What you will be doing:
Provide individual counseling and group facilitation to clients with substance use disorders.
Maintain concise, timely, and thorough documentation in the client's electronic health record according to agency standards.
Develop and implement discharge plans based on the client's needs.
Demonstrate a "whole person" approach to care, including the patient and family/support system when appropriate.
