Within our Diversion Services is the Legal Intervention and Network of Care, better known as our LINC team. The Legal Intervention and Network of Care (LINC) Program is a prosecutorial diversion program that provides behavioral health treatment in lieu of prosecution for individuals with mental health and substance use conditions, who have been booked into jail for misdemeanors and low-level felonies and have a history of legal competency being raised. LINC serves adults with behavioral health conditions who have been referred by prosecutors who are willing to dismiss or not file charges if individuals agree to work with LINC. The program is short-term and is designed to connect individuals to long-term mental health and/or substance use treatment and social services resources.
Community House has a fantastic opportunity for a LINC Case Manager.
Our LINC Case Manager will work as part of the LINC team to provide outpatient behavioral services and community support to their clients. This position is responsible for providing treatment and care coordination based on client needs and the treatment plan, progress notes, and collaborative reviews for the assigned caseload, as well as support to all clients served as part of the care team.

RESPONSIBILITIES
Serve as Primary Case Manager for assigned caseload, including advocating for and coordinating client needs and services; ensuring Service Plans, Crisis Plans, and other documentation are completed on time and kept up-to-date, as applicable; and providing consistent contacts, skills training, and care coordination to work towards goals of the client, program, and contract.
Coordinate with relevant internal and external departments to coordinate client referrals, ensure timely intake into program, and develop individualized plans.
Collaborate with care team to ensure all clients access needed treatment, support, case management, and appointments, including transportation to external providers.
Outreach to client in their homes or where they are staying in the community to assess living situation and stability, and provide treatment interventions when necessary.
Ensure all interactions represent the five principles of trauma-informed care, including safety, choice, trustworthiness, collaboration, and empowerment.
Coordinate access to medications for all clients through trained staff ensuring the seven rights of medication use are followed: right person, right medication, right time, right dose, right route, right reason (including refusal), and right documentation according to agency provided medication training.
Complete daily documentation in electronic medical record on client interactions for billable and non-billable activity, medications dispensed where applicable and assigned, and other required documentation, ensuring an interaction and subsequent documentation are completed for all assigned caseload at least twice monthly.
Providing appropriate levels of treatment, skills training, and assistance are provided to clients in managing mental health symptoms and developing skills and insight to manage symptoms.
Partnering as a team to deliver current programming and fill vacant position duties.
Working collaboratively with larger outpatient team of providers to support client in actively working towards treatment goals.
Coordinating care, such as scheduling, attending, and/or arranging transportation for appointments and other case management needs.
Maintaining and documenting in client charts and program trackers following agency, county, state, and federal guidelines.
Providing and receiving input as part of client's care team on service plan and progress, and collaborating with other providers such as primary care provider, housing staff for entering/maintaining/exiting housing, etc.
Facilitating regular group and individual activities, encounters, and specialized client events, such as outings and holiday gatherings.
Providing After Hours crisis intervention with enrolled clients, when assigned:
Answering all incoming calls and making contact within 2 hours of first call.
Provide crisis response for clients referred by Crisis Connections.
Complete assessments, such as mental status, safety, danger to self and others, etc., as appropriate for the situation.
Coordinate with other professionals, including care team, emergency room, social workers, police, family, etc. to determine least restrictive resolution.
Provide all clinical information regarding the crisis to treatment team by 9am on the next working day and complete documentation per required timelines.
Always maintain client and staff confidentiality.
Assist in staffing the drop-in/resource center.
Performing other duties as assigned.
QUALIFICATIONS
Bachelor's degree in social science or other related field, may consider candidates who have 2+ years of relevant experience serving adults with severe and persistent mental illness and/or in diversion services in lieu of degree. For FHARPS program, credentialing as a Certified Peer Counselor is acceptable and preferred.
Apply for Agency Affiliated Counselor credential with Washington Department of Health within 30 days of hire, if not otherwise licensed, and obtain and maintain registration and/or licensure. For FHARPS program, credentialing as a Certified Peer Counselor is acceptable and preferred.
Successfully pass criminal background check before hire date.
Obtain/maintain valid First Aid/CPR card or certificate within 30 days of hire.
Hold a valid license for 1 year with clean driving record to use agency vehicles.

Able to navigate and comprehend charts in electronic medical record.
Able to work independently and within a team.
Able to communicate information and ideas so others will understand. Able to exchange accurate information in these situations.

Job Qualifications:
PREREQUISITES:
* Must have current Washington State Food Handler's permit.
* High school diploma or GED certification preferred.
* Able to communicate effectively in English, both verbally and in writing.
* Able to work on weekends and holidays.
* Able to meet the professional appearance standards of the organization.
* At least two year's cooking experience in quantity food production/short order cooking/grill cooking required.
* Must have an understanding of basic mathematics in order to follow recipe instruction, make changes, weigh and measure items, and complete daily production sheets.
* Must be knowledgeable about kitchen safety and sanitation practices.
* Previous experience with modified diet preparation is preferred.
* Able to read and understand a recipe.
* Able to prepare food in strict accordance to a recipe.
