Emphasis is placed on coordinating and rendering therapeutic, supportive and restorative measures, through the use of music, to meet the needs of patients and their families; promoting the quality of life for terminally ill patients and their families and participation of the development of the hospice interdisciplinary team care plan. Participates in treatment planning, ongoing evaluation and follow-up. Re-evaluates and documents the patient's response to music therapy. Based on patient response does appropriate modifications to the Care Plan in a timely manner. Recognizes changes in the patient's physical and mental status and informs health care professionals within a reasonable timeframe. Identifies own learning needs and seeks direction/approval from supervisor to meet those needs as documented on the goal section of the performance appraisal. Educates staff about the role of the Music Therapist and the use of music therapy interventions with hospice patients and families. The Assistant Project Manager performs essential project management tasks working in conjunction with the Project Manager to support the overall direction and completion of a project. General Responsibilities including but not limited to: • Provide leadership in supporting the overall direction and completion of a project. • Establish effective working relationships with project team members including the owner, design team, subcontractors, and vendors as necessary to facilitate the successful completion of the project. • Assist the Project Manager during Preconstruction including bidders list and bid package development. This will include but may not be limited to all activities required for solicitation, monitoring of bidder's participation, receipt of complete bids, scope review and contract award. • Establish project specific procedures and lists with the Project Manager as may be necessary for the successful completion of the project in accordance with Company standards. • Expedite, receive, review, and distribute all project submittals in accordance with the Specifications following standards. • Ensure that all the following are properly received, logged, and maintained throughout the life of the project. o Certified payrolls as applicable o Insurance Certificates in accordance with requirements and the project specifications o Performance and Payment Bonds in accordance with requirements and the project specifications o Other project specific documentation such as EEO and S/M/WBE participation requirements o Requests for Information (RFIs) o Architect and Engineer Sketches (SKs) o Architects Supplemental Instructions (ASI) o Drawing logs • Periodically update the drawings and specifications with SKs and other revisions received from the design team. • Issue Change Order Proposals to subcontractors and suppliers as required including any information received from the design team such as SKs, ASIs etc. • Expedite and review Change Order Proposal costs from subcontractors and maintain a detailed COP log-in sheet in conjunction with the Project Manager. Update Project Management software with COP cost information periodically. • Attend, record, and distribute meeting minutes for all project meetings. Masonicare Home Health & Hospice - East Lyme, CT area Day Shift / 40hrs/wk The Hospice Palliative Care LPN is responsible for and provides skilled nursing care reflective of the agency's mission to the ill or injured in the home, hospital or extended care facility. The LPN works under the direction of the Nurse Case Manager and/or Clinical Manager. Essential Duties and Responsibilities: Responsible for coordination of care with community and agency resources and with the designated registered nurse on a regular basis. Provides nursing care as per the established plan of care and documents clinical activities per the agency policy Recognizes and interprets symptoms and institutes remedial measures within the scope/limits of a Licensed Practical Nurse in the state of CT and reports significant changes in the patient condition to the primary RN Case Manager/Clinical Manager immediately. Administers medication and treatments as ordered by the physician, with the exception of IV push medication. LPN must pass competency for intravenous/subcutaneous administration of medication via continuous infusion.
