The Behavioral Health Consultant provides primary care-based behavioral health services to clients with complex or chronic needs depending on the assigned program as determined in the treatment or care plan of the client. Counseling or therapeutic services are provided as part of a team, with primary care-based services being integrated into the care plan directed by the primary care provider. We offer this position at either PsyD/PhD clinical psychologist, or independently licensed Master's-level mental health therapist.
This is a part-time, 20-hour/week position.

Essential Functions/Responsibilities/Duties
Responds to requests from primary care providers to address behavioral health needs of medical clinic patients to maintain a warm hand-off for most patient encounters.
Provides brief intervention for presenting problems and triage for ongoing treatment services as needed.
Completes encounters and associated documentation, including coding and billing.
Presents case studies and reports to funding sources or as requested by outside organizations.
Measures change and adjusts treatment as needed, using appropriate screening tools.

Enters documentation into the electronic medical record (EMR).

Presents mental health/behavioral health findings and recommendations in verbal or written format to appropriate professionals either upon request or as part of a regular case presentation for peer review or clinical meetings.
Leads staff training sessions on new therapeutic techniques and practices. Provides training and education on clinical topics to team members, staff, and the community, as needed.
Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.
Qualifications
Clinical Psychologist Requirements
Education: Doctor of Philosophy (Ph.D.) or Doctor of Psychology (Psy.D.) in Clinical Psychology.
Experience Preferred: One year's clinical experience working within a multidisciplinary team in primary care or other medical settings providing diagnostic and psychological assessments for primary care-based positions, or one year's clinical experience working with children, adolescents, and families for specialty behavioral health settings.
Licenses/Registration: Licensed Psychologist within the state of practice. First Aid/Cardiopulmonary Resuscitation (CPR) Certification obtained within 45 days of employment.
Master's Level Therapist Requirements
Education: Master's Degree in Social Work, Psychology, Counseling, or related field.

Provides culturally competent and appropriate behavioral health treatment to people served by Community Counseling Solutions. RESPONSIBILITIES
Demonstrate ability to assimilate data from a wide range of observations and use various clinical techniques and skills in gathering the client's personal, family, medical, and psychiatric histories.

Provide consultation services to hospital departments and other providers as necessary and appropriate.
If varianced, provide clinical services to children, adults and families. This would include but is not limited to screening, assessment, individual and group therapy, consultation, and all other allowed clinical services.

Complete all paperwork in a way that is professional, clinically sound and timely as prescribed by the OAR's and other pertinent guidelines. Required to ensure that all paperwork is present, and/or the client record is well maintained.
Attend trainings, meetings, conferences, etc. as directed by administration and use the information gathered to benefit the agency and the customers.
Assist other clinicians in the facilitation of groups as requested.
If varianced, provide intensive children's treatment services.
Utilize agency software/computer systems to prepare current client records of treatment and reports as necessary and required.
Transports clients as required
Administer forms for urinalysis testing (UA's), provide accurate information on the UA's to clients, in some cases observes the UA, and record all pertinent information.
Must report all abuse to appropriate legal office, complete all necessary paperwork and investigations, and develop safety plans as required.

