POSITION SUMMARY
The Mental Health Professional will be involved in working with all disciplines on the Behavioral Health Unit to attain effectiveness in serving the patient population.
MINIMUM REQUIREMENTS

The Licensed Mental Health Therapist participates in and conducts in-service programs, as well as assists in the orientation of new staff and provides the necessary preparation of documentation, necessary records and reports.

ESSENTIAL DUTIES AND RESPONSIBILITIES

5. File clinical documents in clinical charts.

Qualifications
Requirements/Qualifications of the Social Services Assistant
At least one (1) year experience in social work, preferably in a long-term care facility.
Effective verbal and written English communication skills.
Highest level of professionalism with the ability to maintain confidentiality.
Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
Customer service oriented with the ability to work well under pressure.
Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.

Responsibilities
Responsibilities of Social Services Assistant
Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Social Services Director (“Director”) or Administrator.
Participate in discharge planning, development and implementation of social care plans and resident assessments.
Interview residents and families to obtain social history.
Involve the residents and families in planning social service programs when possible.
Assist in arranging transportation to other Facility's when necessary.
Refer residents and families to appropriate social service agencies when the Facility does not provide the services or needs of the resident.
Provide information to residents and families as to Medicare and Medicaid, and other financial assistance programs available to the resident.
Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
Interpret social, psychological, and emotional needs of the resident and family to the medical staff, attending physician, and other resident care team members.
Assist in obtaining resources from community social, health, and welfare agencies to meet the needs of the resident.
Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.
Review departmental complaints and grievances from personnel and make written reports to the Administrator of action(s) taken. Follow Facility's established procedures.
Assist in developing and implementing appropriate plans of action to correct identified deficiencies.
Develop, implement, and maintain an ongoing quality assurance program for the Social Services Department.
Participate in Facility surveys (inspections) made by authorized government agencies, as necessary or as may be directed.
Ensure that all charted progress notes are informative and descriptive of the services provided and of the resident's response to the service.
Maintain a reference library of written material, laws, standards of practice, etc., necessary for complying with current standards and regulations and that will assist in maintaining quality social service.
Involve the resident and family in planning objectives and goals for the resident.
Meet with administration, medical and nursing staff, as well as other related departments in planning social service programs and activities.
