Job SummaryProvides safe, ethical, and competent evidence-based nutritional therapy that meets the standards of practice and the scope of practice as stated by the Academy of Nutrition and Dietetics. Manages the individual's nutrition care in collaboration with the health care team and other nutrition professionals.
Job Duties
Assesses individual needs through synthesis of information obtained through the assessment interview, food and nutrition and related histories, review of the medical record, and discussion with other members of the interdisciplinary health care team.
Develops a plan to meet assessed needs including recommendations regarding: the composition of diet and meal patterns and supplements; and the volume, rate, and composition of parenteral and enteral feedings.
Confers with the health care team to develop goals related to patient care.
Plans and coordinates the use of special dietary regimens.
Documents initial assessment plans for nutritional care and progress relative to same in the medical record in accordance with established standards.
Evaluates nutritional status of individuals and makes recommendations regarding provision of optimal care.
Counsels individuals, families, and care provider on normal nutrition and therapeutic diets with consideration given to the individual's educational, cultural, and socioeconomic background.
Plans and conducts group classes for individuals in various areas of nutrition as appropriate.
Writes nutrition related orders for hospitalized patients in accordance with the Clinical Nutrition policy.
Consults, educates, and advocates on behalf of individuals, groups, and special populations regarding food, culinary, nutrition and health issues, food security and insecurity, nutrition programs, nutrition wellness and resources.
Plans for continuing nutrition care for individuals, as needed, after discharge from the hospital using ancillary resources.
Contributes to and designs referral process and systems to facilitate public access to Registered Dietitians.
Receives referrals for services and make referrals to other relevant health care professional.
Implements outcome-based quality assurance and performance improvement activities to document outcomes of service; compliance with regulations, policies, and procedures; and monitor and address customer satisfaction.
Self-evaluates results and monitors for trends and changes in productivity related to workload.
Compiles statistics and completes Clinical Productivity worksheets on a timely basis.
Creates, evaluates, reviews, and revises patient instruction materials and recipe and food products for the patient menu.
Actively participates on appropriate committees for the organization and represents the Clinical Nutrition department in decisions made by these committees.
Participates in planning, implementation, and evaluating research studies conducted by members of the Clinical Nutrition Services or nutrition-related research coordinated by other departments, as available.
Develop and use electronic information management tools for practice, research, and education.

We are seeking a passionate and experienced Licensed Dietitian to join our virtual Intensive Outpatient Program with the ability to practice in Oregon, with a focus on supporting individuals in our new eating disorder-specific track. Responsibilities
Provide individual nutritional assessments and personalized meal plans as part of a holistic approach to care for clients struggling with eating disorders and related mental health challenges.

Strong knowledge of eating disorder treatment, including Anorexia Nervosa, Bulimia Nervosa, Binge Eating Disorder, and Avoidant/Restrictive Food Intake Disorder (ARFID).
Experience providing virtual care and/or telehealth services preferred.
Familiarity with evidence-based practices such as Health at Every Size (HAES), Intuitive Eating, and Cognitive Behavioral Therapy for eating disorders (CBT-E) is a plus.
Ability to work collaboratively in a multidisciplinary team environment.
Proficiency with cloud-based tools such as Google Suite, Slack, Zoom, and EMR software.

The total target base compensation for this role will be between $43.00 and $55.00 per hour at the commencement of employment. Your Role
As a Dietitian with Evergreen Nephrology, you play a pivotal role in managing the nutritional aspects of patients enrolled in Evergreen Nephrology's Care Management program. You will provide the best clinical care for your patients, focusing on creating a dietary care plan that is integrated into the overall care plan. You leverage an integrated technology platform and are complemented by an entire team of professionals including Nurse Practitioners, Nurse Care Managers, Care Coordinators, Pharmacists, Licensed Clinical Social Workers, Population Health Social Workers, and Psychiatrists.
This will be a remote position, but it will support patients in Central, Mountain, and Pacific Time Zones.
Primary Functions
Collaborate with nephrologists and other healthcare professionals to develop and implement individualized care plans that address the unique dietary needs of patients with kidney disease while aligning seamlessly with overarching care
Serve as a key member of the interdisciplinary care team to follow a team- identified panel of patients until their needs have stabilized by delivering care interventions to patients or their care givers via telephone or other
Support high risk, high need patients and their families by providing comprehensive diet education and counseling based on individualized patient needs including nutrient requirements, knowledge abilities, chronic medical, economic, and social conditions.
Provide specialized dietary counseling and education to patients and caregivers with CKD and ESRD, while focusing on managing nutrition intake to slow disease progression and improve outcomes.
Incorporate preventative care and early interventions while supporting interdisciplinary care team in executing customized care plans to reduce hospitalizations and improve long-term outcomes.
Develop and deliver educational materials and methods appropriate for patients to meet their unique dietary needs.

Responsibilities And Duties:
Performs nutrition assessment using age-appropriate standards, utilizing validated assessment tools and nutrition focused physical assessments. Includes documentation of nutrition related diagnosis when indicated
Demonstrate proficiency in accordance with Standards of Practice and Professional Performance for Registered Dietitian Nutritionist while providing age-appropriate plan of care for patients and families. Coordinates nutrition therapy with physicians and other health care professionals. Participates in multidisciplinary rounds and care planning sessions. Plans, implements, and adapts nutritional care plan to meet physician orders, age, and disease specific physiologic needs. Intervention plan includes appropriate feeding route, mode, and nutrition prescription. Monitors and evaluates patient response to medical nutrition therapy using specific goals and determines change if indicated. Performs patient, family, and care giver education related to nutritional therapy and incorporates best method of learning style and communication into sessions. Educates and trains interdisciplinary team members, students, and interns. Fosters teamwork and establishes effective strategies to implement innovative programs which leads to optimal clinical nutrition and financial outcomes. Supports quality improvement, research, and outcome studies. Identifies and communicates opportunities for waste reduction and service improvements. Documents accurately and completely, including appropriate utilization of order writing privileges in electronic medical record. Remains current on research developments and other methods of practice and incorporates this into current programs and services. Supports the development and revision of nutrition related department tools, training materials, policies, and procedures. Participates in the evaluation of clinical nutrition products and equipment. Complies with all regulatory standards, including federal, state, and accrediting agencies and adhere to facility confidentiality, HIPAA regulations, and patient rights policies. Participates in and attends departmental meetings, team development, and professional programs, as appropriate. Serves as an active member on multidisciplinary teams and/or committees. Ensures national program standards are met and always maintained for certified and/or recognized programs, as applicable. Reviews research/peer-reviewed publications and practice recommendations and modifies practice as needed. Other duties as assigned.