An Applied Behavior Analysis program at Catholic Charities provides intensive services including a Day Program model which serves children aged 2-5 with an Autism Spectrum Disorder in a comprehensive clinic-based model, group services, and early intervention model. A Positive Behavior Support framework is used to develop tailored services and treatment plans to meet individual children and families' needs.
The services will include; Functional Behavioral Assessment and treatment planning for children diagnosed with Autism Spectrum Disorder (ASD), providing Applied Behavioral Analysis (ABA) services, parent training, and a conducting a therapeutic day program.
Position Summary:
This Board Certified Behavior Analyst (BCBA) - Lead is responsible for the implementation of services for children with a diagnosis of ASD from a Center of Excellence by using ABA principles including functional behavioral assessments, creating individualized treatment plans, as well as supervising Behavior Technicians who are providing one-to-one services based in the Day Program. This position is supervised by the ABA Program Manager and works collaboratively with Speech Language Pathologist, BCBA interns and other allied specialists providing services for children with ASD. He or she is also responsible for parent training and successfully transitioning the child into the home and school environment.
Responsibilities:
Provide ABA Services within the Day Program and meet program, funding, and HIPAA requirements.
Ensure assessments and treatment plans are submitted and approved by the Managed Care Organization (MCO) or other funding entities.
Observe confidentiality and compliance with HIPPA Standards in all aspects related to program, staff, and families.
Report any suspected child abuse or neglect according to policies.
Implement best practices for ABA services using a Positive Behavior Support framework to address disruptive behavior and embed early, intensive behavior interventions in developmentally appropriate context.
Complete and document Functional Behavior Assessment which includes: direct face-to-face assessment, records review and observation of client in the home, school, or appropriate community setting when warranted.
Develop, with family input, a specific, individualized, time-limited treatment plan which includes: service type, family engagement/training goals, child behaviors with targeted outcomes, safety plan, description of measurable treatment interventions to each behavior to be accomplished in the 12-week day program and strategies for generalization of skills at home and school.
Implement services in a multi-disciplinary, child-centered, family-focused, community based, culturally competent and minimally intrusive manner.
Ensure treatment is implemented in a coordinated manner with consideration of school and community resources available, with evidence that the requested services are not redundant to other service provided.
Coordinate a plan for treatment and transition (e.g., special education, speech, occupational therapy, physical therapy, family counseling, and medication management).
Ensure the plan includes measurable discharge criteria and discharge plan as appropriate.
Ensure the treatment plan is signed by the client/family and reviewed at least every 180 days and resubmitted for approval with the MCO or funding entity.
Document all assessments, treatment plans, case notes, behavior data and services provided according to policies.

Provide direct ABA Services to children and their families
Implement interventions to target each child's individual needs across all developmental domains (cognitive, language/communication, social, play, motor, and behavior).
Refer child and family for home-based, center-based, or therapeutic classroom services as appropriate for the identified needs.
Ensure all treatment methodologies are evidenced-based and reflect best practice.
Train and collaborate with families in the development and implementation of positive behavior support strategies
Coordinate interventions with parents and team of allied service providers (speech and language, OT, PT, Psychiatric, and others)
Collaborate with parents on interventions that can be implemented in the home environment for skill-based training which will decrease behavior problems, teach specific skills and maximize child outcomes.
Maintain effective ongoing communication and collaboration with the ABA team and other providers in Children's Village.
Collaborate with school districts, Educational Service District, Birth - 3, Primary Care Providers, and other entities to provide a coordinated service plan in collaboration with ABA services.

Participate in concurrent discharge planning to ensure a smooth transition to community/educationally based service.

Esther's Home direct service advocates provide comprehensive residential services, meeting the residents' basic needs, working with a team to build opportunities for their development, and fostering and maintaining a safe, clean, abuse-free home.
Responsibilities
Under supervision of On-site Program Manager provide innovative, trauma-informed, intensive case management services to up to five residents aged 11-17 who have experienced sexual exploitation.
Build trust and rapport using a strengths-based approach and trauma-informed engagement practices with residents.
Work cooperatively and collaboratively with all site staff, partners, and volunteers to create a nurturing, dynamic, and constructive environment for residents; take ownership for fostering a safe, trauma-informed, home-like atmosphere where residents can feel they are not at risk for abuse.
Provide on-site resident supervision, including crisis intervention and behavioral redirection to residents who use less effective or maladaptive coping skills. Collaborate with staff on crisis prevention and assistance strategies.
Responsible for working with Esther's Home Care Team to coordinate all aspects of resident care. This includes the following: school enrollment, medical care, mental health treatment, social services navigation, criminal justice system coordination, and substance use treatment. Work with former service providers to minimize relocation trauma and disruption.
Create individualized treatment plans incorporating all aspects of care to help residents set and achieve the goals, communicating to the wider team for consistent, cohesive support.
Conduct assessments throughout each resident's stay to inform ongoing service provision and to identify and eliminate barriers to progress.
Run one-on-one and group mental health, life skills, and coping skills curriculum.

