Reviews and countersigns assessments and treatment plans for quality and accuracy for Counselor Interns. Manages all counseling staff. May be required to counsel a caseload of patients. This role requires excellent communication and organizational skills, as well as a strong commitment to supporting vulnerable populations. PRINCIPLE ACCOUNTABILITIES Duties and Responsibilities: Client Assessment: Conduct comprehensive assessments of clients' needs, strengths, and challenges to develop appropriate service plans.Service Planning: Develop and implement individualized service plans in collaboration with clients, outlining goals, resources, and timelines. Resource Coordination: Identify and coordinate access to community resources and services, including housing, healthcare, employment, education, and social services. When necessary, provide crisis intervention services as needed, helping clients navigate immediate challenges and access emergency resources. Advocacy: Advocate on behalf of clients to ensure they receive the necessary services and support to achieve their goals. Case Monitoring: Monitor and evaluate clients' progress toward their goals, making adjustments to service plans as needed. Documentation: Effectively manage case records by assisting program participants with completing applications, forms, collecting case documents, conducting program assessments, evaluations, and other documents related to the participant case file, timely complete CRM updates while maintaining the highest levels of quality, accuracy and program integrity. Collaboration: Work closely with other service providers, community organizations, and government agencies to ensure comprehensive support for clients. Client Education: Provide clients with information and education on available resources, rights, and responsibilities. Compliance: Ensure all activities and documentation comply with relevant policies, regulations, and standards. Provide timely reporting to management. Customer Service: Provide excellent customer service to program participants, prospective participants, and the community. Assist participants with troubleshooting and provide technical assistance as needed to resolve inquiries and concerns. Effectively explain program eligibility, program rules, and provide referrals when necessary. QUALIFICATIONS Required: Bachelor's degree in social work, psychology, human services, or a related field. Minimum of 3 years of experience in case management, social services, or a related field. Strong understanding of community resources and services. Excellent communication, interpersonal, and organizational skills. Ability to work effectively with diverse populations, including individuals with varying needs and challenges. Proficiency in Microsoft Office Suite and case management software (Salesforce). Valid driver's license and reliable transportation. Preferred Qualifications Master's degree in social work or a related field. Licensure or certification in case management or social work. Experience working with specific populations (e.g., homeless individuals, individuals with disabilities, veterans). Bilingual skills are an asset.
