Position Summary:
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions. This position integrates national standards for case management scope of services including: Transition Management promoting appropriate length of stay, readmission prevention, and patient satisfaction; Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care. Compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy; Education provided to physicians, patients, families, and caregivers; and Leads a population of patients by service line and/or leads the team by being a resource to Tenet performance standards.
Responsibilities
This individual's responsibility will include the following activities:
a) complex psycho-social transition planning assessment and reassessment and intervention, b) assistance with adoptions, abuse and neglect cases, including assessment, intervention and referral as appropriate to local, state and /or federal agencies, c) care coordination, d) implementation or oversight of implementation of the transition plan, e) leading and/or facilitating multi-disciplinary patient care conferences including Complex Case Review, f) making appropriate referrals to other departments, g ) communicating with patients and families about the plan of care, h) collaborating with physicians, office staff, and ancillary departments, I) assuring patient education is completed to support post-acute needs, j) timely complete and concise documentation in Case Management system, k ) maintenance of accurate patient demographic and insurance information, l) precepts new staff members and acts as a resource to all staff, m) facilitates TEMPO as needed, n) participates in department quality improvement initiatives, and o) other duties as assigned.

Experience:
Preferred: 2 years of acute hospital experience
Certifications:
Required: MSW. Must be currently licensed, certified or registered to practice profession as required by law or regulation in state of practice or policy. Active license for state(s) covered.

About the job…
As a counselor, your primary role is to provide care and guidance for one specific group of children. In addition, there are times in the day when counselors will work with other groups of children providing skill instruction.
Counseling Staff are expected to arrive between June 7 -14, 2025 and must be able to work through August 16, 2025 unless other dates agreed upon.
As a counselor you will:
Learn that it is our mission to help youth discover their potential through life-changing experiences and relationships. Counselors are the most important part of ensuring campers get the most from camp.
Work with a cabin or age group and ensure the health, safety, and well-being of the campers in your group.
Participate in staff training to learn important skills for working with campers.
Plan and lead activities for your group and also for other participants in the program.
Help with large group activities.
Participate in emergency drills and in the event of an emergency, carry out the emergency plans.
Help ensure that camp stays clean and well cared for by supervising campers in daily chores, caring for equipment, and picking up litter. All members of the camp community are expected to pitch in on all chores and duties.
Working at camp requires every staff member to cultivate a positive attitude and willingness to help in all areas of the camp experience.
All staff will work 6 days a week with one, 24-hour period off each week and one night off. Staff will be given scheduled breaks during the work day but the days are long and counselors are on-duty much of the day and on some evenings.
Pay & Benefits:
$400 - $440 weekly commensurate with experience, less applicable taxes and withholdings.
Housing and meals are provided on-site at no cost to the employee as part of the experience. Day Camp staff are not required to live on site but may if they choose to. Please note that if you live on property you will be expected to follow the rules for the site you live on.

Planned staff outings are available for staff time off. Camp provides transportation to local towns and cultural experiences for staff who do not have their own vehicle.

Qualifi cations:
Must be 16 years of age and/or older. Prior experience with one of our programs or with another camp program is preferred.
Prefer staff who are certified or who are willing to become certified in Lifeguard Training, First Aid, and/or CPR.
Prefer candidates who can commit to the full summer; however, we will work with staff to accommodate scheduling conflicts.
Must be able to provide proof of a physical within the last 18 months and proof that you are up to date on all required immunizations.

General Working Conditions
Ability to work in excess of a 40-hour week with irregular work hours.
Ability to walk, stand, swim, and sit (including on the floor) for long periods of time.
Ability to speak concisely and effectively communicate in English.
Possible exposure to communicable diseases and bodily fluids.
Must be able to lift and/or assist children in certain program activities.
Must be able to lift and carry supplies weighing up to 20 pounds and occasionally 50 pounds.
Ability to stand or sit while maintaining alertness for several hours at a time.
Position may require bending, leaning, kneeling, and walking.
Possible exposure to communicable diseases and bodily fluids.
Visual and auditory ability to respond to critical situations and physical ability to act swiftly in an emergency.
Must be willing to share a livng space with other camp staff and camp groups.
Abilty to work in an environment where smoking, vaping, tobacco use are not allowed and where the possession and/or use of alcohol or recreatonal marijuana, even if staff is of legal age, on the camp property is prohibited.
