Groups Recover Together is an outpatient addiction medicine provider that delivers comprehensive outpatient treatment for opioid addiction. Based on the principle that recovery is tied to aspects of life that are beyond the reach of an office visit, our care model includes three core modalities that are proven to help individuals get their lives back on track - medication, weekly group therapy, and care navigation. As a Staff Provider, you will work with a small team of experienced providers, counselors, and support-staff to facilitate rapid-access intakes and enhanced medical support throughout the early weeks of treatment for Groups members. You will leverage telemedicine extensively in your work, so must be comfortable with technology and enthusiastic about the benefits of this exciting modality. You will work daily to overcome the barriers faced by members early in recovery and must adopt a “member-first” attitude that prioritizes keeping members safe and engaged in treatment at all costs. Flexibility, empathy, and commitment to member success are critical attributes for a successful Staff Provider. You will also provide longitudinal maintenance care to a panel of members (pending prescription cap capacity). Additionally, you will support your regular provider colleagues by providing coverage when medical questions or needs arise and these providers are unavailable.
Your knowledge of the Groups Recovery System and the Groups culture will be critical for ensuring successful launches of Groups into new markets and new geographies within existing markets. Finally, you will be an integral partner of the medical leadership team in developing and deploying new medical service lines for our members.
Responsibilities:
Adhere to all clinical policies and procedures as outlined in the GRS Manual
Intake medical duties
Determine each patient's medical necessity and appropriateness for treatment pursuant to applicable clinical models, policies and procedures, and manuals (e.g., Recovery System Manual) as required by Recover Together. For patients admitted to treatment, prescribe buprenorphine/naloxone and, where appropriate, other approved medications to treat withdrawal symptoms, in a manner consistent with Recover Together's policies and procedures and Recovery System Manual.
Maintain a regular schedule of “intake shifts”, and be available to provide rapid-access telemedicine intakes across multiple geographies, including new markets.
Complete all intake documentation thoroughly, efficiently, and accurately.
Be accountable for successful initiation of MAT during the early weeks of treatment (weeks 1-4, approximately).
As needed, coordinate with community providers to support the successful transition of new members into Groups.
Collaborate with other provider colleagues to transition the care of new members into regular maintenance groups.
Escalate controversial situations to appropriate medical and/or clinical leadership;
Maintenance medical duties
Maintain a panel of maintenance members and serve as their dedicated provider at monthly provider groups, per the guidelines of the Groups. Recovery System.
Meet with each patient for whom the Physician prescribes buprenorphine at least once every thirty (30) days. This interaction occurs in a group setting, with individual conversations occurring before or after the group as needed. Such meetings may occur via telemedicine, to the extent Physician complies with all applicable federal and state laws, rules, and regulations and any requirements set forth by applicable payers (e.g., Medicare, Medicaid, commercial plans) governing telemedicine.
Be available by phone and email to discuss each patient's medical issues in between monthly visits (e.g., dose changes; prescription troubleshooting, interprofessional communication and collaboration).
When indicated and medically necessary, meet with patients for a pre-scheduled individual session to review medical progress. These visits are separate from the individual conversations that occur before and after regular shared medical visits. Such meetings may occur via telemedicine, to the extent Physician complies with all applicable federal and state laws, rules, and regulations and any requirements set forth by applicable payers (e.g., Medicare, Medicaid, commercial plans) governing telemedicine.
Medical Records
In compliance with HIPAA and other applicable federal or state laws, rules or regulations regarding medical records, maintain patient records in confidence and complete records in a timely fashion and in accordance with policies of the Company.
Miscellaneous medical duties
Provide regular coverage for “makeup groups” and makeup 1:1 sessions.
Provide as needed cross-coverage for colleagues and answers to medical questions when other providers are unavailable.
Participate in rotating “on-call” schedule of urgent prescription coverage for unavailable providers.
Facilitate access to the State's prescription drug monitoring database so Groups staff can regularly monitor the Provider's patients' access to controlled substances, per Recover Together's clinical protocols.
Administrative duties - approximately 15% per week
Completing high quality documentation, specifically all medical encounters accurately and thoroughly on the same business day as the encounter
Sign all prescriptions on the same business day that they are generated
Manage group and individual sessions in a timely manner so members and office staff stay on schedule
Work with the office staff to maintain a clinical schedule that is mutually beneficial to provider and the office;
Stay familiar with all Recover Together policies, procedures, reference materials, contracts, etc.
Follow up on interprofessional communication
Assist with onboarding, training, and support of new providers.
Work with the medical leadership team to pilot new initiatives to improve outcomes during early treatment,
Assist with other duties as assigned.
Qualifications:
Nurse Practitioner with Active, Unencumbered Indiana APRN License and DEA.
Board Certification and CSR Registration in Indiana.
Willing and able to become licensed in multiple other states where Groups operates or will operate soon (with support and reimbursement from Groups)
Willingness and ability to become credentialed with all government and private health plans with whom Groups Recover Together is in network.
Ability to see member in-person at several state locations, if required.
At least 1 year of experience managing OUD with buprenorphine, preferably within the Groups Recovery System.
At least 1 year experience working in a team-based-care setting with behavioral health providers (therapists, social workers, case managers, etc)
At least 2 years experience working with a similar member population, preferably within primary care of behavioral health setting.
Adequate internet connectivity to support telemedicine (if working from home)
Access to reliable internet and telephone services, specifically 10M download and 5M upload package or higher, as well as a strong WiFi signal from your remote work location.
Ability to demonstrate recovery-sensitive behaviors, including choice of language, attitudes and interactions with members and staff.
Ability to maintain courteous and professional in all communications (verbal and written) with staff and members.
Ability to provide in-person services at our Scottsburg and Salem locations weekly, if required.

