$20.75 / hour + $4 differential for Flex Team
On a flexible, as-needed basis, provides clinical services to all residents residing at Communities of Christian Living Communities (CLC). These services will include assisted living neighborhoods, and all shifts. Coordinates care and services based on resident needs and individual preferences as determined by ongoing assessment and evaluation. Performs duties with education-based expertise, to provide a holistic and person-centered model of care for residents.
Flex team members are expected to fill-in for a minimum of 2 shifts per month. Most of these shifts are 12-hour shifts. This position is considered PRN status and employees may sign up for multiple locations, dates, and shifts. With these expectations, the Community Staffing Team will communicate closely with team members toward meeting their preferences and CLC's needs.
Essential Duties
Attends to the personal care needs of residents in a timely manner and helps with ADLs as requested by residents or directed by supervisor.
Provides personal care services to residents such as assistance with bathing, dressing, attendance at
meals, laundry, etc. with attention to individual resident needs (i.e., hair is combed, shoes are tied,
oral hygiene, etc.)
Anticipates and identifies any resident care needs and reports to supervisor (i.e., hygiene needs, broken eyeglasses, skin rash, lost hearing aids, supply needs, etc.).
Responds to the needs of family members as appropriate
Maintains open communication with supervisor and/or peers to promote awareness of resident issues.
Monitors the safety and well-being of residents
Reports suspicion of abuse, neglect or misappropriation of resources as required by regulation.
Reports changes in resident status, health or otherwise, to supervisor.
Monitors and documents resident's condition as directed.
Assists residents with prescribed daily medication routine as appropriate and in accordance with state law.
Alerts RN/LPN and/or supervisor of any questions or concerns with medication records/supply.
Ensures that proper documentation occurs timely including any refusal of medication and/or treatment.
Restocks medication carts, drawers and/or storage areas with miscellaneous supplies only (i.e., gloves, sanitizer cups, etc.)
Attends, participates, and contributes to monthly staff meetings addressing resident and departmental needs
Other duties as assigned.
Qualifications
Basic Qualifications & Experience
Per the state of operation requirements, Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation
Must be able to read, write and speak the English language.
High school or Equivalent (GED)
One (1) year experience in some level of elder care preferred
Proficient computer skills preferred
Strong self-management and organization skills
Strong multi-tasking abilities with the ability to handle competing deadlines, flexible and adaptable (Open to change).
Ability to effectively handle difficult and sensitive issues and maintain confidentiality
Working Conditions / Physical Requirements
Moderate physical activity required by handling objects up to 50 pounds occasionally and/or up to 20 pounds frequently.

Essential Duties:
* Provides milieu management, guidance, safety, stability, and direction to clients in early recovery from a trauma-informed lens.
* Maintains acceptable attendance and punctuality and is consistently at work on time.
* Interacts with client population to provide guidance, safety, stability, and direction including de-escalating potentially dangerous situations.
* Performs accurate and timely documentation at all times, including but not limited to admissions, shift notes, Per Diem notes, discharges, inventories and screening of clients' personal belongings, individual and group sessions, Medication Occurrences (MOR), and Adverse Incident Reports (AIR).
* Monitors and documents client's vital signs, physical condition and behavior, as needed. Must be able to administer blood pressure assessments, utilizing blood pressure cuffs, when Nurse is not available.
* Assists with the planning, conducting and supervision of patient activities, including community meetings, groups, external meetings on-site, family/friend visitation, recreational activities, and life skills.

* Assist in conducting Random Urine Drug Testing.
* Facilitate or co-facilitate psychoeducation groups.

