1. Provides services in order to maintain required productivity/billing standard set by the department.
2. Meets deadlines and ensures accuracy of various reports/paperwork, mileage sheets, and electronic timesheets.
3. Maintains accurate and timely documentation of service provision. Completes progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.

6. Provides support/mentoring to child, teaching self responsibility and goal achievement.

Essential Duties and Responsibilities
Provide a safe, clean, and nurturing environment that meets and/or exceeds all requirements set forth by Heartspring.
Provide a structured environment for students by following the schedule of activities that promotes the student's learning opportunities and their independent living skills.
Implement and document the students' training programs as prescribed in the Individualized Education Plan (IEP) or informal training programs.
Implement, document, and provide input into the Student Support Plan (SSP), as outlined by the psychology department.
Implement and document student-specific training in specialized areas, as prescribed by the students' teams (i.e., Occupational Therapy, Physical Therapy, and Speech Therapy).
Escort students to off-campus events, including special outings, field trips, and healthcare appointments.
Perform and assist with non-violent behavior management system for crisis intervention as needed.

POSITION SUMMARY
The PRN Direct Support Professional (DSP) is much more than a caregiver. Major Duties and Responsibilities
Implement Person Centered Support Plans for assigned individuals.
Assist with daily living skills including, but not limited to, meal preparation, general hygiene, toileting, general housekeeping, work and leisure activities.
Administer medication and treatment(s).
Complete documentation as required through the general course of duties.
Report to the appropriate Program Coordinator, Program Manager, Case Manager or Wellness Liaison any occurrences that affect the immediate well-being of the individual.
Attend all meetings as assigned and required.
If approved as a driver of agency vehicles, employee may transport individuals.

Qualifications
Knowledge and Skills:
Experience: Prior direct care experience preferred, but not requested.
Education: High School diploma/equivalent preferred.
Interpersonal Skills: Must be able to communicate effectively with all levels of personnel and individuals served. Must demonstrate compassion for individuals with disabilities.
Other Skills: Basic mathematical, grammar and computer skills preferred. Valid drivers license preferred.
Physical/Environmental: Must be able to lift, push and/or pull a minimum of 50 pounds, stand for periods of time, bend, push wheelchairs, and provide physical intervention on a regular basis. Noise level may vary, depending upon duty assigned.
Due to the potential intimate nature of care and physical proximity involved with some of those receiving HCBS funded I/DD services through Disability Supports, and in order to promote a safe work environment for both the caregiver and those for whom we care, you may, from time-to-time, be required to wear PPE, (Personal Protective Equipment). If PPE is required, Disability Supports will provide it.
