Education and Experience Needs
Associate degree in Exercise Science, Health Education or a closely related field; OR equivalent experience required. Bachelor's Degree preferred.
Educational preparation in Basic Cardiac Life Support required. Prior experience working in a gym setting, providing group exercise classes or providing fitness/exercise programs for seniors preferred.
CPR and First Aid certification is required (or must be obtained within 60-days of employment). Group exercise certification preferred.

Perform daily leadership rounds of the health center service department to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Provide direct care as necessary.
Review nurse's notes for accuracy and to ensure that they reflect the resident's response to the care.
Monitor medication passes and treatment schedules.
As part of the nursing management team shares responsibility of being on call for in-house staff. This may mean assuming coverage as staff nurse on the nursing units in the event of inadequate numbers of staff to care for residents.

Must posses, as a minimum, a Nursing Degree or Diploma from an accredited college, university or nursing school

Elliot Sleep Lab is located at the Homewood Suites Hotel in Manchester, NH. We have a secure wing with controlled access for patients and staff. It is conveniently located off 293 and provides a very comfortable atmosphere for both patients and staff. We find our patients are much happier and more satisfied with our testing process being in a relaxed environment.
We have 4 board certified Sleep Physicians and specialize in both pediatric and adult sleep testing. We provide start to finish long term care for our sleep patients, which includes, initial consultation, home sleep testing, in-lab sleep testing, mask fittings, CPAP clinics, remote patient monitoring, yearly follow up. We also do testing for other organizations in the community that don't have their own sleep lab.

Performs sleep studies as per order of physicians.
Scores sleep stages, sleep apnea, hypopnea, nocturnal myoclonus and recognizes certain EEG and EKG abnormalities and artifacts.
Applies and adjusts PAP according to procedure manual.
Responds appropriately to medical emergencies that may arise during sleep studies.
Calibrates and maintains all equipment necessary for sleep monitoring.

What You'll Do
Prepares for patient evaluation by conducting medical record review identifying areas of focus, contraindications and precautions. Ensures orders for therapy are active and appropriate.
Evaluates the skilled therapy needs of the referred patient and develops a written prioritized plan of care, including short- and long-term rehabilitation goals developed in collaboration with the patient and/or other members of the health care team.
Updates treatment plan re-evaluates patient and communicates with referrer and other clinicians involved in the patient's care.
Establish and implements discharge plan and instructs patient, family member(s), and other essential personnel in effective performance of home exercise programming or other strategies to maximize function. Supports EHS Rehabilitation Services commitment to clinical development by participating in the department's clinical affiliation program. After practicing successfully for a period of 1 full year, the occupational therapist is expected to act as a clinical instructor, taking on a student as assigned every 18-24 months.
Participates in EHS Rehabilitation ongoing clinical development programming, with the delivery of an educational presentation once per year on a topic of their choice. Participation may include but is not limited to; in services (network or clinic), journal club, community outreach, or other educational opportunities.
Education and Experience Needs
Graduate of an accredited Physical Therapy Program with corresponding Entry Level Degree required. Licensed as a Physical Therapist by the state of New Hampshire Physical Therapy Governing Board required. CPR certification required or able to complete within first 90 days.

About the Job:
Delivers home health care to client/patients of all ages and families whose needs range from basic to complex and are rapidly changing by utilizing the nursing process. Creates a therapeutic environment while identifying and treating human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, and the provision of care supportive to or restorative of life and well-being. Practices under the direction of the Clinical Manager and supervision of the RN within the departmental policies and procedures.
What You'll Do:
Assesses patients' physical and psychosocial status and the home environment.
Reviews all medical data prior to delivering patient care, documents and communicates patient status and assesses needs to the Home Care Nurses.
Under direction of Home Care RN, participates in the development of a patient care plan which will facilitate health care, promote patient comfort and well-being, and includes patient/ family.
Communicates appropriate information to patients, peers, managers, physicians, department, and other agencies involved.
Integrates problem solving methodology and quality improvement in approaching problem areas related to patient care and organizational processes.
Provides nursing care while promoting client/patient dignity and provides a calm, supportive and therapeutic environment for client/patients and their families.
Obtains prior authorizations and documents adequately to ensure prevention of write-offs.
Explains insurance coverage and cost of care appropriately to patients/significant others.
Who You Are:
Education: Graduate of an accredited school of Licensed Practical Nursing required.
Experience: Minimum of 2 years previous clinical experience required, preferably in a home care setting.
Certification/Licensure: LPN NH licensure required. Current BLS certification or must obtain within 90 days of date of hire required.
Schedule: Work schedule may include eight (8) to twelve (12) hours per day with weekend, evening, and night coverage based on VNA needs. Staff are expected to cover for absences by rotating or working extra time. Staff are assigned to the services area of VNA to meet client/patient needs.
Other: Valid Driver's License required to travel throughout southern New Hampshire to provide home health care service and other job-related activities. Proof of auto mobile insurance required. Motor vehicle background report will be conducted.

Assists providers in the administration of patient care in assigned clinical areas. Assists with patient examinations, procedures, diagnostic studies, treatment, and dressing changes. Performs general administrative duties in accordance with JCAHO, CLIA, NCQA and government standards, as well as company policies and procedures.
Key Responsibilities
Performs visit chart preparation activities based on the daily scheduled appointments including diagnostic test results, emergency/urgent care reports, discharge summaries and consult/PCP reports are located in the medical record and accessible to the provider.
Performs rooming activities for provider visits according to the documented rooming criteria including chief complaint, allergies, vital signs, medication list review, tobacco history, LMP, health maintenance overrides and documentation in a timely and accurate manner.
Obtains and documents age and/or condition-specific measurements accurately including vital signs, height, weight, head circumference, peak flow, and pulse oximetry.
Conducts and documents hearing, vision, respiratory and ECG testing.
Performs and documents specific screening based on condition/need including depression and asthma.
Obtains, documents and processes specimens using the appropriate collection process and equipment.
Performs and documents point of care testing accurately.
Performs venipuncture procedures (as needed) and processes specimens as outlined in policies and procedures.
Administers medications and immunizations including oral, nasal, injectable, rectal and topical routes as appropriate.
Processes medication prescription refills per guidelines using telephonic, electronic or print methods.
Handles prior authorization processes for medications and/or procedures and documents appropriately.
Manages clinical supplies and medication sample areas to ensure that stock is current, stored appropriately, labeled and par levels maintained.
Education/Experience/Licensure
Education: High School Diploma or GED required. Graduate from an accredited Medical Assistant/LPN program required.
Experience: One (1) to three (3) years medical assistant experience preferred.
Certification/Licensure: Medical Assistant Certification/ABR-OE strongly recommended.
Software/Hardware: Computer/EMR experience preferred.
