Key responsibilities of the role
Your North Star: Provide direct patient care and care navigation by delivering evidence-based care that is founded in awareness, education, and prevention for the perinatal mental health issues affecting our patients.
We are hiring a creative, thoughtful, and skilled person who is eager to improve access to the highest quality perinatal mental and behavioral care, with a particular focus on reducing disparities. As a member of the clinical team, you would support our whole-person program of emotional, mental and behavioral care for pregnant people before, during, and after pregnancy. This is a part-time, salaried position. The following shift is available: 12p - 6p MT or PT.
Reporting to the Director of Therapy, your key responsibilities will include:
Providing one-on-one virtual care visits for Pomelo patients
Providing behavioral health and health related social needs screenings and appropriate response including diagnostic, therapy, and behavioral health interventions to pregnant and postpartum patients.
Performing ongoing psychosocial assessment of patients, providing reassurance and building rapport
Establishing individualized care plans to meet specific health needs based on evidence-based standards
Developing wellness goals by determining motivating factors and leveraging them for best possible health outcomes.
Identifying and addressing barriers that have been identified by patients.
Reducing care gaps (missed appointments, medication management, etc.) by frequent engagements with patient
Triaging of patients with urgent screening results and concerns to appropriate virtual and in-person emergency services
Collaborating with all involved teams and support services to ensure that patient and family needs are met.
Documenting all care/interactions in a timely fashion and escalating to appropriate multidisciplinary teams, as needed
Supporting group care, as needed
Who you are
Independently licensed as an LCSW, LMFT, LPC or LMHC for at least 5 years
Completed the NCMHCE and/or NCE
Have at least 120 hours of clinical supervision completed
Have 5-8 years of experience providing therapy services in a hospital and/or healthcare practice
Have experience with perinatal mental health care, including: perinatal mood and anxiety disorders, substance abuse disorders, trauma-informed care. Previous training with this population is required.
Are enthusiastic for promotion of diversity, equity, and inclusion in all aspects of Pomelo's internal and external programs
Possess strong organizational skills and the ability to prioritize and follow through on multiple projects in a timely manner
Are facile with email, spreadsheets, EHRs, and other commonly used software, and willingness to work with new software
Understand the prevalence and role that birth inequity and structural racism plays in maternal morbidity and mortality
Are professionally engaged
Have experience working with clients eligible for Medicaid and/ or other public health programs
Must be willing to cross license in other states where there is a company need (you'd be supported with this process by our licensing and credentialing team).

A Group Meeting Facilitator interacts with groups that need coaching to work productively as a team to reach a common goal. The group meeting facilitator guides discussions of the topics laid out in group meetings and project/taskforce initiatives agenda, with the purpose of moving the team towards a common goal.
A skilled Group Meeting Facilitator fosters collaboration and teamwork and manages conflict among team members. The ProSidian Consultant | Group Meeting Facilitator will coordinate, lead, and support all phases of team meetings to ensure progression and completion of work as well as supports the Global Project Lead (GPL) And Project Team, and is accountable for the quality, timeliness, and effectiveness of group meetings and/or project/taskforce initiatives and ends with delivery of the final outputs that achieve stated mission objectives.
The job of a “Group Meeting Facilitator” is more of the "conductor" than a participant throughout the Program Period. The ProSidian Group Meeting Facilitator shall serve as a consultant to clients across private companies, fortune 1,000 enterprises, and government agencies of all sizes. You direct the flow and energy throughout the program period. Qualifications
A Bachelors' or Masters' Degree from an accredited school (Preferable) and/or a minimum of 3 years' experience in a similar position

