What you'll do in this role:
Manage Journey patient census and referrals for the assigned area from Journey candidate list, Journey Administrative Assistant, Customer Service, Clinicians, and other sources
Review all assigned patient charts to determine eligibility and appropriateness for the Hospice benefit and prepare for conversations with the patient, family, and/or caregivers
Make initial phone calls to all new Journey referrals to schedule home visits as well as follow-up calls to maintain contact with the patient and assess patient decline
Conduct patient visits for the purpose of building relationship with the patient and/or family and in-depth discussion of the hospice benefit when appropriate
Maintain Journey patient charts, documenting all Journey visits and contact in Journey Visit/Phone call notes or as Coordination notes when applicable
Build relationships with and maintain effective communication with Clinicians to ensure effective collaboration for addressing patient needs
Assist clinicians in a more accurate understanding of the philosophy, goals, eligibility, and benefits of Hospice care

Responsibilities
Resident Care Supervisor / Licensed Practical Nurse (LPN):
Supervise/Administer and/or assist with medications to assigned residents; monitor monthly residents participating in self-medication program.
Monitor the personal care provided by Resident Assistants.
Assist Resident Assistants with direct care of residents, as necessary.
Report to Director of Nursing change in condition, daily needs, and progress of residents.
Follow the plan of care for each resident.
Responsible for all aspects of receiving, storing and distributing controlled substance medications; ensure proper placement and use of medication stored by facility including med cart and medication room refrigerator.
Maintain documentation regarding the provision of care to residents in the Case Notes, Medication Administration Records and other required records.
Assist in problem-solving related to staffing, supplies and resident care during shift.
Ensure that resident care standards are maintained in performance of duties, and that the rights of residents and preferences in care and treatment are respected.
Receive and store lost/found articles and notify the Director of Nursing and/or Case Manager
Ensure that residents are treated with respect and kindness at all times
Qualifications
Resident Care Supervisor / Licensed Practical Nurse (LPN):
Credentials as a Licensed Practical Nurse with a current State license required .
Experience in geriatric nursing preferred.
Organizational skills and ability to supervise department staff effectively and work well with personnel of other departments required; ensures high standards of care are maintained.
This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In this role, you will be responsible for testing, troubleshooting, and repairing electronic assemblies down to the component level. You will play a key role in Factory Acceptance Testing (FAT), working closely with customers, engineers, and the manufacturing team to ensure products meet quality standards before shipment.
Key Responsibilities:
Perform testing and troubleshooting of electronic assemblies to the board and component level.
Identify and repair issues using advanced soldering and wiring skills.
Serve as the primary liaison between Manufacturing, Customers, and the Engineering team during customer-witnessed Factory Acceptance Testing (FAT).
