Assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre-admission through one week post-discharge. Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes.

1. Evaluates patient needs, provides case management.
2. In collaboration with the M.D. /CNS, provides direct patient care to patients, including, but not limited to, medication management follow-up and decision-making about follow-up care, crisis stabilization, medication changes, and wraparound services.
3. Assists in the planning and implementation of medical care and coordination of services with other patient care units, both within the hospital and through outside institutions and links families with community resources.
4. Care is established from the time of admission to the service through termination by utilizing the nursing process in such a manner as to maximize outcomes.
5. Additional duties include the development and implementation of materials necessary to facilitate patient care with varied care providers, patient and family education, staff and care provider education, development of patient and family educational materials.
6. Participation in research, basic supportive counseling, procurement of community resources for families, and assessment of changing patient needs throughout the course of care with appropriate referrals for consultation as needed.

SCOPE OF ROLE
Reporting to the Program Manager, Crisis Services, the Counselor, Crisis Services will provide counseling/crisis services and intake assessments to patients experiencing behavioral and emotional difficulties at the local jails, Signature Health locations, client homes, and within the community. Crisis Services
* Provide crisis and intake assessments at local jails, client homes, and various community settings.
* Provide crisis prescreening and intervention at designated location to determine the appropriate level of care for the safety and welfare of the client and others. This entails providing diagnostic assessment, with a provisional diagnosis, with appropriate disposition within available resources.
* Obtain appropriate "Release of Information" documents at the time of assessment to allow for two way communication with other organizations, providers and contacts. Function as a liaison between agency and other organizations to facilitate client care and aftercare in an expeditions and professional manner.

* Conduct crisis intervention and assessment, and care coordination for both adults and children.
* Actively engage in supervision with the Program Manager, Crisis Services.

Position Overview:
As a Behavioral Health Counselor Provide individual behavioral health therapy to all age ranges with diverse mental health needs using clinical therapeutic techniques to help clients cope better, manage their mental health and promote their own well-being.
Key Responsibilities:
* Identify and counsel at-risk clients, encouraging patient participation and understanding.
* Screen, assess, counsel, and provide foll-up care for clients referred by providers or staff.
* Collaborate, coordinate, and consult with healthcare teams to ensure integrated care.
* Perform telephone triage, crisis interventions, and provide guidance to staff as needed.
* Prepare and maintain accurate patient charts, documentation and computer records.

Summary

Essential Job Functions
* Assesses/investigates referrals involving child abuse and neglect, dependency, or other families in need of services. Conducts in-depth, culturally sensitive interviews with clients, caregivers, children and collateral contacts through face-to-face home visits, office visits, phone calls and written communication.
* Following assessment/investigation and in consultation with a supervisor, determines the next appropriate steps which may include providing short-term interventions.
* Works with schools, medical professionals, law enforcement, and other community-based providers to gather information and to provide ongoing support and services to children and their families.
* Coordinates and/or attends a variety of meetings including family team meetings and team decision making meetings (staffings). If court intervention is necessary, works with the agency's legal team to prepare documents and testimony.
* As applicable, provides extended services for families with more complex challenges who are court involved from the point of transfer through permanence.
* Conducts work in accordance with federal and state guidelines and follows agency policies, procedures and best practice standards. Completes paperwork and case documentation in a timely and thorough manner.
* Performs other related duties as directed. Working hours are variable and family needs. May require working beyond regular county business hours.
Minimum Requirements
Bachelor's degree in Social Work or in a field related to Social Work (Sociology, Psychology, Counseling, Criminal Justice, Criminology etc.) OR Bachelor's degree in a non-related field and two (2) years' experience working in a social services capacity. Must possess a valid Ohio Driver 's License and have the use of an insured automobile.
This position requires travel in the field, at clients homes and occasional travel outside Cuyahoga County.
