Manage a caseload of a minimum of 10 participants in treatment
Required: 10 hours per week within the hours of 9a-8pm local time. The 10 hours may be completed during the week or on weekends. Candidate can choose the set schedule that works for them, but it needs to be consistent once chosen.
$40-$60 per session hour (dependent on state of residence)
Primary Responsibilities:
Deliver remote care throughout short-term, evidence-based programs
Monitor participant progress, provide follow-up communications and care coordination recommendations, and document session notes
Participate as a member of the care team and assist participants as needed with accessing resources, as well as obtaining consent and authorization forms
Identify and respond to high-risk participant encounters with elevated safety concerns, following company protocols
Participate in regularly scheduled meetings with supervisors, monthly team meetings, and Town Halls
Maintain the highest standards of confidentiality as described in HIPAA Privacy & Security policies, and ensure compliance with annual training requirements

Required Qualifications:
One or more of the following active, unrestricted independent clinical licenses in the state of Oregon: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Mental Health Counselor (LMHC), or state equivalent
Ability to maintain a consistent schedule for patients 10+ hours/week
Currently eligible to be enrolled as a provider in the Medicare program or able to terminate opt-out (if applicable) prior to starting contract
Preferred Qualifications:
Clinical experience conducting psychotherapy utilizing Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and other solution-focused approaches
Interest in obtaining licensure in other states

The hourly range for this role is $28.61 to $56.06 per hour based on full-time employment. The overall purpose of the Licensed Behavioral Health Clinician / Care Advocate is to improve the enrollee's ability to remain stable in the community and out of the hospital. Care Advocates engage people in the treatment process and assist them to access the appropriate community services so they can pursue their treatment goals in the community and avoid institutionalization. This position is also designed to be a direct clinical liaison between health plan staff and the clinical network. The program fosters a collegial and mutually beneficial relationship between the providers and payers of medical and behavioral health services.
Location: Syracuse, NY - 80% field-based meeting with Medicaid enrollees at hospitals, clinical facilities, community sites, and in their homes. 20% is spent working remotely at home.
Schedule: Monday - Friday, 8 am to 5 pm, no weekends, evenings, or Holidays
Patient Population: Adult Behavioral Health
If you are located in Syracuse, NY, you will have the flexibility to work remotely*, as well as work in the office as you take on some tough challenges.
Primary Responsibilities:
Identify and provide community service linkage
Assist enrollees with aftercare appointments
Provide subsequent member follow-up as determined by individual member needs
Work with enrollees to identify gaps in care or obstacles to care and problem solve for successful connection to needed services
Review IP Census each day to determine which enrollees on the census are re-admits within the last 12 months
Go to facility meetings with treatment team to develop a successful aftercare plan
Manage the aftercare follow-up of identified high risk enrollees (manage follow-up calls/outreach and documentation)
Meet with community providers/facilities as company liaison to foster good rapport and relationships and to identify new programs

The overall purpose of the Licensed Behavioral Health Clinician / Care Advocate is to improve the enrollee's ability to remain stable in the community and out of the hospital. Patient Population: Adult Behavioral Health
Primary Responsibilities:
Identify and provide community service linkage
Assist enrollees with aftercare appointments
Provide subsequent member follow-up as determined by individual member needs
Work with enrollees to identify gaps in care or obstacles to care and problem solve for successful connection to needed services
Review IP Census each day to determine which enrollees on the census are re-admits within the last 12 months
Go to facility meetings with treatment team to develop a successful aftercare plan
Manage the aftercare follow-up of identified high risk enrollees (manage follow-up calls/outreach and documentation)
Meet with community providers/facilities as company liaison to foster good rapport and relationships and to identify new programs

Responsibilities: Enrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs. The goal is to provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. Services are provided in accordance with the National Association of Social Workers (NASW) Code of Ethics and compliance with federal, state, and local guidelines and regulations, company policies and procedures, and standards of care. Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being. This includes educating staff on the role of social services. Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs. Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life. Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers. Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population. Qualifications: Bachelor's degree in social work or human services field required. Must possess any certifications/licensures as required by State of employment to practice in long-term care. ESI is seeking a Certified K-8 School Guidance Counselor who performs administrative work in the academic operational area of the organization and develops a comprehensive counseling program.
QUALIFICATIONS:
Master's Degree from accredited college or university in Guidance and Counseling or School Counseling required.
Possession of a Florida Educator Certification in School Counseling K-12 or Statement of Status of Eligibility showing eligible status required.
ESE and Gifted endorsements preferred.
Experience in a public-school setting or counseling environment preferred.
Excellent written and verbal communication skills.
Ability to read, write, speak, and understand the English language fluently.
Ability to walk, stand, stoop, bend, climb, crawl, push a cart, or lift during the entire shift.
Ability to lift up to 25 pounds, including small packages and supplies.
Ability to pass Level 2 background check.
DUTIES AND RESPONSIBILITIES:
Develops and implements a comprehensive counseling program that aligns with state guidelines.
Plans and implements the counseling program to address the academic, career, and personal/social needs of every student.
Coordinates the delivery of an annual needs assessment for the counseling program.
Delivers the four program components: the counseling curriculum, individual student planning, responsive services, and system support.
Provides short-term individual and group counseling opportunities for students to promote school success, personal growth, self-understanding, communication, problem solving and decision-making skills.
Provide guidance on course selection, academic policies, and graduation requirements.
Assist students in identifying and overcoming academic challenges, such as time management, study skills, and test anxiety.
Collaborate with faculty and staff to coordinate support services and resources for students.
Conduct workshops and seminars on topics related to academic success, career planning, and personal development.
Maintain accurate records of student interactions and progress towards academic goals.
Stay informed about educational trends, academic programs, and career opportunities to better support student needs.
Participate in professional development activities to enhance counseling skills and knowledge.
