Job Summary
Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plans in coordination with patient, family and interdisciplinary staff as necessary. Essential Functions
Maintains the standard of nursing care and implements policies and procedures of the hospital and nursing department

Assigns or delegates tasks based on the needs and condition of the patient, potential for harm, complexity of the task, and within scope of practice of the staff to whom the task is delegated
Assigns nursing care team members in accordance with patient needs, team member's capabilities and qualifications
Documents patient admission assessment and reassessments, patient care plans and other pertinent information, completely in the patient's medical record according to nursing standards and policies
Performs assessment on all patients on admission and reassessments as per policy. Makes referrals to other disciplines based on assessment
Develops nursing care plan of assigned patient on admission, updates plan of care as needed and ensures plan of care is coordinated with patient, family, and other members of the team
Assesses and reassesses pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management
Revises the plan of care as indicated by the patient's response to treatment and evaluates overall plan daily for effectiveness
Performs patient care responsibilities considering needs specific to the standard of care for patient's age
Receives physician's orders, ensures transcription is accurate and documents completion
Administers medication utilizing the five rights of medication administration reducing the potential for medication errors
Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate
Assists physicians with examinations, treatments and special procedures and performs services requiring technical and manual skills within scope of practice
Performs treatments and provides services to level of licensure
Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians, and other healthcare providers
Interacts professionally with patient/family and involves patient/family in the formation of the plan of care
Interprets data about the patient's status to identify each patient's age specific needs and provide care needed by the patient group
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors
Initiates or assists with emergency measures for sudden adverse developments in patients' condition
Answers telephone, paging system, patients' call lights, anticipates patients' needs, and makes rounds of assigned patients and responds as appropriate
Consults other departments as appropriate to provide for an interdisciplinary approach to the patient's needs
Provides end of shift report to oncoming nurse, narcotics are counted, documentation is complete, and physician orders signed off
Communicates appropriately and clearly to management, co-workers, and physicians
Identifies and addresses psychosocial needs of patients and family; communicates with Social Service/Discharge Planner regarding both in hospital and post discharge needs
Participates in orientation, instruction/training of new personnel
Manages and operates equipment safely and correctly

Knowledge of medications and their correct administration based on age of the patient and their clinical condition
Basic computer knowledge
Able to organize tasks, develop action plans, set priorities and function under stressful situations

Job Duties:
Develop and implement clinical programs while providing clinical oversight, coaching, and mentorship for our therapists.

Role Mission: Students and families are the most important stakeholders at IDEA Public Schools. The role of School Counselor leads the campus family engagement vision, social emotional learning program, and coordination of mental health and counseling supports for all scholars on the campus.
Our key tenets of family engagement, social emotional learning, and student social emotional supports are implemented through the role of the School Counselor to ensure all staff are building authentic relationships with students and families. Data maintenance and analysis is an integral component of effective school-based intervention implementation. SSAs own the validity and accuracy of their goals to drive campus strategies and tactics. Family engagement and student success is the job of all campus team members. The School Counselor will direct and hold all campus team members accountable to fulfill their role.
When the work is lead effectively, the entire school staff is clear on their roles in supporting students and families, has the data needed to drive action, inspired, and supported to engage with students and parents in a meaningful way which would result in meeting the goal for all students to have the academic and social skills they need to achieve their goal of going to and through college. Through this work, School Counselors ensure our schools are a place where students feel safe physically and emotionally safe and therefore can learn, grow, and succeed in and outside of the classroom.

Salaries for people entering this role typically fall between $72,785 and $84,431, commensurate with relevant experience and qualifications and in alignment with internal equity. This role is also eligible for a performance bonus based on team performance and goal attainment.

