Become a part of our caring community and help us put health first Humana Healthy Horizons in Ohio is seeking a Care Guide/Care Guide Plus (Care Coach 1) who will assess and evaluate member's needs and requirements to achieve and/or maintain optimal wellness state. The Care Guide serves as a single point of contact for care coordination for members who are not assigned to a Care Coordination Entity (CCE), OhioRISE Plan, and/or CME involvement and short-term care coordination needs are identified. The Care Guide Plus serves as a single point of contact at the MCO for care coordination when there is Care Coordination Entity (CCE), OhioRISE Plan, and/or CME involvement and short-term care coordination needs are identified. Connects members to CCEs, the OhioRISE Plan, or MCO Care Management if the member's needs indicate a higher level of coordination.
This position employs a variety of strategies, approaches, and techniques to manage a member's health issues.
Identifies and resolves barriers that hinder effective care.
Ensures member is progressing towards desired outcomes by continuously monitoring care through use of assessment, data, and conversations with member/member representative.
Assesses and evaluates member's needs via telephonic and face to face assessments and evaluations, to coordinate services and address member short term needs.
Assures completion of an HRA or other assessments.
Assists members to remediate immediate and acute gaps in care and access.
Assists members with filing grievances and appeals.
Provides information to members related to MCO requirements, services, and benefits.
Provides members with information and/or referrals to community resources.
Coordinates services, and monitoring and evaluating the care coordination plan against the member's personal goals and identified needs.
Guides members/families towards resources appropriate for their care.
Services are driven by facilitating interactions with other payer sources, providers, interdisciplinary teams, and others involved in the member's care as appropriate.
Work assignments are often straightforward and of moderate complexity.
Makes decisions regarding own work approach/priorities and follows direction.
Understands own work area professional concepts/standards, regulations, strategies, and operating standards.
Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

Required Qualifications
Bachelor's degree in health and human services field
2 or more years of related experience
Knowledge of Microsoft Office Word and Excel

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.

In this role, you have the opportunity to work with families to teach them valuable skills that increase coping strategies and reduce tough behaviors while connecting them to appropriate behavioral health care and/or community resources when needed.

Assessment Review:
Thoroughly review assessment results gathered during the onboarding process to gain a deep understanding of the unique needs, challenges, and goals of each member seeking coaching or care coordination services.
Risk Assessment and Safety Planning:
Refer members to the most appropriate crisis resources in the case of imminent risk. Note: Brightline does not provide immediate, 24/7 crisis services.
Support members on safety/risk-related concerns as they arise either in live or asynchronous communication by conducting a comprehensive risk assessment and creating a safety plan with the family.
Provide safety support for all members in Brightline's care (i.e. receiving services from a coach, therapist, or prescriber).
Complete mandated reports and other documentation to ensure appropriate legal processes and protocols are followed.
Re-evaluate members' fit for Brightline services (Coaching or Therapy) after a safety event.
Care Coordination and Referrals:
Refer families to relevant community resources, including support groups, local agencies, and other social services (e.g. food, housing, financial assistance) that can significantly enhance their care experience.
Provide hands-on support to California families who may need assistance in navigating services within or outside of Brightline.
Facilitate internal referrals to Brightline Therapy services, if appropriate, ensuring families can access clinical services in a timely manner.
Coordinate external referrals to partner organizations such as schools, community health services, external therapists, primary care, EAP, and affiliate networks when necessary.
Coaching Members:
Provide evidence-based, culturally responsive behavioral health coaching to children, teens, and parents.
Tailor care plans to align with the family's preferences, values, and cultural context.
Deliver personalized coaching through video sessions and asynchronous chat using our digital tools.
Documentation:
Ensure accurate and organized record-keeping of assessments, safety plans, referrals, and interactions with families.
Adhere to relevant privacy and compliance standards for all documentation.

Masters degree in a psychology related field
1 year experience in a behavioral health setting

This position is 100% remote.

