At the request of the Contractor agrees to provide secure, qualified Dental professionals (Dentists and Dental assistants) to work at the Dental Clinic and provide dental services to incarcerated juveniles. The Contractor will provide a fully certified Dentist with a South Carolina license in good standing and an x-ray-certified Dental Assistant. The personnel assigned to DJJ by the Contractor are employees or independent contractors of the Contractor. All matters concerning wages, expenses, hours worked and, paid working conditions, and other similar administrative matters shall be resolved between the Contractor and Contract personnel, not between Contract Personnel and DJJ. 2.The contractor agrees to provide personnel who were previously terminated for cause. Furthermore, the Contractor will notify DJJ before assigning any Contract Personnel who are former employees of DJJ. 3.The Contractor agrees to provide dental clinic services at a minimum of three days per week, four hours daily (from 8:30 AM to 12:30 PM) on Tuesdays, Wednesdays, and Thursdays. The contract dentist's hours will be 12-15 hours weekly (depending on workload). The Contracted Dental Assistant's hours will be 30 hours weekly, three days per week, 10 hours per day. 4.The Contractor agrees to provide the following dental services during clinic: These services will include dental exams, oral hygiene instruction, cleanings, x-ray (bitewing and panoramic), dental restorations, simple extractions, and other basic dental procedures not requiring sedation or general anesthesia. The Contractor will prioritize dental services in order to attend to emergency and urgent cases and patients dealing with pain in a timely manner. The Dental Assistant will also perform all administrative clinic functions, including scheduling appointments, ordering supplies, and arranging for medical equipment maintenance. 5.The Contractor agrees not to provide services for maxillofacial reconstruction, mandibular fracture reconstruction, cosmetic/experimental procedures, orthodontic appliances, or partial plate replacement. The Contractor will not provide any care requiring sedation, anesthesia, or dental lab services (root canals/crowns). The Contracted Dentist will refer all cases requiring additional dental care not within their scope of care to external dental specialists or oral surgeons within the local area, as identified. 6. The contractor agrees to provide on-call "after-hours" coverage for dental emergencies as part of comprehensive services. The Contractor's Dentist agrees to respond to calls from DJJ nursing and medical staff. 7. The Contractor agrees to maintain and provide to, upon initial employment (and annually thereafter), a completed, updated personnel file on each Dental Professional, to include the following: a. Documentation of all current licenses and special certifications. b. Provide documentation of current and annual (if applicable) immunization status and/or titer for Hepatitis B, MMR, Varicella, Tdap, influenza and COVID-19. c. Documentation of a negative Tuberculin Skin test (TST) current within 90 days of initial employment, or medical documentation of a normal chest x-ray from a licensed physician, and annually thereafter. d. Current CPR/AED certification from client, American Heart Association, or National Safety Council. e. Criminal background check within 30 days of initial contract and annually thereafter. f. Proof of eligibility to work in the U.S. g. Valid S.C. Driver's License. h. A DSS check within 30 days of initial contract and every five years thereafter. i. Proof of eligibility to work in the U.S. j. Valid S.C. Driver's License. k. DSS check within 30 days of the initial contract and every five years thereafter. 8. The Contractor will maintain Professional Malpractice/Liability insurance for the services requested herein. CONTRACTOR must provide a liability insurance ACORD with the named as Certificate Holder with the Point of Contact and Address included. CONTRACTOR will notify within thirty (30) days of the effective date of any changes to this coverage and provide an updated ACORD. Proof of this coverage must be provided prior to the Contract start date. Failure to maintain Professional Malpractice/Liability Insurance may lead to the cancellation of this Contract. 9. The Contractor agrees to email invoices monthly for services provided on separate invoices per work location by the fifteenth (15th) day of the next month. The invoice must include work location, the correct hourly rate, and dates of work/dates of service (in chronological order). 10.The contractor agrees to email all invoices and statements to the accountspayable@djj.sc.gov email address.
