Responsibilities
Provide group counseling sessions with focus on addiction related topics
Be responsible for a caseload of clients, as identified by program requirements and in accordance with state regulations
Complete required evaluations and assessments, including screening, intake, clinical assessments, ASAM and biopsychosocial histories
Compete individual treatment plans upon client's admission
Document all clinical services and interventions performed within the client's electronic health record and complete monthly treatment reports
Maintain accurate electronic health records on each client and ensure the timely completion of tasks, within 24 hours of service deliver, no later than 72 hours of service delivery
Conduct individual and group sessions based on client's treatment plan goals and level of care
Responsible for maintaining contact with referral sources and other agencies involved in client care (i.e. criminal justice system and housing programs).
Monitor client progress and adjust treatment plans as needed
Coordinate with agency and community resources to ensure a through treatment plan
Assist clients with developing a discharge/continuing care plan and connecting to resources needed to reach personal goals both during the course of treatment and in advance to their discharge
Follow up with clients after discharge based on regulatory guidelines until they have been established with other case management/social services
Qualifications
Associates Degree or higher in chemical dependency or related field
2 – 4 years’ experience in behavioral healthcare, specifically addictions or related field.
Must have minimum of Certified Addiction Counselor (CAC-AD) Status or one of the following certification by the State of Maryland Board of Professional Counselors and Therapists as a: Certified Supervised Counselor-Alcohol and Drug (CSC-AD), LGADC or LCADC.
A minimum of one year of work experience in providing substance and/or clinical services
Experience in conducting needs and risk assessments and an understanding of crisis management

6. Maintains clear and concise documentation in each patient record to reflect physical and functional limitations, psychosocial characteristics, educational needs of patient & family, family/social support systems, financial, economic, and transition needs. Initiates referrals to disciplines as indicated.
7. Participates in nursing unit and department clinical outcome projects as well as process improvement initiatives of care management.
8. Identifies potential or current patient situations which require referral to other members of the health care team such as infection control, risk management, or quality management. Assures plan of care is adjusted as appropriate and that follow-up occurs. Keep leadership abreast of potential issues.
9. Utilizes all risk and predictive analytic tools such as the readmission risk tool. Applies tailored interventions to mitigate potential barriers or risk, prolonged unnecessary hospitalization and readmission prevention.
10. Maintains compliance with all regulatory standards (CMS, commercial insurers etc)
Educational/Experience Requirements:
BSN or ADN with equivalent experience. BSN must be achieved within 5 years of start date in the role
Three years of experience in a clinical setting, ambulatory or post-acute.
Care coordination experience preferred.
Licensure/Certification:
Current licensure as a registered nurse by the Maryland Board of Nursing.
Adherence to the credentialing requirements of AAMC as stated in the nursing bylaws.

JOB SUMMARY:
To facilitate discharge planning in line with goals for length of stays related to difficult to place, high-cost and escalated patients. The Complex Care Social Worker will use a multi-step escalation protocol to identify those patients with barriers that prevent an effective discharge within the goals for length of stay. In conjunction with the interdisciplinary team the Complex Care Social Worker will assume responsibility for the escalated patients in accordance with set standards for discharge planning. The Complex Care Social Worker will also monitor the difficult to place and high-cost patient to identify early intervention opportunities to effect a discharge in a timely and cost-saving manner within the accepted utilization review guidelines.

team members and community partners.
Provides leadership and creativity for programs and projects to enhance the effectiveness of care

Provides non-medical, short-term, solution focused counseling directly to eligible children and youth of service members at assigned primary and secondary locations. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
CYB counselors will also have a focus on supporting the staff and personnel of Child Development Centers (CDC), youth programs and summer camps (if available/assigned) as well as providing parent support, training, and guidance.
Provides training and health and wellness presentations, participate in health fairs and other base/installation activities focused on children and youth.
Enters counselor activity data daily through smart phone or web application to assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service/family member.
Establishes and maintains working relationships with community resources focused on children and youth and provides appropriate linkages. Consistently visible within the assigned location(s) which allows MFLC services to be accessible. When working with children, counselor must abide by line-of-sight protocol. The CYB will flex schedule to respond to urgent requests as needed or at the request of the Regional Supervisor/Regional Director.