* Participation as a member of a BHIP treatment team and actively participates through collaboration with Veterans and families as well as interdisciplinary treatment team members, including but not limited to Psychiatrists, Nurse Practitioners, Psychologists, Social Workers, Licensed Professional Mental Health Counselors (LPMHC), RNs, Pharmacists and Administrative Support staff, in the development and implementation of a treatment plan.

Suicide Prevention Case Management
* Provides emergency/crisis intervention services to outpatients experiencing acute psychological crises, helping to stabilize these individuals, assisting in the referral arrangements for transfer to a psychiatric facility for inpatient care if necessary and providing follow up care as needed.
* Identifies and prioritizes the Veteran's and their family/caregiver's desired or expected goal(s) and outcome(s), as well as a resource assessment to identify available benefits, assistive options, and appropriate services and resources within and outside VHA.
* Provides consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning, compliance with treatment, and discharge planning.
* Plans and implements a care plan, including safety plans through coordination and collaboration with the Veteran, the family, or caregivers, and VHA and community providers, facilitates proactive, patient-centric communication and information sharing between the Veteran or Servicemember, their family or caregiver, providers, and other care team members.
* Coordinates effective and timely referrals to ensure the appropriate level and type of care, working collaboratively with VA teams based in Spoke Sites, and other resources across VISN 20. The incumbent manages transitions of the Veteran to other VHA, DOD, other Federal, State, and local home, and community-based services (to include the Office of Community Care).
* Facilitates discharge planning through collaboration with Veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate discharge plans are executed in a timely manner.
* Coordinates effective and timely responses to Veterans Crisis Line calls and referrals.
Psychotherapy
* Uses a high level of skill in assessing and treating complicated psychosocial problems. Incumbent completes thorough psychosocial assessments when the Veteran is referred and will complete a thorough treatment plan when a Veteran engages in mental health services.
* Possesses a working knowledge and experience in use of mental health and substance abuse diagnoses and treatment procedures, including acute, chronic, and traumatic illnesses, in the provision of psychotherapy with individuals, groups, couples, and families, crisis intervention, supportive counseling, referral for resources and benefits.
* Independently works with Veterans and their families who are experiencing a wide range of complicated mental, psychiatric, emotional, behavioral, and psychosocial problems.
Referral Management
* Serves as liaison between Behavioral Health and a variety of referral sources for the administration on day-to-day coordination of care issues.
* Responsible for monitoring and timely processing of referrals to BH Services within assigned work queue(s).
General clinical functions included in this role include:
* The incumbent serves on committees and work groups at the facility as deemed appropriate by the supervisor.

