Responsibilities: The Financial Case Manager is responsible for a multifaceted role, providing comprehensive support to clients. They ensure compliance with State and Federal guidelines, conduct weekly discharge needs assessments, and develop individualized aftercare plans, including making appointments for ongoing care and coordinating continued care needs with referents and payors. This role involves collaborating with the treatment team to align aftercare plans and assisting clients with employer and legal concerns. Additionally, the Financial Case Manager works closely with Facility Leadership to offer timely financial assistance, addressing insurance inquiries and collecting Copays/Deductibles as per Admission Department directives for each admission. Works effectively with the facility leadership team to ensure success of the facility by completing the following: Core Job Duties: 1. Maintain open communication and collaboration with the multidisciplinary treatment team to address aftercare and discharge planning needs. 2. Develop individualized aftercare appointments by collaborating with patients. 3. Ensure follow-up appointments are made within 7 days of discharge from our facility with a behavioral health provider. 4. If a patient is on MAT, be sure the patient has an appointment with a MAT provider after discharge. 5. Coordinate directly with referents and payors to ensure continued care needs are met. 6. Collect Co-pays, Deductibles, and private pay fees within 24 hours of the client's admission. 7. Assist patients in staying focused on their treatment by addressing employer and legal needs. 8. Conduct weekly discharge needs assessments and develop individualized aftercare plans. 9. Be able to assist in validating patients' benefits and financial responsibility using online applications such as InstaMed, Navient, and Availity, or other relevant applications. 10. Follow the outlined procedures for the "Collection of Patient Responsibility." 11. Demonstrate a thorough knowledge and understanding of collection policies and procedures, specifically related to Co-pays, Insurance Deductibles, setting up payment plans, and upfront private pay payments for patients. 12. Assist client with COBRA election while obtaining confirmation of payment. 13. Collaborate with the Admission Team to find solutions and collect owed funds. 14. Ensure clients sign the “Coordination of Benefits”, “Authorized Claims Representative”, “Payment Plan/Agreement”, “Financial Checklist”, other necessary consents, and complete the first CMDP within 72 hours of admission. 15. What We're Looking For: Santé is seeking a Part-Time Mobile Clinician Licensed in Social Work or related field to join our frontline Mobile Response Stabilization Services team serving Kent, Queen Anne, and Dorchester Counties fully in-person. As an MRSS Clinician, you will partner with a Family Peer Support Specialist and will work with families on crisis stabilization, skill building and connecting to area resources. Additionally, you will be conducting assessments and creating care plans as a path to achieve the Family Vision. Clinicians will work with youth and identified supports for a period to serve as liaisons for collaboration with involved parties. Some of these entities include families, schools, mental health providers and other community partners ensuring a smooth transition to long-term services and support. NOT A REMOTE POSITION. IN PERSON ONLY. What You'll Do: Operate as a Clinician on a two-person team including a Family Peer Support Specialist Respond to calls for intervention in the community (schools, homes, parks, churches, etc.) Perform on-scene assessments and effectively perform crisis intervention, including de-escalation, crisis planning, and implementation. Once youth is stabilized, design appropriate referral recommendations using available natural supports and community resources. Partner with youth, family/caregivers, and other community agencies to broker positive relationships over an 8-12-week period Attend community stakeholder meetings as pertains to the involved youth Complete documentation in an accurate and thorough manner. Attend outreach activities to promote the program What We Require: Minimum Masters Degree in Social Work, or related field of study and at least one (1) year professional experience. Current licensure in the state of Maryland (LMSW, LCSW, LCSW-C, LCPC) or (Santé will assist with licensure transfer). Valid driver's license and proof of current automobile insurance. Two (2) years related experience. Current licensure in the state of Maryland (LCPC or LCSW-C).