International SOS is looking for qualified individuals to be the Social Service Associate for the Sailor Assistance and Intercept for Life (SAIL) Program for The Navy Fleet and Family Support Program (FFSP) The FFSP serves active duty members of the Navy, Marine Corps, Army, and Air Force plus their eligible family members; active duty members of foreign military services who are stationed in the area; reserve military personnel and their family members when they are eligible for military medical care, military members designated as severely wounded, ill and injured and their identified caretakers and, on a space available basis, military retirees and their family members and widows, eligible family members of military decedents and other groups as directed and authorized by higher authorities in response to special circumstances.
Key Responsibilities
Maintain current schedules of SAIL Case Managers and Contract Remote SAIL Clinical Counselors providing individual appointments. Utilize FFSMIS to view schedules, to make client appointments, and send reminders to clients to confirm appointments.
Manage daily scheduling to ensure clients are notified of unscheduled SAIL Case Manager absence as soon as possible, rescheduling appointments in a timely manner at the client's request.
Maintain direct service metrics for SAIL Case Managers and Contract Remote SAIL Clinical Counselors for whom scheduling services are provided.
Ensure initial paperwork is forwarded to clients, following up to ensure paperwork is
complete, returned and uploaded into the appropriate client's FFSMIS case record.

SAIL Social Service Associates must respond well in pressured, crisis situations, and have excellent customer service skills.

For those that meet the basic qualifications of a Tier I provider as described in SECNAVINST 1754.8, they may provide SAIL case management tasks and have the following criteria:
Minimum educational qualifications include a master's degree in counseling accredited by the Council for Accreditation of Counseling and Related Educational Programs; a master's degree in social work accredited by the Council on Social Work Education, a master's degree in marriage and family therapy accredited by the Commission on Accreditation of Marriage and Family Therapy Education, or a doctoral degree in clinical or counseling psychology and a one-year clinical internship accredited by the American Psychological Association.
Tier I clinical providers must perform all clinical duties under the supervision of a licensed practitioner and under no circumstances can they provide independent clinical care. Independent licensure/certification shall be attained within a 36-month period from the date of hire. The following are key responsibilities for those meeting the criteria of a Tier I provider:
Receive Command and non-Command referrals and make an introductory contact to referred Sailors to provide information about the SAIL program.
Provide an initial clinical assessment and ongoing risk assessments using the Columbia Suicide Severity Risk Scale (C-SSRS). Provide ongoing safety planning using the Veterans Affairs Safety Plan.
Provide clinical case management to Sailors enrolled in SAIL per published protocols and procedures. Ensure enrolled Sailors are connected to behavioral health providers and other supportive resources, as indicated, throughout SAIL engagement.
Collaborate closely with Sailor's Command providing routine updates regarding
Sailor risk, safety, and progress in their recovery in accordance with the DoDI 6490.08.
Collaborate with Sailors behavioral health treatment provider(s), and other service providers upon receipt of release of information from the Sailor.
Participate in virtual, program-specific, quarterly quality assurance procedures to ensure that all SAIL services provided are safe and of good quality.
Document and maintain current records in the FFSMIS SAIL Record System for all SAIL contacts, assessments, and case management services provided IAW Navy requirements.
Ensure Command referral information for Sailors who decline or who cannot be contacted is documented in the FFSMIS SAIL Record System. At the conclusion of SAIL services, alert command that SAIL services are ending and advise on procedures if the Sailor's risk of suicide related behavior returns.
Report suspected child abuse and domestic abuse/violence to the local civilian authorities and the Family Advocacy Representative IAW Navy and state requirements. For clients wanting to make a restricted domestic abuse report, provide a warm handoff to the installation Family Advocacy Representative or FAP Victim Advocate. For clients wanting to make a restricted sexual assault report, provide a warm handoff to the installation Sexual Assault Response Coordinator or SAPR Victim Advocate.
Ensure Sailors receive an initial evaluation by Military Treatment Facility/Emergency Department qualified staff after suicide-related behavior or ideation in accordance with local emergency response procedures and in coordination with the Sailors command
Required Work Experience
SAIL Social Service Associates must possess demonstrated experience providing administrative support to a medical or other social service office. Excellent facility with computer and office automation software is required.
Demonstrated experience providing advocacy or case management services is required.
