Child Welfare Case Manager Credentialed Locations: Daytona Beach, Deland, Palm Coast and Palatka Salary Range: $49,500.00+ DOE Employment Type: Full Time Department: Case Management / Operations To identify and assess child and family needs and evaluate, coordinate and ensure necessary services are provided; provide in-home services to clients; assist and counsel individuals and families by using such activities as delineating alternatives, helping to articulate goals and providing needed information. Principle Duties and Responsibilities:
Provide case management services (information, plan referrals and linkages).
Develop and maintain case records and service documentation according to contract and Community Partnership for Children standards (family assessments, case plans, chrons, JRSSR's, etc.).
Consistently document case activities in FSFN within 24 hours of the event.
Complete all required court reports and submit within established deadlines.
Testify in court as necessary.
Monitor quality of care by conducting face-to-face home/residential visits of the child every 30 days.
Coordinate services with other professionals and paraprofessionals.
Conduct community outreach as needed.
Provide and/or arrange for transportation of clients to appointments, community agencies, etc., as needed.
Conduct monthly face-to-face visits with parents of children in Out of Home Care (OHC) to discuss case plan progress.
Provide and/or refer families to services, understand and use information from service providers, psychological reports and psychosocial evaluations.
Conduct child safety assessments.
Serve as on-call case manager after hours as assigned and determine and respond to immediate risk and needs of child.
Arrange for emergency placement for children at risk.
Explain rights and responsibilities to children and family members.
Provide/coordinate support to foster homes and relative/non-relative placements.
Provide post placement planning and services.
Perform needed interpretative duties to assist the family with obtaining needed services and assist them with understanding the tasks that they must complete to reunify their family.
Expand knowledge base of service providers in the community that can serve the needs of the special population client.
Participate in the quality improvement process.
Assist the Case Manager Supervisor by serving as a leader for the unit in the Level 4's absence

Finance Manager Salary Range: $87,500.00+DOE FLSA Status: Exempt Employment Type: Full Time Department: Accounting Supervises the Accounting Department staff, including the calculating, posting, verification, and record storage of financial data for use in maintaining accounting, statistical and master trust records at Community Partnership for Children (CPC). Supports the Chief of Business Operations & Finance in the financial analysis of operations and programs.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Oversees the daily operations of the CPC Accounting Department and provides leadership for daily accounting operations and staff.
Reviews and approves coding of standard and recurring invoices and deposits. Codes and analyses non-standard AP/AR/GL entries.
Monitors payroll entries to include mapping of company payroll general ledger interface.
Reconciles GL accounts and submits proposed adjusting entries to the Chief of Business Operations and Finance for approval.
Reviews monthly Master Trust account balances, ensures compliance with Social Security regulations.
Prepares monthly FSFN OCA reconciliation for approval by the Chief of Business Operations and Business Finance.
Identifies positive and negative financial trends and makes recommendations to the Chief of Business Operations and Finance accordingly.
Monitors grant expenditures; provides required reports.
In coordination with the Chief of Business Operations and Finance, ensures agency operations are in accordance with established financial policies and standard operating procedures to remain compliant with Uniform Grant Guidance 2 CFR Part 200, including specific requirements associated with the array of funding streams that encompass the Agency's funding mix.
Oversees the review and validate IV-E budget templates for contracted services in accordance with Uniform Grant Guidance 2 CFR part 200.
Maintains internal control procedures to ensure compliance with sound accounting practices and DCF regulations.
Participates in quality improvement process as required.
Participates in the preparation of organizational budgets and assists management in developing proper expense allocations for funding purposes.
Provides financial analysis support to the Chief of Business Operations and Finance and other senior management, including policy interpretation and financial analysis.
Regularly reviews and tests financial records for irregularities, conducts due diligence assessments as needed under the guidance of the Chief of Business Operations and Finance.
Coordinates and participates in all fiscal monitoring/audits performed by DCF and contracted audit firms.
Prepare and update the Cost Allocation Plan.
Participate in agency-wide strategic planning.

Post Adoption Specialist Salary: $49,500.00 FLSA Status: Salary non-Exempt Employment Type: Full Time Department: Case Management Operations This position is responsible for assisting adoptive families with information and support services. This person will provide case management crisis intervention, advocate through interagency programs, hold monthly support groups, seek education training for families and providers to become adoption competent, provide yearly follow up calls with adoptive families, and assess and complete subsidy increase requests. Additionally, this person will process out-going ICAMA requests for post-adoptive families moving out of state.
PRINCIPAL DUTIES AND RESPONSIBILITIES:

QUALIFICATIONS:
Education/Experience

JOB SUMMARY:
This is highly responsible managerial work providing supervision and direction to a team of Family Care Managers (FCM) and one Family Support Worker (FSW) working with children and their families with severe and persistent mental illnesses and/or substance abuse. This position also provides oversight for quality assurance, risk management, financial management and clinical direction of unit. ESSENTIAL FUNCTIONS:
Oversee and track critical data elements involved with management report/State Scorecard via Kids Central Inc. electronic case management system.
Oversee provision of case management services, planning and linking, within 5 days of Early Service Intervention staffing, monitoring, and advocacy for assigned children and families from intake through discharge from the system within assigned care team unit.
Monitor and manage all Family Care Team reports and outcomes, including monthly visitation, judicial progress and case status.
Ensure FCM is meeting Kids Central Inc. contractual obligations provided to each supervisor annually as it is renewed. Should a FCM fail to meet any contractual obligations, the supervisor will ensure the FCM is compliant with any action needed to bring the FCM into compliance with the contract.
Ensures follow up by FCM on all case reviews and recommendations from Supervisor, Quality and Permanency Reviews.
Ensure that every child in foster care from their unit is referred for a relative search and if deemed necessary, ensures that Family Finding strategies are integrated into case management functions.
Monitor FCM during home visits and court hearings at least once every two months. Should the supervisor assess the FCM to need additional training in any area, the field visits will be increased to allow for mentoring by the supervisor or referral to the training unit.
Participate in staff development programs and opportunities. Participate in team meetings, client care and utilization reviews and staff meetings.
Provide administration and management duties related to staff supervision including interviewing and selection, training and orientation, supervision, performance management and development and scheduling.
Establish and maintain effective working relationships required for proper functioning within the role of Family Care Manager Supervisor.
Supervise day to day operations of Family Support Worker.
QUALIFICATIONS:
Education/Experience
Bachelor's degree in Social Work or related field. If degree is in an unrelated field, must be eligible for a waiver. Master's degree preferred.
Bachelor's degree from an accredited college of university in social work, psychology, or other behavioral healthcare discipline and five years of experience working with children and families, one of which is in providing supervision to direct service workers in related field may be considered in lieu of Master's degree.
Three years of supervisory experience in current program, DCF or YFA transfer to be considered eligible for FCMS II.
Licensure/Certifications
Child Welfare Certification required. Must maintain certification in good standings.
Must maintain a clear criminal background and driving record.
Must maintain and show proof of the appropriate automobile insurance limits ($100,000/$300,000) per Florida Statutes.