Responsibilities

The Wellspring Post Arrest Diversion (PAD) Team supports adults with serious mental illnesses who are involved with the justice system and have been recently incarcerated or are at imminent risk for being incarcerated.
The PAD Peer Specialist participates in regular team meetings and works in partnership with the PAD Case Manager, Clinician, and Program Manager. Working as part of an interdisciplinary team, the PAD Peer Specialist will assist clients with the coordination of court dates and collaborate with the Office of Probation and Parole and HIP. The PAD Peer Specialist also assists clients in gaining access to medical, social, educational, housing, addiction treatment, and other resources ensuring uninterrupted provision of all services. The PAD Peer Specialist completes electronic documentation of services demonstrating medical necessity within 24 hours of client contact.
Special Skills and Requirements:
The PAD Peer Specialist will identify as a mental health consumer and must be willing to share their personal experience of recovery and support others in need. The PAD Peer Specialist will have a belief in recovery from mental illness and an enthusiasm for helping persons with disabilities, rooted in empathy and compassion, demonstrating a careful and caring attitude.

Summary
The Direct Service Provider (DSP) will assist in implementing training programs for ADT participants with autism and related disorders, working under the supervision of the ADT Group Skills Supervisor and Director. This role involves direct support to participant in achieving goals outlined in their Person-Centered Care Plan.
Essential Duties and Responsibilities - including but not limited to:
Provide person-centered care, ensuring the needs of participants are met with respect to their specific requirements such as allergies or level of supervision.
Implement daily group schedules with a minimum of 5 clients per session, with groups potentially including up to 8 clients.
Model positive instructional skills, including appropriate praise and reinforcement.
Assist in creating and implementing training materials and leading programs for participants under the direction of the ADT supervisor or Behavior Analyst.
Ensuring that all scheduled activities and services are delivered as planned for the benefit of the individuals in the program.
Maintain participant daily records as directed by supervisor, including recording behavior data, graphing, daily summaries and tracking the progress of target skills.
Complete daily communication logs and necessary documentation by deadlines.
Collaborate effectively with all BCA team members, including but not limited to ADT Supervisor, Behavior Analysts, and the Clinical Director.
Ensure all data/billing documentation is submitted accurately and timely.

