The team will also work to link people with communityâbased primary healthcare and psychiatry, will assist people with getting prescriptions filled, and will coordinate transportation to followâup medical appointments. The Assertive Community Linkage team will also work closely with Thresholds Evaluations Department to monitor member engagement and outcomes and report to CCHHS.

Qualifications
· Education: High School Graduate or GED Substitution for Education
· Maintain a valid Driver's License
· Demonstrate functional 8th Grade literacy through standardized exam
· Obtain/maintain standing as an Authorized Qualified DSP to pass medication (paid training)
· Complete DSP training in accordance with state regulations, and maintain status on the Health Care Worker Registry (HCWR) (paid training)