The TPA, or Hanford Federal Facility Agreement and Consent Order, provides a cleanup schedule to bring the Hanford Site into compliance with state and federal environmental laws.
The Department of Energy, Richland Operations Office, is in need of general support services to support the Hanford Advisory Board for six issue-based standing committees: 1) Executive Issues Committee; 2) Tank Waste; 3) River & Plateau; 4) Health, Safety, & Environmental Protection; 5) Budgets & Contracts; and 6) Public Involvement and Communications. Each of the committees may meet up to twice per fiscal year quarter (i.e. around 24 total meetings per year) dependent on the required workload of the HAB. Joint committee meetings may be held for cross-cutting topics.
The ProSidian Engagement Team is responsible for labor, supplies, equipment, all logistics for meeting space, meeting execution/administration, recordkeeping, and other activities needed to facilitate interaction among the Tri-Party Agreement (TPA) agencies and members of the Board. Team ProSidian provides these services in accordance with the Federal Advisory Committee Act (FACA), the Environmental Management (EM) Charter and the HAB Operating Ground Rules. Team ProSidian coordinates at least weekly with DOE, specifically the DOE Deputy Designated Federal Officer and the DOE Federal Coordinator. JOB SUMMARY: Under the supervision of the SSVF Program Director and in partnership with the SSVF Housing Stability Coordinator, the Shallow Subsidy Case Manager will oversee the Shallow Subsidies (SS) program; a service funded through the VA's Supportive Services for Veteran Families (SSVF) program that provides rental assistance payments of up to 50% of total rent to landlords on behalf of the Veteran household for up to two years. The SSVF Shallow Subsidy Case Manager will oversee a caseload of SSVF Shallow Subsidy participants to ensure the service is maximized for Veterans moving to the Shallow Subsidy program from SSVF Rapid Rehousing and Homelessness Prevention services. This position employs a variety of evidence-based interventions including Motivational Interviewing, understanding of the Housing First model, Harm Reduction, and Trauma Informed Care to provide client centered services to Veterans. This position will provide stabilization and case management services, coordinate with landlords and community partners, and ensure programmatic grant compliance.
OVERVIEW OF RESPONSIBILITIES: Includes, but is not limited to:
Partner with Housing Stability Coordinator to oversee the internal and external referral processes and parameters based upon shallow subsidy program screening criteria that identify those whose needs align with this program.
Maintain a caseload of shallow subsidy program participants providing case management of subsidy program participants, and provide more intensive case management as needed. Provide proactive follow-up office and home visits utilizing progressive engagement to ensure stability and further progress towards self-sufficiency; this includes support, advocacy, reducing isolation, active listening, problem-solving, identification of and connecting to community resources.
Implement a "Housing First" philosophy to ensure that treatment for mental health, substance abuse needs, or the resolution of other barriers are not a condition of services or program participation.
In collaboration with SS participants, develop Individual Housing Stability Plans (IHSP) which aim to provide longer term housing retention and stabilization goals
In accordance with BMAC and SSVF program requirements, maintain up-to-date case files and case notes through recording service coordination efforts and outcomes in HMIS and CAP60, and ensure thorough data collection and monitoring of case files for subsidy program participants as required by grant programmatic compliance.
Develop and maintain familiarity with all relevant public and private agencies and service providers and effectively leverage resources available within the community to benefit program participants, including the VA, public health, law enforcement, social services, faith-based organizations, medical and mental health services, and substance abuse treatment services.
Ensure subsidy payments are submitted and approved for on-time payment monthly.
Communicate at least monthly or as needed with Veteran families and Landlords to ensure timely rental payments and lease compliance.
Assist in the development and maintenance of a network of professional contacts in rental housing to cultivate access to affordable units.
Use program orientation materials to provide education and information for program participants, landlords, and community partners on Shallow Subsidy program.
Teach and model landlord relationship building and communication.
Assist Veteran households with referral to internal BMAC SOAR professional or to other community agency for assistance in applying for the Social Security Administration (SSA) disability programs: Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI).
Skillfully manage Veteran participant crises, providing brief risk assessment, safety planning, and de-escalation with the goal of connecting program participant to a therapeutic service.
Regularly report program participant progress to Director and team as requested at weekly case staffing meetings. This includes but is not limited to completing assessments, service transactions, and exit forms as necessary for the program or organizational requirements.
Actively participate in service coordination reviews and conferences.
Attend program, department, and BMAC meetings and trainings, as well as meetings with collaborating agencies as required.
Perform other related duties as assigned.
QUALIFICATIONS:
Education & Experience
Relevant experience and/or education may be substituted for either education or work history.
Associates degree in Human Services or related field required; bachelor's degree preferred.
Two years' related work experience administering social services
Experience working with persons experiencing homelessness and Veterans preferred.
Experience working with vouchers and/or subsidized housing and navigating housing waitlists preferred.
Bilingual: English/Spanish is preferred.
Knowledge, Skills & Abilities
Working knowledge of issues and resources in relation to housing and persons experiencing homelessness and other community services for very low-income households.

WORK ENVIRONMENT: Work to take place in and out of the office.