Ensure DSP staff have the necessary skills and knowledge to provide exceptional care to individuals and are following agency policy. Be available to DSP staff by telephone as needed for technical assistance while they are on shift.
Ensure a high level of customer satisfaction among BluWell customers, including individuals, family members, guardians, case managers, and other agencies.
Ensure each individual is being supported by DSP staff in a manner that is consistent with his/her individual needs, preferences, and desires. Ensure each individual's plan of care is being carried out in the residential setting.
Conduct random house inspections as designated by the Residential Coordinator. Provide required documentation to the Residential Coordinator from these checks. Be willing to correct, in a positive manner, the problems that have occurred or been noticed and accept responsibility for these actions. Hold staff meetings and in-service trainings to ensure that problems are addressed and corrected.
Monitor all medications in any staffed residence, even if a client self- medicates. Report to Nursing Department all medications needing to be refilled, or new to the home. As well as make sure all scripts are current and exactly match what the medication given is. Continually check and monitor MAR's for correct printing and doses. Residential Team Leader must review medication counts and ensure that individuals will not run out of medication at any time. Be sure that there is a refill coming or call in a refill to ensure not running out. Ensures a minimum of five (5) days of medications are present in the home at all times.
Make and transport to doctors' appointments according to when needed. Transporting to appointments may be designated to appointment staff, but the Residential Team Leader has the responsibility of overseeing this and ensuring all appointments are made and kept. This includes medication refills, as well as physicals and any routine appointments. After doctors' appointments are completed, ensure prescriptions are placed in client files and a copy give to the Nursing Department, and ensure the pharmacy has the scripts to fill and deliver. Ensure medication is delivered to the home and given as directed by the physician.
Be aware of all medical needs of any individual residing in the home, as well as be sure their medical needs are taken care of properly and timely. This includes ensuring DSPs receive training on specific medical conditions and equipment related to the individual.
Attend all scheduled Team Meetings for individuals living in assigned homes. Residential Team Leader input is essential to team meetings.
Serve as primary contact for outside agencies and families for the individuals in assigned homes. Return phone calls and emails in a timely, professional, and courteous manner. Remember that you are representing BluWell of Kentucky and that your behaviors directly reflect on how others perceive BluWell of Kentucky
Be able to train all staff on policies of BluWell of Kentucky DSP roles in the house and the duties expected on shifts, and any documentation needing to be completed in the home.
Follow-up and makes sure DSP staff is completing all documentation correctly. If not completing documentation correctly, hold an in-service with all staff in the home and have all staff sign in. Make sure that all staff are aware of the correct way to fill out documentation before ending the meeting or in-service.
Hold monthly house meetings to ensure all staff in the home are able to express interests, problems occurring with other staff, individuals, or questions they might have. Turn in training documentation to the Human Resources and Training Coordinator on the date, length of training and topics covered in the meetings and ensure all DSP staff in attendance have signed the sign-in sheet.
Alert the Residential Coordinator and Executive Director of any major concerns or repeated problems occurring so that they can address the issue at a different level. This will occur after you have tried to address the situation yourself.
Turn in all documentation no later than the 3rd of the following month. This will include water temps, outcome sheets, attendance log, MARs, etc. Any incident reports need to be turned in the next business day. Ensure all incident reports are turned in on time and are completed correctly by the staff before submitting to the office.
Complete a monthly residential summary for each individual in assigned houses no later than the 15
th
of each month for the previous month. Provide case managers and guardians with copies of residential summaries as requested.
Ensure each individual's office and home binder is complete, up to date, and contains all documentation required by Medicaid wavier regulations and agency policy.
Attend all trainings requested by BluWell of Kentucky. This could include DDID trainings, BluWell of Kentucky trainings, individual specific trainings or management skills training. As a part of trainings, you will also need to hold a training session in the homes of staff for individual specific or office specific items.
In the event of a call in or no show, Residential Team Leader should first try to find someone employed and experienced with the individual to cover the given shift. If no one can then Residential Team Leader will report to duty.
Grocery shopping will be completed weekly. It is the Residential Team Leaders responsibility to make sure that there are always adequate supplies in the house. This includes toiletries, cleaning supplies, food, drinks, and medical needs.
The Residential Team Leader will report corrective active needs to Program Director and Human Resources and Training Coordinator for investigation and appropriate discipline. This is to include reporting of corrective actions for staff that are inappropriately conducting themselves (Including but not limited to: not following job descriptions, acting inappropriately at work, sleeping on the shift, failure to show up on a shift, etc.). Complete staff supervision forms for all on-the job coaching provided to staff and submit to the Human Resources and Training Coordinator.
Respond to crisis situations as needed or instructed by the Residential Coordinator.
Create a staffing schedule for your assigned homes and ensure staff are available at all hour's individuals are in the homes. Obtain approval for all overtime from the Residential Coordinator
Any other duties assigned to you by BluWell of Kentucky. This includes following all directives and requests of the Executive Director and Residential Coordinator.

POSITION DESCRIPTION:
Mental health Peer Support Specialists serve as symbols of hope that recovery is possible for other consumers who are struggling with mental illness. The fact that “they have been there” and can relate to many of the feelings and experiences of those they work with gives them a power and authenticity that make them an invaluable members of the care team. The primary function of the Peer Specialist is to facilitate the recovery of those with whom they work. Their job duties are as diverse as the population they serve but include peer counseling, life skills and illness management skills coaching and assistance, support in attending 12-step meetings and step work, development of social networks, and assistance with community navigation/engagement.
EDUCATION REQUIREMENTS:
High School degree / GED is required.
Adult Peer Support Specialists must complete 30 hours of state approved training with an approved curriculum and pass a state approved written and oral exam. Core components of the curriculum for Adult Peer Support Specialists (30 hours) include: problem solving; Wellness Recovery Action Plan; stages in the recovery process; effective listening skills; establishing recovery goals; and using support groups to promote and sustain recovery.
EXPERIENCE REQUIREMENTS:
A lived experience of mental illness or co-occurring disorders (mental illness and substance use disorder) and a desire to assist others in their recovery process. Regular supervision is required and each qualified Kentucky Peer Specialist must earn at least 6 hours of continuing education each year.
SALARY:
$15 - $18 / hour

A mental health Peer Support Specialist is or has been a recipient of mental health services whose personal experiences of recovery provide a level of authenticity & understanding of the client's experience that is unique. Their personal experience with mental illness, substance abuse, the system of care, families, social communities, etc. coupled with specialized training & self-sufficiency serve as the foundational requirements for the position.
A valid drivers license and a personal vehicle (driving between work sites and transporting clients) are essential for this position.
