Reporting to the Lead Physician, the Advanced Practice Provider (Nurse Practitioner or Physician Assistant) performs comprehensive medical services for members of families within scope of training. This includes preventative care, immunizations, primary care and treatment, prescribing of medications, ordering and interpreting lab testing and physical examinations.
Responsibilities:
Provide routine, acute, urgent, preventive, curative, and rehabilitative medical care to members of families, within the approved skills profile and/or scope of practice.
Take detailed histories and perform complete physical examinations.
Order or execute various tests, analyses and diagnostic images to provide information on patient's condition.
Analyze reports and findings of tests and examination and diagnose condition of patient.
Make diagnoses (or differential diagnoses if a specific diagnosis is pending) for each patient, educate the patient and parents on the diagnosis, and institute treatment and follow-up care based on current standards of care, official guidelines, and evidence based medicine.
Administer or prescribe appropriate pharmacological agents and treatments.
Discuss any possible side effects to medication or immunization with patient.
Qualifications:
Graduate of an accredited School of Nursing or Physician Assistant Program
Master's Degree from an accredited Nurse Practitioner Program approved by the California Board of Registered Nursing.
National Provider Identification Number (NPI).
Computer proficiency, particularly with Electronic Health/Medical Record systems.
Current and unrestricted California Nurse Practitioner or Physician Assistant License
Board of Registered Nurses Certification Furnishing Number issued by California Board of Registered Nursing
Current and unrestricted Controlled Substances Registration Certificate from the U.S. Drug Enforcement Administration (DEA)
Valid California Driver's License, proof of auto liability insurance and clean driving record
Preferred Qualifications:
Bilingual in English and Spanish (may be required at some clinics).
Experience with Epic EMR.
At least two (2) years of clinical practice experience, outpatient, family medicine.
Experience in providing medical care to underserved populations.
Current certification with Pediatric Advance Life Saving (PALS) and/or Advanced Cardiac Life Support (ACLS).

The pay range for this role $140,000 to $220,000 on an annual basis.

Our Goal
The goal of the Direct Care Staff is to provide quality active programming and positively influence the well-being of all clients served in the residence. The DCS provides direct services to the clients to meet their functional health, safety, educational, and psychosocial needs.
Responsibilities
To ensure the supervision, safety, and well-being of the clients at all times.
Provides the continuous active treatment program for each client, including all informal and formal, training and service objectives for each client, ensuring the implementation and documentation of the ISP of each individual client and the facility group.
Participates in the development and evaluation of the Individual Service Plans (ISP) directed by the QIDP, Administrator and Facility Manager.
Monitors and supports the client's health and well-being and reports observation to the QIDP, Administrator and Facility Manager. The DCS shall ensure that health information is documented and brought to the attention of the RN consultant/ Health Coordinator.
Ensure the daily documentation of client status, behavior, and treatment responses.
Participates in the provision and maintenance of a clean, safe, healthful, and nurturing environment.
Provides quality training and positive interaction with residents at all times.
Adheres to and implements the facility's active treatment philosophy and goals, and the organizational policies and procedures.
To ask questions and learn as much as you can from those around you.
To promote learning, growth, socialization and independence of the residents.
To develop relationships with other Hope House departments, staff and external agencies.
To go above and beyond for the clients and staff with which you interact.
What We Need From You
You need to be at least 18 years of age.

ABOUT THE POSITION:
Familias First is looking for dedicated individuals to join our team. This is a part-time position with a minimum of 15 hours per week, however; full time position may be available depending on your availability and client needs. The successful candidate will work closely with the client and parent by providing 1:1 ABA therapy. The candidate will work independently following a Behavior Intervention Plan for the client, Take data on client progress & Teach parents ABA techniques.
This is an entry level position, and no previous ABA experience is required. The candidates will go through a month-long paid ABA training. Training includes the 40-hour modules & RBT certification, CPR, CPI training, hands-on practice/role play of ABA interventions.