GCI's Remote Camp Guide will provide small boat operation and guiding services for guests of the company, serve as a fishing, hiking, sightseeing, kayaking, sailing and skeet range guide, and to conduct various operational tasks and facility maintenance tasks as needed. Operate, drive, and navigate passenger boats according to regulations. Instruct guests on boating safety and use of life jackets. Report to supervisor all accidents, injuries, and unsafe work conditions.
Position's Customer(s): Customers are Retreat guests including GCI customers, clients, employees, and other invited guests.
ESSENTIAL DUTIES AND RESPONSIBILITIES AT ALL LEVELS:
Provide guided fishing, sightseeing, kayaking, hiking and sailing services on area streams and rivers to guests of the company.
Conduct routine daily boat and fishing equipment maintenance.
Fresh fish processing, handling and packaging.
Assist other Retreat staff with loading and unloading of supplies, luggage and guests from Retreat airplane, boats, and vehicles, as well as other various guest support duties.
Assist other Retreat staff with routine facility, grounds, vehicle and equipment maintenance duties.
Function as Range Master on Retreat skeet shooting range.

Additional Job Requirements: The Remote Camp Guide is responsible for leading and managing outdoor activities in remote camp settings. This role requires a blend of guiding expertise and skills, to ensure the safety, enjoyment, and satisfaction of all camp participants.
Facilitate various outdoor activities such as hiking, kayaking, fishing, or wildlife observation.
Provide instruction on outdoor skills, techniques, and safety procedures.
Offer engaging and informative information about the local environment, wildlife, and geography.
Minimum Qualifications:
Required:

Minimum of two (2) years' experience in boating on lake/ river or employment in a similar position.
First Aid/CPR/AED training - within one month of hire.

USCG Operator Uninspected Passenger Vessel certification (OUPV).
DRIVING REQUIREMENTS:
This position requires driving a company-owned vehicle, company provided vehicle. Must possess and maintain a valid driver's license, proof of insurance, a satisfactory driving record, and successfully complete Defensive Driving course.
PHYSICAL REQUIREMENTS and WORKING CONDITIONS:
Must live and work at a remote retreat location (with employer provided room and board) for a period of approximately five (5) months.
Work week schedule is set at seven (7) days per week.
Most duties are performed outside with exposure to weather.
Must be capable of performing a variety of physical functions involving safely move, transport, position, maneuver loads up to 70 lbs., efficiently and safely entering and exiting aircraft, boats and other motorized equipment, operation of, 4-wheeler and Bobcat, climbing ladders to heights of 20 feet, rowing an 18 foot river boat for sustained periods, standing and operating an outboard motor in rough water conditions and be able to demonstrate both fly fishing and conventional fishing techniques.
Daily moving, traversing, and positioning self on varied uneven or unstable surfaces such as roofs and in trenches, often hampered by weather elements.
High degree of dexterity and coordination to operate hand tools and to connect, separate, and twist multiple smaller items daily.
Visual acuity is necessary to identify, inspect, observe, and assess details at near, mid, and far ranges.
Ability to clearly distinguish colors for driving, identifying cables or other components, and safety signage.
May encounter unrestrained animals/pets.
Physical stamina to lead tours and engage in activities for extended periods.

What will you be doing?
Experience First Guides are the heart of what we do. They bring the brand to life. Guides work closely with operations and other staff to:

* Synthesise and share information in an engaging way about sites visited, with an emphasis on personal story-telling
* Help travellers maximise their time and engage with a destination through local lived experience
* Check in guests using the Experience First Check-In App and reconcile any paperwork with the operations team
* Gather guest reviews and act as a brand ambassador pre, during and after the tour
* Work together with other Guides to share information, manage busy tour routes and cover/swap shifts where required
* Manage tour logistics while the tour is underway, placing the safety of guests first at all times.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Encourage patron participation in exhibits through age-appropriate interactions
Monitor and safeguard young patrons, ensuring that children are with their caregiver as they enter and exit the Museum and the surrounding Museum Campus
Assist with answering patrons' questions or resolving issues and needs; elevate issues to management when necessary
Set and reset seasonal exhibit areas throughout the Museum's operational hours
The successful candidate will possess the following:
Work experience in a museum or customer-facing role, preferred
Experience working with children, preferred
Proficient customer service and interpersonal skills
Friendly demeanor and positive attitude
Ability to learn Early Childhood Education content as it relates to seasonal museum exhibits and guest experiences
Must have flexibility with work schedule, as necessary, to support weekend scheduling and/or evening activities as needed