What You Bring -- Competencies
Qualifications:
Education: Master's degree in School Counseling, Counseling, Social Work, Marriage and Family Therapy, or related field
Experience: Previous experience in a school or school setting
License or Certification: Valid TX School Counselor certification or counseling licensure (LMSW, LCSW, LCSW-S, LPC-A, LPC, LMFT-A, LMFT) required
Knowledge and Skills:
Proven track record of success
Excellent organizational, communication, and interpersonal skills
Ability to manage student behavior

What You'll Do -- Accountabilities
90% Student Persistence
Leads the execution of the school vision for retaining all students at the school by directing and managing all stakeholders at the campus in their roles and responsibilities in family engagement and student persistence.
Provides individual/group counseling for at-risk students and refers students who need intense or specialized services campus-based Licensed Mental Health Provider and/or outside agencies.
Leads the planning of and execution of student and family engagement events that are aligned to the school's culture, community and family needs and promotes the authentic relationship building between the school and the family.
Leads BOY Professional Development for entire campus to clarify the roles every staff member plays in family engagement.
Manages the campus overall student persistence data to ensure 100% of data accuracy and reporting through close partnership with the PEIMS team on campus. Notify all stakeholders who own the persistence goal of the data and rationale as to why students are leaving to identify and analyze trends. Data management includes At-Risk Coding, Potential Leavers and Dropouts and Leaver Coding for withdrawals.
Partners with the grade level teams to help identify and coordinate intervention for at-risk students with responsibility and accountability structures for Grade Level Admin, API's, GTLs and Teachers.
Leads parent conversations and problem solves to ensure we are meeting the needs of our students and families in collaboration with campus lead team.
Coaches and develops teachers to work with all students and families. Partners with the appropriate lead team/campus team members to support the staff and the student to work together to achieve goals.
Maintain knowledge of community agencies as a resource to meet student and school needs.
Directs implementation of state mandated guidance lessons to all students to develop life skills, promote good character, and develop resilience.
Provides support through feedback, training, coaching, and developing teachers in working with students social/emotional needs in the classroom.
Member of the crisis response team on the campus to provide and ensure student social/emotional needs are being met
Conducts individual and group check-ins and advising meetings with students to address social-emotional issues and provide campus-based supports.
Leads family communications platforms: Remind, Smore (Weekly Parent Newsletter), and Facebook
Administers 2 parent survey: 1 in the Spring and 1 in the Fall to get feedback from parents on communication and events. Is responsible to analyze and create a strategic plan to respond

Essential Duties

The ACC-TDCJ Program offers a variety of academic and technical correctional education programs to TDCJ offenders. This is an instructional position within a correctional environment teaching an all-male offender population. Offenders are required to meet academic and security requirements for enrollment consideration.
Teach freshmen and sophomore level Mental Health courses to college students in the Texas Department of Criminal Justice (TDCJ).
Attend a 7-hour in-person Pre-Service TDCJ security training, located in Huntsville, TX.
Submit annual In-Service Training via online
Instruct and supervise a diverse population of students in the classroom at various times and locations
Possess a commitment to student engagement, student success, and instructional excellence
Demonstrate effective communication skills, both written and oral
Prepare and utilize a course syllabus and assessments for each course using guidelines established by the institution
Maintain current knowledge of effective teaching methodologies and utilizes a variety of instructional delivery methods, classroom media, and educational resources
Assist in the recruitment and retention of students
Advise students in academic matters or refers students to appropriate resources
Assess students' performance through a range of measurement activities and keeps them informed of their progress in a timely manner
Engage students face-to-face and through electronic communications
Utilize technology to facilitate learning and to access data, maintain records, generate reports, and communicate with others
Assist in the development, distribution and collection of assessments for courses and program objectives

WORK ENVIRONMENT

Minimum Qualifications
Associate's or Bachelor's degree in Human Services area (Psychology, Counseling, Social Work, Human Services, Addictions Counseling, or Mental Health) with current licensure as a Licensed Chemical Dependency Counselor (LCDC), Licensed Professional Counselor (LPC), Licenses Clinical Social Worker (LCSW) and 3 years non-teaching work experience in the field.
At least 3 years non-teaching related work experience
Must have a working knowledge of Microsoft Office programs, as well as intermediate computer skills.
Must be familiar with interactive teaching methods and instruction via the Internet
· Candidates must pass a criminal background check by the Texas Department of Criminal Justice (TDCJ).