The Business Services Professional provide operational management support to the Department of Radiology. This position requires a working knowledge of all current University policies that are relevant to business services which may include, but not be limited to procurement, human resources, budget, events, and travel. This position supports the daily business needs of the department and faculty. Tasks assigned to this position require critical thinking and the ability to independently resolve noted discrepancies. This position will work with section chiefs on clinical scheduling in Qgenda. This task requires organization, focus, analytical, and technical skills. The position is responsible for maintaining accuracy in Qgenda reporting. This position will provide support for faculty promotions and tenure. This requires keeping the timeline of midcourse review and promotion procedures moving forward. This position reports to the Administrator for HR & Finance in Radiology. Individual Related Duties:
Supervise and assist residence in planning and completion of daily, weekly and monthly routine such as laundry, hygiene, meal planning / preparation and activities, ensuring full participation of residents, as per individual need and according to their SP and ISSP, completing documentation as per agency guidelines.
Implement and document Individual Service and Support plans, safety plans, and behavior management plans as stated, documenting after completion using progress notes and data collection forms specified per individual.
Be aware of individual behavioral issues, providing appropriate intervention, counseling, and follow-up for unusual behaviors or reactions as needed. Document unusual behaviors/reactions in case file and using behavior tracking and/or incident report form as per agency guidelines.
Provide transportation to and from the community as per individual need and as assigned, including recreational activities, medical appointments, shopping, or job/day program as assigned.
Promote community integration in use of public transportation, use of good pedestrian skills, knowledge of neighborhood, and use of public transportation whenever possible.
Participate as a member of Interdisciplinary Team, attending meetings and completing skills assessments as assigned, giving input regarding observations/recommendations of individual in service progress and needs.
Assure that resident gets needed medical attention by documenting medication administration, familiarizing self with individual medical concerns, supervising appointments, documenting all medical visits with consultation sheets, and any medical concerns in individual notes as per agency guidelines, responding to emergency situations as per Emergency Data Form in individual file.
Maintain inventory of individual possessions on an annual basis, or as possessions are obtained or disposed of and as requested by supervisor.
Evaluate individual skills using evaluative tools prior to staffing or upon request.
Maintain in-house case and medical files, ensuring that all individual case and medical information is up to date.
Assist with family visits, participation, and requests in a polite and helpful manner reflective of agency philosophy, referring family questions or problems to supervisor and following through on requested actions within designated time.
Cooperate with and support vocational staff or day program placement including transportation or related training to and from program as requested, ensure needed supplies for individuals are provided (change of clothes, medications, money, lunches, etc.), submitting needed leave request forms to appropriate program prior to medical or vacation absences, maintaining good public relations and communication with day program, and notifying CCA office of questions or concerns.
Facility Related Duties:
Providing guidance and assistance to residents to ensure that units are maintained in a clean and healthy manner, meeting guidelines on facility checklist on ongoing basis, and assisting with plan of correction in deficient areas.
Conduct and document monthly fire drills using specified procedures and fire drill log, assisting residence in increasing independence in evacuation skills and general safety knowledge.
Notify supervisor of maintenance or household supply needs, obtaining such needed supplies as are stocked at the CCA office prior to end of shift, and submitting a list of such supplies as are not stocked to office.
Meet resident nutritional needs by assisting them in development of balanced, written menus twice monthly, making substitutions within food groups and recording changed same day on menu. Encourage appropriate portions and healthy snacking habits as per dietary orders, and ensuring residents participate in regular grocery shopping to increase knowledge of available foods. Direct Support Professionals will sit at the table with residents during mealtimes to model table manners, monitor portions, and provide any assistance needed.
Maintain daily log and information exchange with other staff by recording events of the day and needed communication on daily log sheets, dating and signing each entry, and by reading and initialing log daily and at shift change. All entries are to be legible and professional in nature.
Agency Related Duties:
Record actual hours worked daily on specified timesheet as per agency guidelines, having available for supervisor approval on last day of pay period or as otherwise directed. All leave requests must be submitted in writing and approved by supervisor prior to leave, as per agency guidelines.
Record all mileage using specified forms as per agency guidelines and submitting with time sheet on last day of pay period for approval.
Attend staff meetings and scheduled inservice as requested by supervisor and meeting agency inservice requirements, completing all assigned tasks within designated time frames.
Be present in unit during scheduled working hours excepting scheduled medical appointments and agency related duties to ensure that individual emergencies, medication pick-ups, and interagency communication can be handled. This may include providing coverage for individuals at other units as requested.
Obtain residential personal needs purchases and assist with banking and maintaining individual personal needs funds according to needs or request of resident and based on availability of individual funds; always obtain a receipt and completing in-house receipt at time of purchase; completing ledgers per agency guidelines, counting monies daily or at beginning/end of shift, notifying supervisor immediately of discrepancies.

* Community Connector: This service is used to develop abilities and skills necessary to participate in community life, such as community education, training, volunteer activities, etc. Family Services DSPs will help the individual to build and develop relationships and natural supports in the community. These services include socialization adaptive skills, and personnel to accompany individuals in the community. Community Connector enables the individual to utilize the community as a learning environment through conducting services in a variety of settings.
* Youth Day Services: This service is specifically designed for individuals ages 12-17-years old. This service is specifically designed to provide care while the primary caregiver works, pursues education, or volunteers. This service is capped at 10 hours per calendar day.
* Enhanced Homemaker: This service is used to increase the individual's skill levels with cooking, cleaning, and laundry.
* Personal Care: This service provides support to the individual for them to maintain or increase their skill level with activities of daily living. This includes, but is not limited to, bathing, dressing, feeding, toileting, routine assistance with medical appointments, budgeting, meal planning, grocery shopping, etc.

Primarily sedentary physical work requiring the ability to lift a maximum of 20 pounds; occasional lifting and carrying; frequent walking and standing. Sufficient clarity of speech and hearing or other communication capabilities that permits the employee to communicate effectively on the telephone and in person. Sufficient vision or other powers of observation that permits the employee to review, evaluate and prepare a variety of written documents and materials. Sufficient manual dexterity that permits the employee to operate computer equipment and other office equipment. Sufficient personal mobility and physical reflexes, which permits the employee to work in an office setting. Work may include extended periods of time viewing a computer video monitor and/ or operating a keyboard. Ability to work under stress from demanding deadlines, public contact, and changing priorities and conditions.
Work Environment
Works primarily in a clean, comfortable, indoor environment. Works in a setting subject to continuous interruptions and background noises.
