelor's degree + 0 years of relevant experience. $24.04 /hour ($50,000 annually) - Bachelor's degree + 2 years of relevant experience.