Catholic Charities of Southern Nevada has created a separate entity for its Adoption Services Program, called Loving Hearts Adoption Services. The new name builds off nearly a century of experience in facilitating thousands of adoptions. Loving Hearts Adoption Services is an accredited adoption agency and 501(c)(3) nonprofit agency that provides lifetime support to expectant families, adoptive families, and adoptees before and after placement. Position Summary
The Adoption Social Worker is employed by Catholic Charities of Southern Nevada and provides services to Loving Hearts Adoption Services. The Adoption Social Worker is responsible for day-to-day work related to Adoption cases. The primary focus of the Adoption Social Worker is to provide education, support, and adoption services to birth families, adoptive families, and adoptees throughout all phases of the adoption process. This position requires the applicant to be a Licensed Social Worker (LSW) in the state of Nevada.
Essential Responsibilities
* Train, support, and assess readiness of prospective adoptive parents and birthparents to ensure that all appropriate legal compliance is met.
* Adhere to program standards, procedures, and department budget.
* Facilitate support groups.
* Assist with the drafting and completion of paperwork for all stages of the adoption process.
* Provide information regarding the adoption process to expectant parents and birth parents.
* Provide information regarding the adoption process to prospective adoptive applicants.
* Conduct intakes with expectant parents considering adoption and participate in compiling case file documentation.
* Conduct intakes with prospective adoptive parents and participate in compiling case file documentation.
* Schedule and perform the home study process under direct supervision by the Director.
* Facilitate matches between expectant parents considering placement and prospective adoptive parents.
* Work closely with hospital staff. Participate in discharge process at hospital.
* Facilitate and witness relinquishment of parental rights paperwork with birth parents.
* Facilitate placement process and required paperwork with adoptive families.
* Schedule and conduct required in-home supervision visits for each adoptive placement.
* Share responsibility of being on call outside of regular business hours to handle urgent matters.
* Represent agency at finalization hearings and other court proceedings.
* Help to facilitate and participate in the Adoptive Parent Education classes.
* Help to facilitate and participate in agency events.
* Maintain confidentiality of all client contacts, records, and case work.
* Represent agency in ethical, professional, and positive manner at all times.
* Protect all confidential information, company property and electronic data. Comply with safety rules.
* Collect fees or arrange for fee collection from applicants when due.
* Adhere to NASW Code of Ethics, ensuring professional conduct, integrity, and ethical decision-making.
* Other related duties as directed.
Knowledge, Skills and Abilities:
* Must be able to make sound decisions and exercise good judgment.
* Must be able to solve problems by analyzing complex situations, identifying key issues, and developing effective solutions.
* Must be able to effectively handle the highly charged emotions of clients as well as personal emotions raised by the adoption process, crisis pregnancy, substance abuse and poverty.
* Must have a strong sense of focus and be task-oriented and non-judgmental with a clear sense of personal boundaries.
* Strong knowledge of adoption-related state laws, child development and behavior, family dynamics and child placement procedures.
* Experience in client interview and assessment techniques.
* Excellent interpersonal, verbal and written communication skills.
* Must be computer literate and detail-oriented.
* Must have a respect for confidentiality involving both clients and employees.
* Working knowledge of community resources.
* Ability to work in a variety of settings with culturally diverse groups.
* Fluency in Spanish is preferred.
* Share responsibility of carrying emergency cell phone and responding as needed, including after regular business hours and on weekends.
Qualifications:
* Bachelor's Degree in Social Work and State of Nevada Licensed Social Worker (LSW) required.
* Must have a minimum of two (2) years of experience in child welfare; adoption experience a plus.
* Must be able to pass criminal and child abuse background clearances.
* Knowledge and adherence to the NASW Social Work Code of Ethics.
* Flexible schedule to work after hours and weekends, as necessary.
* Must be at least 25 years old with a valid Nevada Driver's License with current minimum Nevada state required auto insurance.
* Must have transportation available to fulfill field assignments.
* Must have pediatric CPR certification or obtain within 30 days upon hire.
Physical Requirements
* Time is split between office work and in-home visits. Occasional visits to hospitals, jails, prisons and other off-site locations.
* Ability to communicate with clients, co-workers, and others involved in the adoption process.
* Ability to carry work cell phone and respond appropriately as needed after regular business hours.
* Ability to operate and use office equipment, especially copier, computer, keyboard and mouse.
* Ability to continuously sit and/or stand. Ability to walk continuously if needed and climb stairs as needed.
* Ability to operate a vehicle, including sitting for extended periods and entering and exiting the vehicle.
* May occasionally lift up to 25 pounds.

The medical social worker is responsible for the implementation of standards of care for medical social work services.
DUTIES & RESPONSIBILITIES
1. Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
2. Assesses the psychosocial status of patients related to the patient's illness and environment and communicates findings to the registered nurse.
3. Carries out social evaluations and plans intervention based on evaluation findings.
4. Provides patient, caregiver, and family counseling.
5. Providing services that are ordered by the physician as indicated in the plan of care.
6. Communicates with the physician who is responsible for the hospice plan of care and other health care practitioners (as appropriate) related to the current hospice plan of care.
7. Assists physician and other teams members in understanding significant social and emotional factors related to health problems.
8. Prepares clinical notes on all patients referred to social work.
9. Prepare IDG notes to reflect timely compliance and patient needs and status.
10. Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
11. Provides education to patients or families/caregivers and community agencies.
12. Serves as liaison between patients or families/caregivers and community agencies.
13. Maintains collaborative relationships with Organization personnel to support patient care.
14. Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
15. Participates in the development of the total plan of care and case conferences as required.
16. Participates in discharge planning.
17. Supervises, as directed, any Social Worker Assistants (SWA's).
18. Participating in the hospice quality assessment and performance improvement program and hospice-sponsored in-service training and director of services.
19. Other duties as delegated by the Director of Nursing /Supervisor.
20. Work a total of 15 hours per week. However, there will be cases where you will have to cover 40hrs per week when the Director of Social Services takes time off.
JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
1. Must have a master's degree from a school of social work accredited by the Council on Social Work Education. (LSW, LMSW, LCSW)
2. Minimum of one year's social work experience in health care setting.
3. Experience in a hospice care preferred.
4. Demonstrates good verbal and written communication, and organization skills.
5. Must have an active social work license in the state of Nevada.
6. The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile.
