The RT will utilize all the resources available within the agency and community to accomplish care objectives. Maintains proficient knowledge of ventilator patients including compliance software, new technology, units, and supplies supported by Adapthealth.
Ability to demonstrate and instruct on use of vent units and supplies. Ability to make decisions for patients based on compliance data and assessment.
Communicates with team, physicians and referral sources and other patient agencies ensuring prompt attention to patient care issues.
Maintains working knowledge of Medicare/Medicaid and other third-party payer guidelines related to ventilation.
Electronically documents patient care activity, intervention provided and all communication regarding the patient. Documentation is accurate, complete and follows company standards.
Appropriate steps taken to ensure recommendations and orders sent are acknowledged and followed up in a timely manner.
Responsible for accuracy, clarity, and timeliness of verbal and written communications as it relates to role.
Responsible for documentation that supports data collection to track and trend outcomes.

Job Description
Is this a good fit for you? We are looking for candidates who:
Love Animals! You don't need prior animal work experience, but should enjoy providing companionship to dogs, cats and other little critters.
Enjoy being outside in all four seasons and the weather that comes with them! (Rain, snow, heat, cold, wind...)
Live near our service area: Because Heaven Sent does not compensate for travel time/commute, we are mainly interested in candidates who live in or near (within 10-15 miles) one of our service areas (Wilmette, Evanston, or Kenilworth).
Provide excellent customer service through personalized notes, photos, and messages
Enjoy walking and are able to handle the activity level of walking dogs for 3-5 walks per day (1½ - 2½ hours). You must be able to do so with a focus on safety
Have strength to safely control dogs of all sizes and breeds. (It's a plus if you can manage 2 dogs at a time (from one household))
Qualifications
Requirements:
OVERNIGHT STAYS: must sleep over in client's home with client's animals from 7pm-7am (at a minimum).
Available Holidays: be available to house-sit during the entire week for at lease 2 of our “Premium Holidays” (Thanksgiving, Christmas/New Year's and Spring Break)
Equipment Needed: (1) smartphone with data plan and (2) reliable insured car (preferred)
Background & References: must (1) be at least 21 years old; (2) provide personal and professional references; (3) submit previous employers for verification; and (4) willing to submit to background check if a position is offered
Pet Care: able to manage (1) all pet feedings & medications; (2) other tasks such as bringing in mail/newspaper, plant care, cleaning up messes (ie: hairballs, accidents), and (3) keeping the home in order while clients away.

A TYPICAL DAY FOR YOU AT HH:
Communicate all individual/client needs to supervisor including schedules, medical, behavioral, and family needs.
Provide supervision to, and actively engage individuals while training and supporting through individualized plans in daily living, community integration, leisure, and work skills either individually or in a group setting.
Assist in the development of curriculum/lesson plans, implement and present lesson plans to individuals in class settings through various teaching methods.
Assist with ADLs (Activities of Daily Living), feeding, and toileting as per the individualized need of each person served.
Assist with welcoming, monitoring and transitioning of new clients into the program.
Implement and document individual training objectives.
Participate as a member of the individuals interdisciplinary team and attends team meetings/staffing as required.
Complete all paperwork, required documentation, and conduct emergency drills as assigned.
Assist in teaching, training and supporting new team members on shift as they learn their new role.
Cohesively work with and support volunteers within program setting.
Attend all team and agency meetings and complete training requirements as scheduled.
Set up programming areas in the most effective manner to maximize training potential and maintain assigned space in a clean and orderly manner.
Monitor individuals throughout the shift, remaining awake and alert.
Implement activities and recreation per activity calendar.

Qualifications

Education:
High School Diploma or GED required.
Experience:
Ability to fulfill all necessary training requirements of the agency, including but not limited to crisis management (crisis management training includes de-escalation strategies and physical management techniques).
Proficient and comfortable with technology such as Microsoft Office, Teams, Zoom, etc.
DSP Certified preferred. If not already DSP certified, the ability to complete DSP training within 90 days of hire.
Experience working with intellectual and developmental disability population (preferred).
Skill Sets:
Possess problem-solving and time management skills.
Sound organizational skills and ability to multitask while prioritizing multiple urgencies as required.
Present interpersonal communication skills, both verbal and written.
Valid driver's license with acceptable driving record and ongoing motor vehicle insurance (preferred).
Must be able to apply for PACE certification.
Ability to work in a fast-paced environment and the flexibility to adapt to changes throughout the shift.
Flexibility to work at any assigned HH site.
Must be able to maintain client confidentiality at all times.
